Health
Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The submitted 24-hour report for the voyage ending in Port Everglades, FL on 6 February listed 11 reportable passengers and 2 reportable crew. After reviewing the AGE log, the actual number of reportable cases for the voyage was determined to be 9 passengers and 3 crew. According to staff a crew member was mistakenly entered as a passenger twice on the AGE log.
Recreational Water Facilities-Forward Pool
The sand filter media was replaced on 19 February but the cleaning and sanitizing of the filter housing was not documented.
Potable Water-Aft Bunker Station
Water was continuously leaking from the relief valve on the reduced pressure backflow prevention device indicating a possible failure,

One section of the distillate water line heading toward the boiler tank was striped blue/green/blue indicating potable water.
Potable Water-Medical Dialysis
The cross connection control plan had an entry for a dialysis machine in the medical center that stated the device used was a dual check valve with an intermediate vent. When the connection in the medical center was inspected no device was installed. According to medical staff, the last dialysis performed onboard was over a year ago. If dialysis was needed for a passenger medical staff had to call the engineering department before the voyage started to install the backflow prevention device in the medical facility. It could not be determined what device was used.

Medical-Foodhandler Isolation
A crew member (food handler) reported to medical on 10 January with one episode of diarrhea. Antidiarrheal medication was dispensed and the crew remember was listed as a reportable case on the AGE log. However, the crew member was only isolated for 24 hours.
Medical-Immediate Contacts
Medical staff was asking reportable crew members if they had additional immediate contacts (i.e. boyfriend, girlfriend, spouse, etc.) besides cabin mates but it was not documented if they did not.
Galley-Deck 4 Main Galley - Hot Galley
On the range top in the hot galley service line there was a large stock pan filled with fish stock at a holding temperature of only 129°F. Staff said this stock had been cooked overnight and strained some 1-2 hours prior to being transferred to the pot and the stove. The temperature was immediately increased to above 135°F.
Galley-Deck 4 Main Galley - Coffee Station
There was a water leak observed at the aft juice machine in the lower rear exterior panel, with pooling onto the counter below.
Galley-Deck 4 Main Galley - Dishwash
During active use of the flight-type conveyor dishwash machine several measures were made using thermal labels on the interior final sanitizing rinse manifold, which were all below the minimum 180°F. Several manual measures were also made by holding a thermometer directly at the dish level of the final sanitizing rinse spray and the temperature ranged from 152 to 158°F. The machine was stopped, repaired and re-tested. In the re-testing both the manifold temperature and dish level temperatures were above the minimum required.
Galley-Deck 5 - Main Galley - Beverage Station
There was visible dark mold residue on the water drip tubes over both cuber panels for the in-use ice machines 5.2 and 5.4.
Galley-Deck 5 - Room Service Galley
There were difficult to clean crevices surrounding the grease pan housing of the range stove top, which was also difficult to clean because of the length of the housing and the narrow access throughout its length.
Galley-Deck 4 Main Galley - Dishwash
During active use of the flight-type conveyor dishwash machine the exterior final hot-water sanitizing thermometer registered from 190-200°F, but through repeated running of the machine and testing of the manifold pipe temperature it never reached the minimum 180°F. There were no other visible or audible issues identified inside this machine to cause such a significant temperature deviation.
Galley-Deck 5 - Room Service Galley
There was visible dark mold residue on the water drip tubes over both cuber panels for the in-use ice machine 5.3.
Galley-Crew Galley - Hot Galley
Four separate older ranges were installed in this galley, and each had grease pan housings with crevices surrounding them that were difficult to clean. The grease pan housings were also very narrow and deep, making access for cleaning very difficult.
Buffet-Staff and Officers Mess
There was less than the minimum 220 lux across the bain marie hot buffet line. There was less than the minimum 110 lux behind the bulk milk dispenser beside the bain marie line.
Buffet-Deck 11 Concierge Club
There were 5 soiled spoons and 2 soiled coffee mugs set in the handwash sink basin at the self-service counter. The area was not occupied at the time of inspection, so it was unclear who placed these items in the sink. Staff stated that although disembarking passengers had no access, those sailing back-to-back did have access through key cards 24 hours/day. There was a utility sink on the same counter and immediately adjacent to the handwash sink.
Other-Izumi and Chops
There were two soup wells installed in the front service counter of Izumi restaurant which staff stated were on time control. There were two soup wells installed in the Chops galley counter which staff stated were managed on time control. In reviewing both time as a public health control plans for these two areas, neither plan identified the soup wells in the list of equipment on time control. This was corrected during the inspection.
Pantry-Pool Bar - Pantry
There was visible mold residue on the two drip tubes over the cuber panels inside the ice machine 9.3. Additionally, there were spots of dark mold along the interior side of the plastic cuber deflector panels. This machine was in active use at the time of inspection.
Bar-R Bar
The two lamps set on the corners of the front bar top each had two individual light bulbs inside which were not fully shielded or shatter-resistant.

