NOTABLE EVENT: near drowning

On May 27, 2014 a 6-year-old boy travelling with his parents on the Independence of the Seas was found "in a state of cardio-respiratory arrest at the bottom of the ship's pool." He was taken by helicopter to a hospital in northwest France where his condition was described as "grave."

The Sky Bar, Squeeze Bar, Solarium Bar, and Pool Bar had swinging doors from the outside bar areas to the pantry areas with gaps measuring 1.5 inches between the door and door frame.

The pot wash machine had been out of order since 4/22/2014, and a work order was in place. Staff provided documentation showing the spare parts needed were ordered.

Previously used metal bread baskets were stored in the locker under the stairs, which was not properly designed for food equipment storage. This locker did not have a finished deckhead or bulkhead and had exposed wiring running against the bulkhead.

The chlorine concentration in the sanitizing bucket was above 200 ppm.

Medical-Crew Acute Gastroenteritis (AGE) Reporting

The review of the AGE logs for the last 5 cruises revealed four crew members worked while experiencing AGE symptoms. Specifically, (1) a facility cleaner had an onset of AGE symptoms on 11 April at 12:30 and reported to the medical facility the same day at 16:43; (2) a cook had an onset of AGE symptoms on 17 April at 05:30 and reported to the medical facility the same day at 13:32; (3) a beautician had an onset of AGE symptoms on 16 April at 10:00 and reported to the medical facility on 17 April at 15:08; (4) a room service attendant had an onset of AGE symptoms on 18 April at 08:30 and reported to the medical facility the same day at 15:30. These employees received disciplinary actions per medical staff.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

On November 10, there was one passenger with two episodes of diarrhea, abdominal cramps, a headache, and they were given antidiarrheal medication, but were recorded as nonreportable on the AGE log. The underlying illness column stated 'none', but according to medical documentation, this passenger's symptoms were a result of eating too much salt in their food. On November 13, there was one passenger with two episodes of diarrhea and they were given antidiarrheal medication. 'None' was recorded in the underlying illness column. According to the medical staff, this individual was not counted as reportable because they had less than three loose stools. There was no information about whether this was above normal for the individual. On November 6, a crew member had one loose stool, abdominal cramps, a headache, and was given antidiarrheal medication, but they were recorded as nonreportable on the AGE log. According to the medical staff, they were not sure if this individual's symptoms were above normal or not.

The medical staff was not asking the patients who had diarrhea if their symptoms were above normal or not during the medical evaluations of patients. As a result, there was one passenger and one crew member that were documented as nonreportable on the AGE log, but it could not be determined from the information collected by the medical staff if these patients were reportable or nonreportable.

The time control plan stated the alto sham and the pretzel maker were on time control, but according to the staff, these units have never been used and are not placed on time control.

Recreational Water Facilities-Turnover Rates

The calculated turnover rates for several different RWFs exceeded the maximum turnover rates, including: H20 Zone Pool #4, Flow Rider, Current Pool, and the Kids Spray Park. RCI had been in contact with VSP regarding these RWFs and the issues with renovating the current designs with larger pumps and filters to meet the new turnover rate requirements. The ship planned to install larger pumps to meet the required turnover rates and documents were provided by the ship showing that outside consultation had begun. A completion date was not currently available on when all the RWFs would be in compliance.

A significant amount of condensation was accumulating on the deckhead in the center of this walk-in unit. No water was observed falling onto the deck and no food was impacted.

Galley-Deck 5 Cold Buffet Cooling Logs

Several trays of thin ham and cheese sandwiches were placed in the walk-in refrigerator to cool from an ambient temperature at 7:03 AM. The time and temperature was recorded on the cooling log. When the inspector arrived in this area at 11:50 AM there was no final temperature recorded on the cooling log to indicate the food had reached 41°F or below within four hours. Temperatures were taken of the ham and cheese sandwiches and all were at 41°F or lower.

The plastic blade guide on the table of the meat saw was extremely worn with frayed pieces of plastic and grooves that were difficult to clean.

Galley-Potwashing

A large mixing bowl stored in the clean utensil storage rack had several white particles of food residue on the interior. The rest of this mixing bowl was clean as were all of the other pots and pans examined in this clean storage rack.

Galley-Deck 3 - Potwashing

The handwashing sink next to the automatic trolley-style potwasher was blocked by an empty trolley. This trolley was rolled to a proper storage space to the left of the canopy-style automatic potwasher by a supervisor on the ship's inspection team.

A severely scored plastic cutting board was noted in the clean utensil storage rack. The other three cutting boards stored in this area were completely smooth. The worn cutting board was removed by staff for resurfacing.

