Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Integrated Pest Management-IPM Plan
The IPM Plan stated that the inventory manager would be trained in IPM procedures but there was no record of such training. The inventory manager was actively checking incoming supplies with the provision master on the day of the inspection.
Pantry-Deck 10 Forward/Starboard Ice Pantry
The top areas over the cuber panels and the curtain deflectors were heavily soiled with a brown material. The machine was last cleaned in November 2014 according to a label on the machine.
Potable Water-Cross-connection Control Program
The previous inspection found that the plumbing system component protected with a backflow preventer or air gap was not always documented on the cross-connection control list for the Engine Department. The current inspection found several entries that still did not have the plumbing system component that was protected: (1) Generator Room/Watts 9D-M2, (2) Garbage handling room behind basin and water cooler/Watts N9, and (3) Garbage handling room between two pulpers/9D-M2. Although the log still needed updating, significant improvements were noticed.
Medical-Symptomatic Crew Reporting
During the voyage between 29 January to 7 February a crew member reported to the medical center at 08:35 after experiencing onset of acute gastroenteritis (AGE) symptoms at 20:10 the previous day. The crew member did not work during this time period. During the voyage between 24 January to 29 January a crew member reported to the medical center at 00:03 after experiencing onset of AGE symptoms at 07:00 the previous morning. The crew member did not work during this time period.
Dining Room-Deck 3 Waiter Stations
The lower shelving units were made of a laminated material that was in disrepair. This was noted in the previous inspection. Steps had been taken to correct this problem and the project was about 50% completed.
Provisions-Deck 3 Vegetable Cold Storage
Pineapple, strawberries, and kiwi were stored uncovered on the shelves in this area.
Buffet-Deck 5 Officer Mess
The chlorine concentration in the sanitizing bucket was below 50 ppm when measured.

The hand soap for the handwashing station was installed partially over a preparation table. Splash was observed on the preparation table. No food was observed.

The stainless steel underside of the aft hot top counter had jagged holes to drain the cooking oil from the electrical compartment, making cleaning difficult.

Sealant was peeling and exposing gaps on the forward hot counter underneath the marble counter where the stainless steel joined the marble. Sealant underneath the aft hot top counter was peeling and exposing gaps underneath the grill equipment.

The waiter stations had lower shelving units made of laminated particle board. On many of the stations the metal support framing was pulling away from the shelving, exposing the rough edge and raw particle board to any food spillage from the soiled return area and creating a difficult to clean area. This was noted on the last inspection report. Additionally, many of the doors were missing and a work order was shown to verify this would be addressed. Staff reported all the dining rooms and waiter stations would be renovated during the March 2015 dry dock.
Dining Room-VIP Special Event Lunch Menu
The menu did not have a public health advisory for the grilled NY steak offered on the VIP lunch menu. This was corrected immediately before the lunch service.
Galley-Deck 3- Potwash
Approximately 16 stainless steel pans were stored wet and not allowed to properly air dry on the potwash clean racks.

The electrical wire of the espresso machine was braided, making cleaning difficult.

A bakery trolley was stored underneath an insect light trap. The trolley was empty.

The chicken curry salad and pasta chicken had a discard date of 10/28. Upon inspection of the cooling logs, the chicken for these dishes was cooked on 10/21.

The microwave was sealed to a table mounted on legs, and a crevice was present in front of the equipment between the microwave and table.