There were two areas of deckhead along the back bar which had large open voids to a space over two meters above.
Ventilation-Deck 6 Fan Room 324
This large fan room had several air-conditioning fan units inside. During the inspection it was noted that fan units 310, 311, and 312 did not have access points such as doors or a hatch to access the condensate collection pans inside for inspection. None of these units had sight windows installed, while other fan units in the same room did have sight windows installed to easily view the condensate collection pans.

There was an open gap at the deckhead behind a structural column near the handwash sink.

A cleaned and sanitized cooking spatula and ice cream scoop were stored in contact with a frequently handled binder consisting of paperwork. The locker was located adjacent to the main dishwasher.

The deck drain basins and drains for the conveyor dishwash were heavily corroded with rust and were cracking in several areas preventing the surface from being easily cleanable.

Approximately, 6-8 fruit flies flew out of the dispensing nozzle to the portable draft beer machine when the inspector examined the unit. The unit was not in use and no beer was connected at the time of the observation. Also, 2 live flies and one dead fly was observed on the bar counter near the soda dispensing guns located closest to the handwash station.
Galley-Deck 5 - Dishwash
Plates were being loaded too close together in the flight-type dishwash machine which prevented proper spray from all cycles.

Galley-Deck 5 - Hood Cleaning Locker
The drain line to the relief vent of the backflow preventer for the hood cleaning unit was directed to the hood cleaning chemical reservoir. The drain line ended in a lower technical compartment of the locker and was not easy to observe if the device was failing. There was a slight leak coming from the drain line, possibly indicating a failure of the backflow device.
Dining Room-Deck 4
A previously cleaned and sanitized portable fresh orange juice press machine had a sticky residue and orange stems on the bottom surface of the orange hopper. The unit was sent to be re-cleaned.

Much of this area had missing and/or recessed grouting between the deck tiles.

Medical-Maritime Illness and Death Reporting System (MIDRS) Report
The MIDRS report for the 3/7 - 3/14 voyage did not match the actual acute gastroenteritis (AGE) case count as documented in the AGE log. The AGE log listed 6 reportable AGE cases while MIDRS documented 3 reportable AGE cases. Also, the MIDRS report that is automatically sent by the ship's database was submitted more than 36 hours before the ship's arrival at the U.S. port.

The potable water pipe disinfection records did not document a free halogen residual level to verify that the halogen value was less than or equal to 5 ppm before being placed into service.

Galley-Windjammer Scullery
On the racks for clean utensils, one coffee cup was still soiled with lipstick and two additional coffee cups had some food debris. This was corrected.
Galley-Countertop # 9.3
According to the staff, the large hotel pan of mozzarella cheese on countertop # 9.3 was on time control, but the pan was not labeled with a discard time. Also, the time control plan did not specify that potentially hazardous foods on this countertop were on time control.

The time control plan for the milk on the coffee stations stated that the outlets had a 24-hour operation and that the milk was to be discarded after four hours of being on time control. There were no set-up and discard times on the plan. The staff confirmed that these stations were in operation from 06:00 to 21:00, including set-up and discard times, and not 24 hours.
Other-Concierge Lounge
The time control plan for the milk on the buffet counter stated that the outlet had a 24-hour operation and that the milk was to be discarded after four hours of being on time control. There were no set-up and discard times on the plan. The area was not in operation at the time of the inspection. The staff confirmed that this buffet counter had several set-up and discard times and was not open 24 hours.
Other-Concierge Lounge
There were menus for all restaurants available to the passengers in this concierge lounge. The menu for the Izumi restaurant had animal-derived foods that could be served raw or undercooked and these were identified with asterisks, but the consumer advisory was hidden under the menu cover. This was corrected.
Galley-Park Cafe
The staff stated that the outlet was open from 12:00 to 18:30 on embarkation days and time control was to be used for potentially hazardous foods. However, this was not stated on the time control plan. Also, under the time control plan heading of 'LUNCH' with set-up and discard times for the lunch services, there were set-up and discard times for a midnight service from 21:00 to 01:00.
Galley-Park Cafe
There were two slotted fasteners in the food-splash zone of the pizza oven. Also, there were 12 slotted fasteners in the food-splash zone over the milk steamers of the espresso machine.

The previously cleaned food slicer had a slight build-up of food residue under the slicer's backplate next to the exit path of the food from the slicer blade.

