Issue:

Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 5/13/13, the report indicated Admitting Clerk A (AC A) was verifying insurance, and she failed to place an "insurance alert" on Patient 1's account. This error resulted in Patient 1's information being sent to an insurance company that he was no longer insured with. The company received the following information: Account number, Patient 1's name, age, date of birth, and health information.AC A was provided a memo and re-educated. All staff was re-educated regarding placing insurance alerts.
