Issue:

Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 5/17/13, the report indicated that while registering at the facility a patient presented aftercare instructions that had the Name of Patient A on them. The facility's investigation revealed the error was made by Licensed Vocational Nurse A (LVN A). The aftercare instructions contained the following: patient name, date of birth, date of service, and medical record number. LVN A received additional counseling and patient privacy training.
