Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 5/17/13, the report indicated Staff A failed to double check patient identifiers prior to handing out an appointment print out. This error resulted in Patient 1's private information being given to the wrong patient. The print out contained the following information: Patient name, address, date of birth, sex, and guarantor number.Staff A received re-education regarding proper patient identification and the importance of double checking all documents prior to handing them out.