Bread crumbs were observed in the crevice in between the microwave and table.
Medical-Food Employees Released From Isolation
During a review of the medical electronic surveillance system for the previous five cruises, a three food employees were released from isolation 12-24 hours before the required isolation period. However, it appeared the system recently produced inaccurate dates and times; the Lead Nurse explained to the inspector she reviewed all follow-up and isolation release dates and times for the past few months, and they were appropriate. She also explained that twice in October the system had issues, which were addressed to the corporate office for maintenance. According to the acute gastroenteritis (AGE) surveillance system, a Utility was followed up on 15 October at 23:14, and his last AGE symptoms were on 14 October at 19:00; he was followed up on 15 October at 17:14, and his last symptoms at 07:00 that morning; he was followed on 16 October at 08:09, and his last symptoms were on 14 October at 19:00. At that time it was noted he was fit for duty and sent back to work. It could not be determined exactly when his last AGE symptoms occurred, but he was released from isolation 12-24 hours early. A Room Service Attendant was followed up on 12 October at 18:01 and was released from isolation at that time, despite the documentation stating her last AGE symptoms occurred on 11 October at 09:50; her release was more than 15 hours early. A Cabin Attendant was followed up on 28 September at 16:14 and was released from isolation at that time, despite the documentation stating his last AGE symptoms occurred on 27 September at 14:00; his release was nearly 24 hours early.

Gaps were present between the lower portion of the undercounter refrigerator doors and the units.

A box of unsalted margarine was soiled with an unidentifiable brown liquid. Many other boxes of food were stored below the evaporator, but these boxes were not soiled or wet.

A broom was stored next to the counter of clean red plates and a dust pan was stored leaning against the side of the clean storage rack.
Potable Water-Bunkering Pretest
The chlorine and pH values from the shore-side water supply were not documented for bunkering that took place on 3 July and 14 June. The chlorine value from the shore-side water supply was not documented on 5 June.
Potable Water-Evaporator #2
The reduced pressure backflow prevention device for the high salinity discharge was installed in a way which caused the relief vent to be bent upward at almost a 45 degree angle.
Potable Water-Cross Connection Control Log
Several of the entries on the Engine Department's cross connection control log for the non-testable devices did not list a specific plumbing connection. Some example entries include: Item 16 water bunker station forward port; Item 17 water bunker station forward starboard; Item 18 water bunker station aft port; Item 19 water bunker station aft starboard; and Items 24.2, 24.3, 24.4 Deck 1 engine crew changing room.

Medical-Acute Gastroenteritis Case Definition
A review of the acute gastroenteritis (AGE) surveillance logs for the voyages between 24 April and 31 May indicated the medical staff was not consistently applying the AGE reportable case definition. For example, (1) on 29 May a passenger had 2 episodes of diarrhea, was issued antidiarrheal medication, had no underlying illness, and was listed as an AGE non-reportable case; (2) on 14 May a passenger had 1 episode of diarrhea, headache, was issued antidiarrheal medication, had no underlying illness, and was listed as an AGE reportable case; (3) on 9 May a passenger had 1 episode of diarrhea, was issued antidiarrheal medication, had no underlying illness, and was listed as an AGE non-reportable case. There were no notations to indicate these symptoms were or were not above normal for the passengers. Also, on 3 April a passenger had 2 episodes of diarrhea, was issued antidiarrheal medication, had no underlying illness, and was listed as an AGE non-reportable case. This passenger's medical notes stated they were listed as a non-reportable case because they had 'only 2 episodes of diarrhea without other symptoms.'
Recreational Water Facilities-Kid's Spa Pool - Safety Sign
The kid's pool was equipped with features of a spa pool. Both adults and children used this facility. At the entrance, the safety sign did not include all of the requirements of section 6.7.1.1.3 of the VSP Operations Manual. Specifically, the sign did not caution users against exceeding 15 minutes of exposure.
Dining Room-Passenger Main
The waiter stations had lower shelves made of laminated particle board. On many of these stations the metal support framing was pulling away from the shelving, exposing the rough edge to any food spillage from the soiled return area and in all cases creating a difficult to clean area. Additionally, the cabinet compartments with doors on them had the protective laminate on the lower shelf eroding by the door scraping across it, again creating difficult to clean surfaces.
Galley-Hot Galley - Deck 3
On the water supply line of the automatic hood cleaning system, the reduced pressure backflow prevention device had a constant leak from the air relief vent indicating a possible malfunction in an upstream check valve.
Galley-Hot Galley - Deck 3 - Soup Station
The supports for the tilting soup kettles had missing and peeling sealant in the support seams and the seams between the supports and the adjacent bulkhead panels.
Buffet-Crew Mess - Deck 2
Three new portable buffet tables were placed between the serving line walkway and the adjacent dining tables. The light intensity along the front of these new tables was 90 - 110 lux rather than the required 220 lux.

