Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The inspector entered the Windjammer around 11:15 a.m. and observed passengers at the starboard side buffet service line taking food items. There was a tray of cut tomatoes, which wasn't discarded from the line until 11:20 a.m. The time control plan stated the breakfast service time at the St. Thomas port was from 0700 to 1100.

Two food employees wearing beard restraints did not wear the restraints appropriately, and the restraints did not cover their mustaches. One food employee with a mustache was not wearing a restraint. A similar violation was noted on the last inspection.

Galley-
Food employees were placing inaccurate 4-hour discard labels on potentially hazardous food items on food preparation counters. Many of the time control labels stated a discard time beyond four hours. Additionally, staff were not following the time control plan for the Windjammer breakfast buffet service on the starboard line.
Medical-Late Reporting
During the 1-8 March voyage, a nonfood worker reported gastrointestinal illness (GI) onset on 2 March at 0500 and reported to medical at 1323. The patient remained in his room during this time. During the 15-22 February voyage, a food worker (pastry) reported GI onset on 18 Feb at 1030 and reported to medical 1729. The crew member did not work, and was reported to have remained in his room during this time. Both crew members were counseled.

The record review of the Solarium Pool showed the halogen levels for this interactive recreational water facility (RWF) were consistently above 5.0 ppm. This facility had spray features and was halogenated to levels of 3.0-10.0 ppm. Most of the readings were between 4.0 and 8.0. On 11 March, staff received instructions from the cruise line corporate office to halogenate the facility in the range of 2.0-5.0, same as for all other interactive RWFs on the ship. Halogenation at this range started on 12 March. On the day of inspection, this interactive RWF had halogen levels of 5.0 ppm when tested by the inspector.

Two water dispensers were located on the consumer counter of the bar for passenger self-service. Although a tray with cups was next to the dispensers, the inspector observed a passenger refilling a personal water bottle touching the lip-contact surface of his bottle to the dispenser spout. Both bartenders working on the other side of the bar did not notice the issue. No sign was posted to direct consumers to use a clean cup or state refills were not allowed.

Two live ants and a fruit fly were observed on the deck near the self-service candy dispensers.

A portable fan was stored under a shelving unit. The fan was removed from the room.

In the hot galley, the temperature of the handwash station was 129°F. The user could not adjust the temperature. Staff stated that at a work order was placed at noon on the day of the inspection because no cold water was available.

The cooling logs were not completed for two potentially hazardous food items. On 12 March, temperatures for a soufflé were 160°F at 12:07, and 67°F at 14:01. No further temperatures were documented. The preparer explained the food item was removed and immediately served. On 16 March, temperatures for Spanish vanilla lollipops were 168°F at 17:21, and 67°F at 19:07. No further temperatures were documented.

Outside the pastry, about four trolleys were full of previously cleaned plates and bowls. Approximately 50 bowls were still wet on the food contact area.

At the entrance to the bakery, small pieces of dough were in the open handwash station waste receptacle.

On the inside of the white plastic deflector panel, silicone from the ice machine manufacturer covered two small holes. It could not be verified this was a food-grade silicone.

Plate covers were observed coming out of the dishwash machine turned on their sides. This prohibited proper washing and sanitizing of all areas of the plate covers.

Inside the dry storage cabinet, adjacent to the starboard ice machine, the front area of a plastic tray was wet. Dry food items were stored on this shelf.

The cooling log was not completed for a potentially hazardous food item. On 14 March, temperatures for a preseared raw curry chicken were 67°F at 09:27, and no temperature was documented for the written time of 12:37. It was not documented that the chicken was cooled to 41°F or less. The preparer explained the chicken was fully cooked and immediately served.

When the inspection team arrived in the area, the crew member working on the clean side of the potwash three-compartment sink was observed on the soiled side alongside the pile of soiled items. He then rinsed his gloves from the faucet of the wash compartment and moved to the sanitizing compartment. The inspector questioned him on his recent actions. He was advised by supervisors to wash his hands and sanitize his gloves.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)
The OPRP had been undergoing revisions with the newest release around the end of October. Some items in the plan must be updated to comply with the VSP Operations Manual section 9.0. For example: the plan described continuous disinfection begins when the ship reaches reportable acute gastroenteritis (AGE) cases of 1.5% or above among all onboard (passengers and crew members), as opposed to separating the response threshold between passengers or crew members; and there were no procedures for informing embarking crew members following an outbreak or those crew members onboard during the outbreak.