Galley-Deck 4 Blast Chiller #1
Two trays of breaded chicken were inside the blast chiller. The cooling log documented an initial temperature of 43°F at 10:50. Upon arrival to the area, the inspector checked with a thermocouple and the temperature of the chicken in different locations of the trays was 46-47°F for the top tray and below 43°F for the bottom tray. One of the chicken pieces at the outer edge of the top tray had the hole where a thermometer had been inserted to measure the initial temperature by a food employee. The center of the top tray had a bigger amount of chicken pieces, making the center area to cool more slowly.

The time control plan for the milk in beverage stations of the crew mess and staff mess stated that the beverage stations had 24-hour operations, no set-up and discard times, and that the milk was to be discarded after four hours of being on time control. The staff confirmed that the beverage station in the crew mess had a 24-hour operation, but that the beverage station in the staff mess had several opening and closing times every day, including several set-up and discard times.
Pantry-Latte-tudes
The deckhead lights over the handwashing station and over the warewashing area were not operational. As a result, the light intensity was less than 220 lux. This was corrected.

There were no rat guards installed on the two port aft mooring lines.
Housekeeping-Outbreak Prevention and Response Plan (OPRP)
There were no procedures in the OPRP on how to notify crew members of an outbreak.
Housekeeping-Public Shower Head Disinfection
The procedures for the disinfection of the shower heads in the public area stated a concentration of chlorine solution would be used between 100 and 150 ppm, but there was no recorded contact time. According to staff, the contact time was 10 minutes.
Medical-Crew Acute Gastroenteritis (AGE) Reporting
The review of the AGE logs for the last 5 cruises revealed that there were three crew members who experienced AGE symptoms and continued to work after those symptoms began. Specifically, (1) a food employee had an onset of AGE symptoms on 10 January at 15:00, worked from 17:22 to 17:45, and reported to the medical center at 18:26 the same day; (2) a nonfood employee had an onset of AGE symptoms on 16 December at 12:30, worked from 12:56 to 16:00, and reported to the medical center at 16:50 the same day; (3) a food employee had an onset of AGE symptoms on 4 December at 11:40, worked from 11:01 to 13:25 and from 13:54 to 15:34, and reported to medical at 17:41 the same day.
Medical-Acute Gastroenteritis (AGE) Surveillance Log
On 31 December, a food employee experienced symptoms of myalgia only and was recorded on the AGE log as a reportable case. According to the medical staff, the employee had influenza and they were not able to go back into this case to update the records. Also, there was no information explaining why this case was entered as reportable. On 4 December, a food employee experienced two episodes of diarrhea and abdominal cramps, was dispensed anti-diarrheal medication, and was recorded as a non-reportable case. According to the medical staff, there was no underlying illness and the symptoms were above normal for this individual.
Pantry-Cleaning Lockers
The lockers for cleaning materials were not labeled 'CLEANING MATERIALS ONLY' in the deck 6 ice pantry #062300 and in the deck 8 food pantry #083303.
Potable Water-Tank Records
The potable water tank 3S was painted on 21 February 2013, but there was no written information on the drying and curing time of the paint or that manufacturer's instructions were followed.

Potable water tank 1P was opened and inspected on 11 February 2013. The documentation only included that the tank was found in good condition, but there were no records the tank was cleaned, disinfected, or flushed before it was put back into service. According to staff, no one entered the tank during the inspection.

During the cruise from 27 November to 12 December, there were 10 people including crew and passengers that were indicated as nonreportable on the AGE log, but according to their symptoms, medical notes, and the medical staff they should have been recorded as reportable AGE cases.

Medical staff did not know how to properly document important required information on the AGE log during some cruises. For example, there were several crew and passengers that were incorrectly documented as nonreportable when there were reportable according to the case definition as outlined in the 2011 VSP Operations Manual.

Symptoms: vomiting, diarrhea
In response to the outbreak, Royal Caribbean Cruise Line and the crew aboard the ship reported the following actions:Increasing cleaning and disinfection procedures according to their outbreak prevention and response plan,Making announcements to notify onboard passengers and crew of the outbreak, encourage case reporting, and encourage good hand hygiene,Collecting clinical specimens from passenger and crew gastrointestinal illness cases for testing by CDC,Making multiple daily reports of gastrointestinal illness cases to the VSP,Sending corporate management, hotel, and housekeeping team to assist the onboard management with infection control response plan,Is consulting with CDC on plans for their comprehensive sanitation procedures in San Diego, CA on April 14, 2015, including: providing additional cleaning crew to complete a thorough public and accommodation super-sanitization cleaning and disinfection,planning staged disembarkation for active cases to limit the opportunity of illness transmission to well guests, andplanning for sanitation of terminal and transport infection control procedures.One CDC Vessel Sanitation Program environmental health officer and one epidemiologist will board the ship in San Diego, CA on April 14, 2015 to conduct an environmental health assessment and evaluate the outbreak and response activities. Specimens are being collected and will be tested by CDC to determine the causative agent for this outbreak.