Blue linoleum deck tiles were cracking in the center of the walking area in front of the three new portable buffet tables and around the column to the right and across from the buffet table. The yellow linoleum deck tiles between the tables and the end of the beverage counter also had large cracks. These areas were in poor repair and difficult to clean.

The previously cleaned soft ice cream machine drip tray chute was soiled with old ice cream mix.
Buffet-Crew Mess - Deck 2
The counter-mounted ice maker/dispensing unit was soiled with mildew in the ice maker chute and storage bin. The ice maker was off and there was no ice in the bin. The staff explained that the ice maker was not working so that part was out of service awaiting repairs, but the unit was still being used for water dispensing. The machine was removed and cleaned while awaiting repair parts.

One fruit fly was near the crew entry handwash sink.

The handwash sink access was blocked by five large and small deck stands in front of it.

Condensate was collecting in the inner lip of the canopy over the automatic potwash machine. The water overflowed periodically at the front corner closest to the 3-compartment sink. The refrigeration engineer checked and found that the ventilation monitoring system showed that the exhaust fan was working at 100% capacity. He thought the problem might be with the cooling coil in the exhaust system not working correctly, which would reduce the efficiency of humidity extraction.
Galley-Hot Galley - Deck 5
Two large clean white plastic bins were stored wet, nested on a counter. A large amount of water poured out of the area between the bins.
Buffet-Windjammer Line 3 - Beverage Station Starboard
A wet wiping cloth was set on the counter between uses.
Buffet-Windjammer - Egg Station
Raw shell eggs were cooked to order at the grill. The consumer advisory signs did not include the initial statement that eggs could be cooked to order and may be served raw or undercooked. The other half of the advisory was complete.
Buffet-Windjammer - Island Grill
There was a heavily damaged decorative deckhead section of approximately one square meter on the passenger side of the buffet, which was also heavily soiled with black mold and what appeared to be a sealant product.

On the worker side of the egg grill section, the decorative deckhead was heavily damaged in a section of about one square meter, creating loose profile strips and gaps, and a heavy brown oil residue was overflowing the attached deckhead speaker cover.

The aft starboard soda dispenser had six slotted fasteners on the underside of the root beer dispensing head. This area was considered a food splash zone.
Other-Deck 9 - Concierge Lounge
The undercounter cabinet beside the handwash station was filled with liquor bottles and mixers for cocktails. The only sink available to this service counter was an in-counter handwash sink. Handwash sinks cannot be used for mixed drink preparation.
Provisions-Fruit Walk-in Refrigerator
One of the deckhead fire sprinkler heads in the center of the cold room had a brown oil type liquid leak around it.
Preparation Room-
One live fruit fly was on a stainless steel pillar connected to the preparation counter.

A chef's knife, which was still wet and covered with food debris from use, was found stored inside the bulkhead-mounted knife locker.

None of the staff in this preparation room were provided with tip-sensitive food probe thermometers. There were sliced tomatoes in refrigeration and staff stated that they also clean, cut, chill and store cut leafy greens.

There was one live fruit fly on the deckhead directly over the forward preparation sink.

At the back bar deckhead directly above the clean glass storage, there was damage and a very loose light fixture with large paint chips wedged into the face of the light.

There was no identification on the two chilled water supply lines to the ice machine to indicate they contained chilled water.
Housekeeping-Individual Cabin Whirlpool Tubs
The superchlorination of the suite cabin whirlpool tubs was conducted only on the embarkation and disembarkation days for the cruises lasting more than 7 days from December 2013 to present. Some were 8 day cruises and others were 9 day cruises.

Next to the drinking fountain, the handwash station did not have a waste receptacle. This was immediately corrected.