Potable Water-Bunkering
The analyzer at the bunker dosing station was found to be out of calibration for halogen levels as compared by manual tests done by the inspector. Staff was using the values from the analyzer for the bunkering halogenation records. Between 08:25 and 00:33, the analyzer had free halogen levels of 2.23, 2.24, and 2.30 ppm, while the manual tests done by the inspector were 3.56, 2.80, and 3.03 ppm. At 08:40, the review of the bunkering log in the engine control room (ECR) disclosed that bunkering started at 07:19 and also had a recorded halogen level of 2.30 ppm for 08:30. Staff stated that the halogen and pH levels recorded on the log were those they could see in the ECR's screens from the readings of the analyzer in the bunker dosing station.
Potable Water-Evaporator #1
There was no air gap for the reduced pressure (RP) assembly. A funnel was installed under the RP and the RP's air relief vent was below the funnel's flood-level rim.
Potable Water-Tank Maintenance Records
Potable water tank maintenance records showed that the tanks were cleaned and sanitized with 200 ppm of chlorine solution for not less than 2 hours. The records also had the following statement: 'All tank surfaces remained wet with the chlorine solution for more than 2 hours'. Staff stated that although they made that statement, the chlorine concentration on the tank surfaces was not tested during the two hours.
Recreational Water Facilities-Solarium Spa Pool Drain Cover
Previous inspection conducted in May 2014 found that the drain cover was approved for multiple drain use only but was installed on a single drain. As a result, the ship requested and received a letter from a licensed engineer stating that the drain cover complied with the 2011 VSP Operations Manual and with the ASME/ANSI A112.19.8-2007. However, the letter did not address the fact that the cover was installed in a single drain and not on a multiple drain set-up as certified by the drain cover manufacturer.

The overflow grating on the ship's side next to the windows was broken and missing in several areas. Staff stated that there was a work order to correct the grating.

The H2O Zone had a children's pool and a shared interactive play area with jets and sprays. The turnover rate for the combined system was 1 hour and not 30 minutes. The inspector explained the staff that for facilities that meet the definition of more than one type of RWF, the more protective turnover rate applies. Specifically, this facility was an interactive RWF and a children's RWF. Halogen residuals were correctly maintained above 2.0 ppm, as required for interactive RWFs.

The upper part of the water dispensing nozzle on the water/ice machine was soiled with a black material. A black gasket was in this area, but it could not be determined if the soil was from deterioration of the gasket or mold growth.

Several open bottles of wine were stored in these refrigerators. There was no coving at the deck/refrigerator cabinet juncture. There was also soft sealant on the deck in front of the refrigerator cabinets.

Three trays of turkey panini sandwiches labeled with a preparation date of 8/28 and a discard date of 9/3 were in the walk-in refrigerator. These sandwiches could not be located on the cooling logs for 8/28, but turkey panini sandwiches were on the cooling log for 8/26. It could not be determined if the sandwiches were labeled with the incorrect preparation and discard dates, or if the sandwiches were not entered into the cooling log.

The top, right removable orange plastic piece on the orange juicer machine had a gouge that created a difficult to clean area. The piece was removed to be replaced.

A plastic bin with several smaller plastic containers of beverage garnishes such as pineapple, oranges, cherries, and limes was on the counter next to the soda dispensing machine. The container did not have a lid. The bar was not open yet and no one was in the pantry at the time of the inspection. A lid was placed on the garnishes.

A Cleaner experienced acute gastroenteritis symptoms on 9 Aug at 08:00, but did not report to the medical center until 17:10. She did not work during this time. She received a written warning for late reporting.
Housekeeping-Ice and Detergent Stations - Chemical Storage
Bulkhead-mounted Oxivir Five 16, glass cleaner, fabric odor eliminator, and acid bathroom cleaner dispensers were set up in 34 housekeeping ice pantries, along with open waste collection containers. The ice pantries included an ice machine and clean utensils. Staff stated only the Oxivir Five 16 dispensers were being moved to other lockers equipped with a water supply.

The Mojo dinner menu did not have a public health advisory for the 'smoked fish rillettes' dish. This dish had cold smoked salmon.