Galley-Deck 3 - Hot Galley

A slight leak was noted from the vent of the backflow prevention device on the automatic hood cleaning system. The plumbing team had the backflow preventer repaired before the inspection team left this galley.

The lid of the rolling sugar bin was warped, did not fit tightly, and was difficult to close.

The sanitizer solution in the three-bucket cleaning system was very soiled with food debris and grease floating in the solution. The staff directed the pastry team to immediately discard the solution, clean the bucket and refill with a fresh sanitizer solution.

One house fly was observed in the preparation pantry. No food activities were being conducted in this area during the inspection.

The container of milk in the coffee machine (#4/3) was identified on the time control plan. The holding cabinet for the milk was not labeled 'Time Control'. A sign was requested to be placed on the cabinet while the inspection team was still in the area.

Galley-Deck 4 - Dining Room

A small special meal menu indicated smoked fish dishes and steak cooked to order were available. The menu did not have a consumer advisory statement and the food items that were undercooked and cooked to order were not identified with an asterisk. According to staff, this was an old menu and other menus with the same food items did have all of the required information.

The handwashing sink with a shelf above it was blocked by a foot stool placed in front of the sink.

The front bar handwash sink was completely dark when the inspection team entered the area that was in active service. The staff explained that it only could be illuminated when the bright back bar cleaning light was turned on.

Recreational Water Facilities-

The turnover rate for the H2O Zone Pool #4 was 3.5 hours. Staff stated they could discontinue the waterfall feature, which would reclassify the facility as a regular swimming pool. Also, the turnover rate for the Flow Rider was 4 hours, the turnover rate for the Current Pool was 3.7 hours, and the turnover rate for the Kid's Interactive Pool was 0.7 hours. A variance for the Flow Rider and the Current Pool turnover rates have been submitted to VSP and are pending review.

Recreational Water Facilities-Current Pool

The Current Pool had two side-mounted suction/gravity drains that were located more than 3 feet apart. The drain covers were of a standard design. Also, there were two other side-mounted gravity drains located more than 3 feet apart. These drain covers were of a standard design and there were no alarms installed. Staff showed the inspector new compliant drain covers and described their proposed installation.

The cooling of a container of cut lettuce and a container of cut tomatoes prepared on 10 April was not documented in a cooling log.

Galley-Deck 5 - Buffet Station

On the food preparation counter, a large lexan box of cut lettuce had two labels, one with '4/9 - 4/15' indicating the food was under temperature control with a 7-discard and another label with '11:00 - 15:00' indicating the food was under time control. When questioned, the staff stated that the potentially hazardous food was on time control and discarded the temperature control label.

Inside the technical compartment of the glaze spray machine, the front bottom edge was soiled with a dark material.

The light intensity at the self-service cup racks was less than 220 lux in front and less than 110 lux behind. In addition, the light intensity along the length of the self-service side appeared to be less than 220 lux.

The deck below the shelves was heavily soiled in some areas.

The splash zones above the milk steamers of the two San Marco espresso machines were open to wires and pipes, making these areas difficult to clean. In addition, the panels over the steamers were corroded, making them difficult to clean.

There were pieces of brown debris in the sugar in the undercounter rolling storage bin and it appeared that liquid had dripped into the sugar and dried. Also, it appeared that liquid had dripped into the pastry cream powder in the undercounter rolling storage bin and dried. There was a small piece of brown debris that appeared to be cake in the almond flour in the undercounter rolling storage bin.

The time control plans posted in decks 3, 4, and 5 of the main galley indicated that set-up for lunch began at 1200, but according to the chef, lunch began at 1200 and set-up began at 1130.

There was condensate collected on the underside of all the shelves above the bains marie. Condensate was dripping from one shelf, but no food was impacted. The steam for the bain marie units was turned down.

The consumer advisories included the correct advisory statement but did not include the food that was served raw or undercooked. According to the staff, undercooked eggs and cold smoked salmon were served in the messes.

There were several pans of potentially hazardous food (PHF) on the preparation counters being used for making sandwiches, including lettuce, sliced tomatoes, Swiss cheese, and cold smoked salmon. Once prepared, these sandwiches would be placed in the walk-in refrigerator for cooling and logged on the cooling log, but the pans of PHF had 4-hour discard labels. According to the process, PHF would be placed on time control and then back on temperature control.

On one shelf in the disposables locker, packaged single-use food service items such as cup lids and doilies were stored under, above and around packaged office supplies such as markers, pens, sticky notes, and printer paper.

The sink in the toilet room for children under 6 was 25 inches above the surface of the step in front of the sink. This was corrected.

