Issue:
Based on record review and interview, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the Facility Unusual Occurrence Report dated 4/29/13, the report indicated an unknown staff person placed the arm band of Patient 1 on the wrong emergency room patient. The information on the band contained the following: Patient name, sex, date of birth, and medical record number.During an interview with the Privacy Officer on 6/11/13 at 11:30 AM, she stated the breach had been investigated, but they were unable to determine who made the error.All staff in the department were reeducated to ensure the error does not reoccur.