The ship's fecal and vomit accident plan stated the scoop for feces/vomit was to be cleaned and disinfected at 200 ppm for 1 minute. This residual and contact time did not meet the requirement per the procedure provided in Annex 13.8 for a loose stool.
Recreational Water Facilities-Antientrapment Drain
The documentation and stamp on the certified drain cover for the Solarium spa pool stated 'multiple drain use only' but it was used on a single drain. Also, it was unclear if the field fabricated retrofit and installation of the drain cover was conducted by a registered architect or engineer, as required by the manufacturer's instructions.

A bar employee was observed opening the spout of a new carton of drink mix with bare hands, contacting the interior of the pouring spout in the process. The same employee was observed closing the spout of each open carton with bare hand contact after pouring, and when re-opening positioned a thumb inside the spout to open.

The live plant vine which was wrapped and tied to the trellis type deckhead metal supports had some sections where the plant extended directly over the bar. On one end in particular the plant was hanging directly above an open container of several cut pieces of mixed fruits for garnishing drinks.

The recently revised time control plans stated operation hours, but it was not clear from the plans if the set-up times were included in the operation hours. This included plans in the galleys, buffets, and for the milk in the espresso machines.
Recreational Water Facilities-Safety Sign
The FLOWRIDER® recreational water facility did not have a sign that specified the minimum required information including: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; and (3) shower before entering the facility. Safety information only was posted. All of the other safety signs for RWFs had been updated since the previous inspection.
Housekeeping-Soiled Ice Bucket Tong Handling
Over a dozen soiled ice buckets, ice tongs and trays were placed directly on the deck outside the cabins as they were cleaned. This practice transfers deck soil to the areas where these items will be washed.
Galley-Deck 4 Galley Port Entrance
The water at the handwash station reached 133 °F/56 °C during use, and the user could not control the temperature. Staff corrected this during the inspection.
Preparation Room-
There were two pieces of raw chicken with internal temperatures of 49 °F during active thawing in the right side Tina upright reach-in thawing refrigerator. It was unclear why only these pieces were thawing out of temperature, as the other pieces in the same bag were at or below 41 °F. The high temperature pieces were immediately removed and discarded.
Galley-Hot Galley
A full hotel pan of brown gravy was found inside the combination oven with a food temperature of 95 °F. Other pans of potentially hazardous foods inside the same oven were at 135 °F. Staff stated the gravy had been reheated and transferred to the oven for holding.
Medical-Acute Gastroenteritis (AGE) Surveillance Log
Medical staff stated the electronic medical reporting system did not allow them to make a case reportable unless the case had 3 episodes of diarrhea within 24 hours or 1 episode of vomiting plus 1 other symptom. There was no way to make a case reportable if a patient had less than 3 episodes of diarrhea but this was above normal for the individual.
Medical-Immediate Contacts for AGE Ill Crew Members
Medical staff was not following up with and documenting significant others (boyfriend/girlfriend) as immediate contacts for sick crew. Only cabin mates were listed close contacts for followed-up. According to the staff, known contacts were not interviewed unless they were cabin mates.
Medical-AGE Surveillance Log
A passenger reported to medical on 7/15 with two episodes of diarrhea within 24 hours. This passenger was not a reportable case because the passenger was lactose intolerant. However, nothing was noted in the 'underlying illness' column on the AGE log. Another passenger reported on 7/15 with two episodes of diarrhea but stated this was normal for her during her menstrual cycle so she was not listed as a reportable case. Once again, nothing was noted in the 'underlying illness' column on the AGE log.
Medical-AGE Crew Reporting
A crew member on 7/7 experienced AGE symptoms but did not report to medical until over 8 hours from the time of illness onset. The crew member worked while symptomatic. The ship's staff had investigated this incident and disciplinary action was taken and documented. This was a repeat violation from the previous inspection.
Medical-4-hour Update Report
A 4-hour update report was submitted on 7/7 even though there were no additional AGE cases reported since the 24-hour report was submitted.

A crew member reported to medical on 7/9 with two episodes of diarrhea and was treated as a nonreportable AGE case. The crew member developed more symptoms later that day, which made them a reportable case. This case was not listed on the AGE log. Medical staff stated after the initial entry into the electronic medical reporting system as a nonreportable case they could go back and change it to a reportable case. This was noted in the previous inspection report.
Medical-AGE Definition
The medical staff did not know that the definition for reportable cases included 3 episodes of diarhhea or what was above normal for the individual. Since 23 June, seven individuals had reported to the medical center with less than 3 episodes of diarhhea and were issued antidiarhheal medication, but were considered nonreportable cases. The medical staff did not question these individuals if the diarhhea was above normal for them so it was unknown whether any of these seven individuals were actually reportable AGE cases.