The thermometer inside an undercounter refrigerator containing several different types of potentially hazardous dairy food items unit showed a reading of 58°F. The following potentially hazardous food items were found out of temperature and discarded: approximately 30 single-serving milk cartons (48°F), 2 bulk bags of milk (47°F), approximately 40 individual containers of yogurt (50°F), and a hotel container of individually wrapped packages of creamy butter. Staff stated the food items were placed in the refrigerator 2.5 hours prior to the inspector taking temperatures.

Buffet-Concierge Lounge - Deck 10

There was no time control plan for the milk being used in the coffee/cappuccino machine nor was the refrigerator compartment physically labeled as on time control. The container of milk had a four hour discard label.

Housekeeping-Bridge Toilet Room

There was no sign advising users to use a paper towel or hand towel to exit.

There was no sign advising users to use a paper towel or hand towel to exit in the stall with a handwashing sink.

Integrated Pest Management-Active Surveillance

There were no follow-up inspection results or action taken for pests found during active surveillance.

Recreational Water Facilities-Turnover Rates

The turnover rate was 0.7 hours for the kid's interactive swimming pool. The turnover rate for the current interactive swimming pool was 3.5 hours. The turnover rate for the flow rider was 4 hours.

Recreational Water Facilities-Whirlpool Spas

There was no SVRS or APS installed on the therapy pumps. According to the staff, these devices have been ordered.

Recreational Water Facilities-Baby Splash Zone Variance

The variance stated that the samples taken twice a day for the presence or absence of total coliform and e. coli needed to be time stamped and that the samples should be taken at closing time and prior to shocking. However, the documentation did not have the time the samples were taken and the samples were taken prior to closing. Also, the variance stated that one of the samples taken three times a day for total coliform and e. coli should be taken at 4:00 p.m. but the documentation stated the samples were taken prior to closing without a specific time. An observation on the previous inspection on 3 April 2010 indicated that documentation for the times of the second daily presence or absence tests were incorrect.

The bulk milk dispensing refrigerator was labeled "time" on the exterior door. The area was not in operation during the inspection at 10:15 am, but staff stated that the coffee station was open from breakfast through dinner. The interior of the bulk milk dispenser had a partially full bag of milk in the left position with a label with 0700-1100 as the set-up and discard times. The right position had a full and unopened bag of milk with no discard label affixed. The written time as a control plan noted only that the bulk milk stations in messes were on time control. No service hours or label information was given. Staff stated the full milk was placed in this refrigerator/dispenser in the morning because two are normally needed. The inspector expressed concern that the full container would not be discarded as required at 11 am, but was assured by staff that it would. The inspector marked the top of the unopened bulk milk bag with his pen. On a return inspection of this dispenser at 12:30 there was one partially full bag of milk in the left position and nothing on the right. The time label displayed a time of 1055 - 1455. On close inspection of the top of the bag the black mark made earlier was present, confirming this was the milk which was to have been discarded at 11am. The bag was removed and discarded.

Galley-Cold Pantry

The previous cleaned honeydew melons had the manufacturer/producer stickers still affixed to them. In one case the ink from the label had stained the outer rind of the melon.

A portable soup pot was at one end of the buffet, outside of the sneeze shield. There was a simple hinged lid on this soup pot. The lid was not self-closing and this was a self-service station.

Recreational Water Facilities-Baby Splash Zone Records

The daily records for the Baby Splash Zone were on a pre-printed form. This form had the sample time for the second IDEXX sample as 1700. On most days the Baby Splash Zone did not close until 1800. The variance states that the second IDEXX sample is to be taken after the area is closed and before superchlorination. Staff stated that the samples were taken after closing.

Galley-Johnny Rockets

A large amount of dust was found on top of the cold hold trolleys. This was corrected.

There was an accumulation of dust and debris in the technical space below the hot hold units.

The front of the grease collection tray for the grill was bent and not easily cleanable. This item was corrected.

Several handles for Sea King undercounter hot and cold hold units were broken. This was also noted in the buffet. The crew indicated that work orders have been submitted.

There was an accumulation of condensate above the soiled side of the flight-type warewash machine. No condensate was observed dripping.

A large fly was observed in Sorrentos above the pizza preparation table.

Bar-Dog and Badger

The beer supply lines leading from the technical space to the temporary beer tap on the counter were draped on the counter, making cleaning difficult.

There was an accumulation of food debris and dust in the track of the pass-through window.

Housekeeping-Deck 6

A tray of soiled square sugar containers was stored above clean dishes and utensils in Pantry 6075. This item was corrected.

Children Area-

The Legos had recently been cleaned and sanitized. Several Legos had not been separated and had water between the connected pieces.

Drowning

A 6-year-old boy travelling with his parents on the Independence of the Seas was found "in a state of cardio-respiratory arrest at the bottom of the ship's pool." He was taken by helicopter to a hospital in northwest France where his condition was described as "grave."