A broom and dustpan were stored behind the waste receptacles at the end of the counter.

The faucet of the dipper well was continuously leaking. This was immediately corrected by the repairman.

On the conveyor glasswash machine, the manufacturer's data plate stated conveyor speeds 1 and 2 in feet per minute and the maximum conveyor speed in racks per hour.

The light intensity was less than 110 lux between the ice/water machine and the juice machine.
Galley-Deck 5 - Across From Glass Refrigerators
Six baskets of previously cleaned silverware were stored upright on the counter tray rail and were not inverted or covered. No crew member was actively sorting the silverware.

One live fly was in the area. The fly was immediately destroyed.
Galley-Potwash Area
On the hood-type potwash machine, the pressure gauge did not work while the machine was in operation. Later during the inspection, the crew reported the gauge had been replaced, but this was not verified by the inspector.

A box of disposable gloves was stored in the soiled area. The box was removed.

The locking mechanism was missing, exposing a hole on the inside of the door. A piece of masking tape covered the hole.

Medical-Crew Member Reporting
According to the acute gastroenteritis (AGE) surveillance log, a Facilities Cleaner had an onset of AGE symptoms on 2 January at 11:00, but did not report to the medical center until 17:47. According to her time card, she worked from 17:03 - 17:58.
Potable Water-Cross-Connection Control Program
The list of cross-connections did not always have the plumbing system component and location for backflow preventers. Also, there were four locations having no description of the backflow preventers installed: crew galley faucet bain marie, deck 3 galley, deck 3 galley ovens, and palace port. In addition, the list stated that an atmospheric vacuum breaker was installed for the 'shower dialysis' but actually there was a reduced pressure assembly for the dialysis connection next to a shower in medical ward #4.
Ventilation-AC Unit 211
The condensation drip pan had a large amount of debris deposits and water.
Recreational Water Facilities-Wading Pool and Slide
The record review of the halogen and pH levels for the area with the wading pool and slide revealed that the halogen levels were consistently above 5 ppm. The wading pool had a waterfall. The area was not open during the inspection. The staff stated that they were treating these systems with whirlpool spa levels of 3-10 ppm chlorine and not 2-5 ppm for interactive water facilities. Staff also stated that they could warm the water of these facilities from 24-28°C.
Recreational Water Facilities-Children's Pool
The turnover rate for the children's pool with jetting features was more than the required 0.5 hours. The staff calculated the turnover rate at 1.9 hours.

There was no serving utensil for the container of cantaloupe.
Buffet-Windjammer B/L/D Time Control Plans
The self-service lines #3 and #4 were open greater than four hours, but there was no instruction on the time control plans to label the potentially hazardous foods with the discard time. Also, the plan did not indicate what foods were on time control unless the food was in a time control refrigerator. For example, the self-service counters were not indicated as being on time control. Also, the hot plan for these areas indicated the 'hot cupboards' were on time control, but these units were not labeled as being on time control. According to staff, these units were undercounter hot cabinets used in certain areas. There were also the same type of cabinets on temperature control in the galley for this area. Finally, on preparation counters there were containers of potentially hazardous foods on time control that were labeled with their discard time, but these containers were not identified on the time control plans. The time control plans for the port and starboard lines and the beverage stations were only posted in one area on the starboard side of the Windjammer and not in close proximity to the port lines or the beverage stations.
Buffet-Line #4
Cartons of fat free and vitamin D milk were not labeled with the discard time on the Sweet Dreams line and the potentially hazardous food on main line #4, such as sausage were not labeled with the discard time. These lines were open more than four hours, but during the inspection none of the food items had been out for service for greater than four hours.

On the voyage ending today (19497) the ship submitted both the 24 hour and 4 hour gastrointestinal (GI) illness report to the CDC Vessel Sanitation Program (VSP) on 11 November, as the ship visited foreign ports prior to the arrival in a U.S. port, but the ship sailed on to other foreign ports prior to arrival in Bayonne, NJ today and no 24 hour report was made to VSP. On the cruise prior (19496) the ship made three reports as required in the middle of the cruise following foreign port calls and prior to U.S. port arrival, but failed to make the 24 hour report at the end of the cruise in Bayonne on 7 November.