The whirlpool safety signs did not include a caution against use by people with low blood pressure. The Solarium pool is considered a spa pool and the safety sign did not include whirlpool safety information. The Flow Rider safety signs did not include the information required for recreational water facilities. These are all repeat violations from the previous inspection. An email correspondence between the ship and the corporate office was observed which stated the new signs are on order.
Medical-Crew Acute Gastroenteritis (AGE) Reporting
On 12/6, a photographer had an onset of AGE symptoms at 3 pm. They went to the crew mess at 4:35 pm, clocked in on the electronic time clock system for work at 4:45 pm, and did not report to the medical facility until 6:20 pm. On 11/9, a dancer had an onset of AGE symptoms at 10 am and did not report to the medical facility until 11/13 at 4:10 pm. The dancer worked during 11/9 and 11/13. The ship's staff had investigated both of these late reporting's and disciplinary actions were taken and documented.
Medical-Acute Gastroenteritis (AGE) Case Reporting
On 11/23, an assistant waiter reported to the medical facility with 2 episodes of diarrhea and this was treated as a nonreportable AGE case. Three hours later, they had an additional episode of diarrhea, making them an AGE case. This case was not changed in the ship's electronic database to reportable and it was not included on the AGE report to VSP. The waiter was isolated for the required 48 hours.

Finally, the STARDOCS medical surveillance system only allowed for inputting the onset of AGE symptoms. Follow-up information, such as additional symptoms that could make someone a reportable case, could not be entered by staff except manually in the 'comments' section of the database.
Medical-Crew Isolation
During the cruise from July 22 to 29, 2012, a stateroom attendant who handles food was a reportable case and had an onset of AGE symptoms on Tuesday July 24, 2012 at 9:37 a.m., but was not classified as a food handler in the STARDOCS medical surveillance system and was not isolated for 48 hours. According to their time card, this person worked on July 25, 2012 from 5:22 p.m. to 10:07 p.m. and on July 26, 2012 from 7:43 a.m. to 12:55 p.m.
Medical-Close Contact Interviews
During the cruise from July 15 to 22, 2012, there was a retail worker with reportable AGE symptoms, but there was no documentation in the STARDOCS medical surveillance system stating this worker did not have any close contacts. Also, during the cruise from July 1 to 8, 2012, there was a waiter with reportable AGE symptoms on July 3, 2012, but there was no documentation in the STARDOCS medical surveillance system stating this worker did not have any close contacts.
Medical-Crew Reporting
During the cruise from July 15 to 22, 2012, one assistant waiter had an onset of AGE symptoms on July 17, 2012 at 9:34 a.m. According to their time card, this food worker clocked in at 4:18 p.m. and clocked out at 4:34 p.m. the same day. This individual did not report to medical until 4:45 p.m. on July 17, 2012. During the last inspection, there were two food workers and three nonfood workers that did not report to medical in a timely manner.
Medical-Knowledge
The medical staff in charge did not know the criteria for isolating food workers and nonfood workers. During the cruise from July 22 to 29, 2012, a stateroom attendant who handles food had an onset of AGE symptoms on Tuesday July 24, 2012 at 9:37 a.m., but was classified as a nonfood worker in the STARDOCS medical surveillance system by the medical staff and was isolated for 24 hours. Also, the medical staff did not understand when the AGE symptom onset should be entered and what symptoms should be entered under the illness onset area of STARDOCS. On July 2, 2012, a food worker was documented in STARDOCS as having 3 episodes of diarrhea and 6 episodes of vomiting at 10:00 a.m. and reported to medical at 5:18 p.m., but according to their medical chart, this worker reported to medical at 10:40 a.m. with severe stomach cramps but no other symptoms and reported to medical at 5:18 p.m.
Potable Water-Incubator
The digital thermometer for the incubator in the medical center used for microbiological samples indicated 24°C, but the documentation for the media used stated the samples needed to be incubated between 34.5°C and 35.5°C. According to the staff, the thermometer had not worked since April 2012 and another thermometer was used to manually check the temperature of the incubator throughout the day. A manual check of the incubator was done during the inspection and the thermometer reading was 38°C. There were at least five water samples in the incubator at the time of the inspection.
Potable Water-Medical Wards 1 and 3
Two reduced pressure assemblies were installed in March 2012 in medical wards 1 and 3, but there was no documentation of the test results after the devices were installed.
Housekeeping-Passenger Public Restroom Handwashing Signs
In the male and female public restrooms on deck 8 aft, including the handicapped stall in the women's restroom, the women's handicapped restroom on deck 6 aft, and the men's deck 8 forward restroom, there were no signs at the handwashing stations or on the adjacent bulkheads advising users to wash their hands. Several of these signs were added at the correct locations during the inspection. During the last inspection, several public restrooms were found to not have signs advising users to wash their hands at the handwashing stations.
Food Service General-Lighting
The light intensity was less than 110 lux behind and around a few pieces of deck- and counter-mounted equipment. This was observed at the aft starboard Windjammer Thirst Quencher 2 between the ice machine and the left bulkhead, and in the Solarium Galley behind the juice machines.

