Issue:
Based on interview and record review, the facility failed to ensure Patient A received a copy of her transition record (documented pertinent highlights of the patient's hospitalization) when discharged from the Emergency Room (ER). This failure could result in confusion surrounding the patients future medical needs.Findings:An interview was conducted with the facility's Compliance Specialist (CS) on October 16, 2012, at 10:15 a.m. The CS stated Patient A's transition record was placed in an envelope and was given to Patient B when the patient was discharged from the ER.A review of Patient A's transition record was conducted. Protected health information reflected in the document included the patients name, date of birth, date of treatment, medical record number, chief complaint, diagnosis, medication administered, radiology exams completed, in addition to all laboratory testing done.An interview was conducted with the ER Director (ERD) on November 27, 2012, at 11 a.m., who stated it is the facilities process to ensure each patient discharged from the ER receive a copy of their transition record. The ERD stated she was unable to determine if Patient A received her transition record as the patient's guardian was unable to locate the document.