A food worker gastrointestinal (GI) illness case reported to medical on 31 October at 11:28 am, with illness onset at 7 am. Their contact, who was also a food worker, did not report to medical for their interview until 1 November at 11:12 am.

Potable water tank #1 port was partially recoated in June 2010 and tank #4 starboard was partially recoated in July 2010. In both cases the records indicated that the tank coating was applied according to the manufacturers instructions, but there was no information about the curing or drying process to ensure they were also done in accordance with the manufacturer's instructions.

Staff immediately installed backflow preventers on each of the clothes washer hose connections for hot and cold water.
Pantry-Deck Pantries
The overhead spray hose was within two meters of the clean dish rack beside the dishwash counter, potentially subjecting clean items stored there to soiled splash. There was no side splash panel on the clean rack in any of the pantries. Staff stated that on this ship the only clean racks used for air-drying of clean items are the deck 10 pantries for suite cabins.
Potable Water-Cross-Connection Control Plan
The facilities listing of cross-connections did not list the backflow preventers installed in the OPP chemical dispensing lockers on decks 1, 3, and tween deck. Additionally the backflow preventer on the sink in the deck 1 locker was not listed. It was unclear if the pressure atmospheric vacuum breaker in the hosptital for dialysis connection can be tested. Currently it is only inspected.
Potable Water-Potable Water and Pools/Spas
During the manual testing of the potable water far points on decks 3 and 10 the ship's staff was observed using a glass vial to conduct the free chlorine test in a high range electronic colorimeter. The free chlorine residual according to both analyzers and the inspector's test kit were below 2 ppm. The manufacturer of the colorimeter specifies the test should be conducted only above 2 ppm with this kit, using a plastic vial. Additionally, the staff making the pool tests use the high range device with plastic vials. The inspector's kit measured on two tests 0.87 ppm and 1.30 ppm in one pool, while the ship's staff measured 2.9 and 3.1 ppm in the same pool. They were also observed using a sample volume of 10 ml for the tests when the manufacturer instructions specify only 5 ml.

The drip pan housing had standing water and food debris. In addition, the soda lines were draped inside the drip pan housing, making cleaning difficult.

A waiter had a last symptom recorded as 4/13 at 22:00 and a return to work of 4/15 at 08:15.

The electronic medical log listed a crew member who had 12 episodes of diarrhea and 12 episodes of vomiting in a 24-hour period as a non-reportable case. This individual was treated as a case and was reported to the VSP as a case. This occurred during a cruise in February.

In response to the outbreak, Royal Caribbean Cruise Line and the crew aboard the ship took the following actions:Increasing cleaning and disinfection procedures according to their outbreak prevention and response plan,Making announcements to both notify onboard passengers of the outbreak and encourage case reporting,Collecting stool specimens from ill passengers and crew for submission to the CDC lab,Making multiple daily reports of gastrointestinal illness cases to the VSP,Preparing additional crew members to join the ship mid-voyage to assist with case management and intensified sanitation procedures,Consulting with CDC on plans for: Passenger notification procedures and the planned delayed embarkation schedule in Bayonne, NJ on January 31, 2014,Disembarkation plans for active cases, terminal and transport infection control procedures.One CDC Vessel Sanitation Program epidemiologist, one contract epidemiologist, and one VSP environmental health officer boarded the ship in St. Thomas, USVI and are sailing on the ship as it travels back to port in New Jersey. This team is conducting an epidemiologic investigation, environmental health assessment, and evaluating the outbreak and response activities on board. One additional CDC Vessel Sanitation Program environmental health officer will board the ship upon arrival on January 29 to assist with the evaluation of the disinfection process. The team will continue the investigation and evaluation on the ship thru the boarding of new passengers for the next voyage. Five clinical specimens were shipped to the CDC lab for testing on January 26.