The chilled water lines for the ice machines and the brine lines for the upright refrigerators in the food preparation areas were not uniquely identified.

According to the staff, the racks of clean glasses were supposed to be stored in the bar equipment storage locker one deck below and had probably been in the corridor since the previous evening.

VSP will review the single, unblockable gravity drain cover installed in the center of the solarium pool to decide if it meets section 6.7.1.2.2 of the VSP Operations Manual 2011 as designed.

According to her supervisor, she worked for 30 minutes that day prior to the onset of symptoms. An Acrobat had an onset of AGE symptoms on 11 January at 04:30, but did not report to the medical center until 16:53. According to his time card, he worked from 11:21 to 13:23, as well as eating at the Garden Cafe at 12:00.

The vessel was using the Natura Water system for still and sparkling water. The water bottles were very tall and had a restricted opening. Because of these features, it was unclear if a regular warewashing machine will sufficiently wash, rinse and sanitize these bottles. The vessel had a standard operating procedure for cleaning the bottles and caps, but deep cleaning (more than warewashing machine) was only conducted once every 7 days.
Medical-Food Employees Reporting
During the 3-10 July 2011 cruise, two food employees had onsets of reportable gastrointestinal illness (GI) symptoms and worked while symptomatic before reporting to the medical center. A waiter had an onset of GI symptoms on 3 July at 14:00, but did not report to the medical center until 4 July at 08:39. An assistant waiter had an onset of GI symptoms on 4 July at 07:30, but did not report to the medical center until 4 July at 17:00. The time cards for these food employees were reviewed to verify work history on these dates.

The program only listed 2 backflow prevention devices for each passenger cabin (shower and toilet). The suite cabins had additional backflow prevention devices on the hoses on the whirlpool tubs and/or in more than one bathroom. These additional backflow devices were not indicated in the program.
Potable Water-Deck 12 Forward and Deck 14 Aft Fan Rooms
There were no backflow prevention devices on the potable water lines to the hose connections at the sinks by the far point stations in the deck 14 aft and deck 12 forward fan rooms.
Potable Water-Multi-Flow Systems
There was no backflow prevention device for non-carbonated potable water connection to the multi-flow spray heads in the bars. This was noted during the construction inspection and was scheduled for installation in January. Also, only "914" was in the column for the type of devices for the backflow prevention device on the carbonators.

In the following locations, the backflow prevention devices installed on the potable water lines were correct, but did not match the list of devices in the cross-connection control program:1. main laundry tween deck and deck 02. spa hair sinks3. engine changing room washing machines.

It was unclear if Oxivir can be used on toys and in children activity centers because the instructions for Oxivir wipes indicate that the product should not be used near children. Additionally, the bottle of Oxivir in the activity center was not labeled..

Bar-Cupcake Cupboard Variance
The vessel had an activity where passengers frosted their own cupcakes in the Cupcake Cupboard. The corporate office has applied for a variance for this activity and it is under review by VSP. There is a handwashing station for passengers and activity participants are advised to wash their hands before beginning the activity. Before the activity begins, the bar manager checked to ensure that no participants were listed on the gastrointestinal illness surveillance log. However, passengers were not asked if they have had any symptoms of gastrointestinal illness within the last 48 hours. This will be a provision in the variance. Passengers are provided with individual pastry bags with frostings and individual cups with toppings and are not supposed to share their items. All unused frosting and toppings were discarded after the activity. Passengers were not required to eat the cupcakes before the end of the activity and were allowed to take the cupcakes with them. Under the variance, passengers will be advised that they must eat their cupcakes within four hours.

