Issue:

Based on interview and record review, the facility failed to ensure Patient B received a copy of her written discharge instructions when the patient was discharged from the facility. This failure could result in inadequate medical follow up post discharge and may impact the patient's medical stability.Findings:An interview was conducted with the facility's Compliance Specialist (CS) on October 16, 2012, at 11 a.m. The CS stated a discharge planning coordinator (with the case management department) gave Patient A's discharge information to Patient B and the envelope was sealed shut when Patient B received the discharge information.A review of Patient B's record indicated the patient was admitted to the facility on September 3, 2012, and discharged on September 6, 2012, with diagnoses of anemia, alcohol withdrawal and inflammation of the stomach. Patient B signed a document at the time of discharge on September 6, 2012, at 10:20 a.m. which indicated she received the written discharge instructions. The CS further stated on October 16, 2012, at 11:15 a.m., when Patient B arrived home after having been discharged she realized she had received another patient's discharge information.A review of the dictated physician's discharge summary report dated September 6, 2012, regarding Patient B indicated a follow-up examination for a repeat EGD, (test to examine the lining of the esophagus, stomach and upper intestine), in addition to a referral to Alcoholics Anonymous were recommended. Two medications, Nexium (to treat stomach ulcers and heartburn) and Thiamine (Vitamin B-1) were also recommended. An interview was conducted with the Case Management Supervisor on November 27, 2012, at 10:30 a.m., who stated she had reviewed Patient B's record, and there was no documentation to indicate Patient B received her written discharge instructions after she returned Patient A's discharge instructions to the facility.
