Project Description

Construction of a 20 bed District Hospital

Project Goals

Provide hospital level healthcare to people from Shibar, Sayghan, Tala Wa Barfak, and Kahmard Districts. This facility will endear the GIRoA to the population, exercising the principles of COIN. This should result in increased security related information, and turn them away from INS influence in the AO. A new hospital in this area shows the population that the GIRoA is providing for their medical needs and future. This hospital improves the quality of life of the population by providing a solution to their health care needs. The human capacity of the region will greatly increase as the next generation recieves care that will improve their quality and length of life.

Jul 10 2010 Still waiting on SOW and IGCE Jul 21 2010 Awaiting IGCE from USACE. New Zealand Dr has LUA and Sustainment memo for signature by Director of Public Health. That meeting is to occur this week.

Sep 18 2010 At Division. Told was approved and funded and out for Contracting? Have not heard anything else. Have not received new revised SOW and IGCE that Div Engineers supposedly produced to reflect changes they made to our SOW. Have not received funded PR&C or Contract.

Oct 6 2010 Awaiting approval from BDE and Division. SFC A 

Oct 31 2010 Uploaded signed CERP Contract with Folad Bamica CC. Contractor stated will have equipment mobilized to work site NLT 10 Nov 10. Groundbreaking TBA.

Nov 16 2010 Contractor coordinating with Provincial Government to conduct a groundbreaking. Contractor also submitted final plans which have been approved by USACE. SFC A 

Dec 6 2010 Contractor has begun excavation. He has run into bedrock and will be back to give an estimate on a new lenth of time and cost to level the ground.

25 Jan 2011, SFC Ramos, Joseph took over this project from SFC Muncy. I have a meeting with the contractor today and he informed me that all they have done so far is eartworks/excavation. They put the first payment which is being process at this time. Contractor also drop his stop work notice dated 26 Jan 2011. The contractor also requested an addional $79,214.00USD because of the hard rock that they excavated on the project instead of a soft ground. The modification of this amount from $475,000.00USD(original contract amount) to $554,212.00(increased amount) was forwarded to the BAF Contracting Office, SFC Pablo Ravizolli for review/approval.

16 Mar 2011: SFC William Hutt took over project from SFC Ramos. After reviewing packet, the ratification for funds is still with SFC Ravizzolli, contacted Ravizzolli via e-mail, the paperwork is in the process. Also updated PM memos for appointment of COR.

23 Mar 2011: updated the project details and justification.

2 may 2011: update photos of site construction. contractor requested 25% pyment. denied payment due to lack of work conducted at this time. wmh

14 May 2011: upload photos and montly update report. no issues. wmh

2 Jun 2011: site visit conducted 20 May. Having contractor wait to start on brick work until his masons recieve a class at Daste Safed with the USACE Engineer. Upload montly update slide and photos. wmh

18 NOVEMBER 2011: Contractor has requested final inspection. COR requested pictures. Inspection planned for 20 Nov 11.

25 NOVEMBER 2011: QA/QC Inspection conducted on 21 Nov 2011. 

The quality of work was good; however some issues/deficiencies were identified during this inspection. As per the SOW Section 3.4.3 all outer doorways shall be made of aluminum. All exterior doorways where wooden frames have been installed will have to be replaced with the required aluminum type.

As per the SOW Section 3.4.1 all exterior windows shall be made of PVC. All exterior window locations where wooden type windows have been installed will have to be replaced with the required PVC type.

Recommendations: If required schedule a meeting with the contractor to address the deficiencies that were identified during the project inspection on 21 Nov 2011. 

Follow-on Actions: Continue to monitor and perform a follow up site visit in the next few weeks weather permitting.

10 December 2011- Submit CERP nomintions in CENTRIX.

09 Jan12: Final Inspection conducted by Project PM 1LT Leopold Odah and USACE Engineer Mr. Barry Brockway. Outside and inside walls plastered. Windows and doors installation in progress. Start tiling the floor. Work currently ceased because original cost estimate was too low. 75% o work completed.

02 MAR 2012: Work halted due to finance issues. Project IGCE is far lower than the lowest bidding contractor. Working with BDE S-9 for additional funds in order to complete the project, what seems impossible. 

02 April 2012: Found original contractor bids. One was for $475,000, one was for $2 mil, and the rest were all right around $1 mil. Apparently this did not raise any flags and they started the project when it should have been reevaluated. As there are no additional funds for this province to finish the project, we are teminating this project and the locals will have a half-complete hospital that which the MoPH never planned to equip or staff regardless. The PM will begin termination paperwork once he returns from R&R. -KMG

14 MAY: Project on hold due to payment issue. Waiting for close of Panjab Hospital before terminating. SM.