Galley-Deck 4 and Deck 5
The posted time only as a public health control plan for the hot foods in the deck 4 and 5 galleys stated that time control begins only when the food is removed from a hot holding unit. The bain marie units used to hold hot foods in the main galleys were not listed on this plan as being on time control, but staff stated that all of the bain marie units were on time control. The cold food plan in deck 5 galley stated that time control begins when cold foods are removed from their cold holding unit. There was a cold basin for soup which was not listed as a time control unit on the plan.

There were two large holes in the bulkhead behind each of the two self-service wine dispensing refrigerators. Additionally, each of the multiple wine bottle dispensers was elevated on legs only 12.5 mm (1/2 inch) above the shelf, making cleaning very difficult.

The shelf beneath each of the two multiple wine bottle refrigerated dispensers was heavily soiled with dust.
Galley-Deck 3 Roast Station
The cooling of some of the cooked foods stored inside the roast station walk-in refrigerator #4963 could not be verified. It appears staff working at night wrote in the log for foods such as cooked fillet of beef the time and date placed in the blast chiller unit, such as 25 December, but the label fixed to the pan of food is actually 26 December, which in a number of cases is when the food is finished cooling. In the case of several sheet pans of cooked prime rib in one trolley there were two stickers affixed to the same trolley, each with a different date of 25 and 26 December. Since the individual pans were not labeled it was not clear which date was for which food and the foods could not be connected back to the cooling logs.
Galley-Deck 3 Roast Station
Two sheet pans of cooked rack of lamb were found in a trolley inside walk-in roast station refrigerator #4963. The label on the lamb was 25 December for production and the closest entry in the cooling log for this product was 23 December.
Galley-Deck 5 Dishwash
The right upper final rinse spray nozzle in the flight-type dishwash machine pointed to the right side panel of the machine, and not down to the dishware. The machine was in use at the time of the inspection.
Galley-Deck 5 Hot Galley
A few of the previously cleaned round and square plates found both in two plate trolleys and on the hot service line were found soiled with food residue. The plates were immediately removed for re-washing.
Preparation Room-
There was an uncovered pan of cut tomatoes on the top shelf in the walk-in refrigerator. Staff covered the tomatoes immediately.
Galley-Cold Galley
Apples in a bin in the cold galley still had code stickers attached to the fruit. The bin was on a counter with other whole fruits as back-up for lunch service.
Food Service General-Natura Water Service
It was unclear if the Natura bottles and bottle caps used for water service in the specialty restaurants can be adequately washed, rinsed and sanitized given the size and length of the bottles and the standard dishwashing machines used onboard.

16SEP2012: D7CC received a report from C/S ALLURE OF THE SEAS of a possible PIW 47NM east of Ft Lauderdale, FL. The CAPT of the C/S ALLURE OF THE SEAS stated that the reporting source was stating on her stateroom balcony on deck 10 and was struck by something she described as large and hard which hurt her, but did not injure her. The reporting source stated that she did not hear any noises, such as screams or voices. After reviewing the onboard cameras it was discovered that an object or a passenger had been captured on camera falling in that area at 2121Q that appeared to possibly be a person. The C/S investigated the whereabouts of the persons one deck above in the staterooms and located a room occupied by 02 females, the first was a 21years old and the other female in her 50s. The woman in her 50s was located in the casino and brought to guest services for questioning. After the C/S security officers interrogation was determined that the PIW was the 21 year old female. 19SEP12: Sector San Juan Investigating officers, Federal Bureau of Investigators(FBI), Virgin Islands Ports Authority, FBI CSI Team and MSD St Thomas went onboard the C/S ALLURE OF THE SEAS in order to investigate the incident. FBI investigation determined that the possible cause of this situation was a suicide and USCG marine casualty investigators determined that the cause of the incident was not related with any negligent marine operation or vessel condition.

Note: Incident investigation treated as a Data Collection due to the injury resulting from an apparent pre-existing medical condition and not from a marine operation. Received call from the ALLURE OF THE SEAS reporting that a male on board was experiencing significant gastro-intestinal bleeding and they requested a med evac. Med evac was sent down from Air Station Clearwater. Injured was dropped off at Key West International and was taken by LifeNet to Baptist Hospital of Miami for further treatment. Patient received an endoscopy which revealed 9 gastric-ulcers which are believed to be from the medication (Aspirin, Plavix, and Mobic) patient has been taking. The level of this investigation has been downgraded to "data collection" as per MSM V Part A5 Section C.
