Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

This room was used for food storage. The deck was vinyl and some of the tiles were lifting up or shifted so there were gaps between. In addition, there were recessed deck areas where the legs of the soda cabinet was installed.

Although the clean storage rack across from the soiled staging area was not being used, the crew member still had to transport the cleaned and sanitized items through this narrow space. The potwash machine was being repaired when the inspector arrived in the area, which accounted for the buildup of soiled items.

The heat lamp built into the countertop had a time control label but was not listed on the time control plan. Staff stated that they did not use the unit for time control. The label was removed.

The coffee machine had been out of order since 22 November 2014. The gluten-free toaster had been out of order since 25 October 2014. Both were waiting for spare parts.

Two small pans of ground beef and ground lamb were stored on top of pans of cubed beef. The pans of ground meat were moved immediately.

The temperature at the handwashing station was 129°F when the faucet was set to the cold side. The water temperature could not be adjusted lower.

The 18 January production records showed the chlorine residual level to be 1.3 ppm 30 minutes after starting production. The correct level was reached approximately 30 minutes later. The 26 October production records showed the chlorine residual level to be 1.5 ppm and pH to be 8.2 30 minutes after starting production. The correct levels were reached approximately 30 minutes later.
Potable Water-Engine Room
The water pipe was striped blue/gray/blue in several places after the chlorine injection point for production.
Recreational Water Facilities-Pump Rooms - Children's Pool & Solarium Pool
Non-continuous pressure-type backflow devices were installed on the hose line connections, and hoses were attached with a spray gun or shutoff valve downstream. The devices were replaced with continuous pressure-type backflow preventers.
Recreational Water Facilities-Fecal/Vomit Accident Records
The 4 December 2014 fecal/vomit accident log documented an accident that occurred in the children's pool, but did not document the type of accident, response steps taken, and free residual halogen level and contact time reached during disinfection.
Recreational Water Facilities-Electronic Data Logs
From the review of the recreational water facility logs, it appeared the electronic data pH logs were sometimes inaccurate in comparison with the manual testing conducted and recorded by staff. For example, the electronic data log for Whirlpool #1 showed the pH to be over 8.0 from 9:38 a.m. to 1:27 p.m. on 20 January when the whirlpool was open. The ship's manual pH readings documented the pH to be below 7.8 during this time. The electronic data pH logs for Swimming Pool #1 showed the pH to be over 8.0 from 2:03 a.m. to 9:50 a.m. on 21 January when the pool was open. The ship's manual readings documented the pH to be below 7.8. No notations were available to indicate why the electronic data logs showed the pH to be over 7.8, or to document work had been done on the pH probe to correct the issue.
Medical-24-Hour Report
The 24-hour report confirmation from the electronic reporting system for the 3-10 January voyage could not be located. The report had been submitted to VSP but the documentation was not found during the inspection.

The ship had documentation from the electronic medical record database showing that the 24-hour (and when necessary the 4-hour report) was successfully sent to VSP but there was no documentation verifying the reports were successfully received by VSP.
Recreational Water Facilities-Shock Halogenation
The shock halogenation records for all three whirlpools listed the starting time but did not list the ending time for the procedure. The ship did have records of the dechlorination process for all three whirlpools that began more than one hour from the start of the shock halogenation.

The light intensity was less than 220 lux around where the area clean dishware was stored. This was corrected during the inspection.

The pulper drain was clogged and standing water was observed in the pulper. Staff addressed the issue immediately and it was corrected during the inspection.

Two live fruit flies were observed on the starboard side of the bar around the deck drain. The pest manager addressed this issue immediately.

Medical-AGE Surveillance Logs
Some of the AGE surveillance logs contained incorrect voyage dates. For example, for voyage 26421 the log stated the voyage dates were 12-19 October, however the actual dates were 12-18 October; for voyage 26422 the log stated the voyage dates were 18-25 October, however the actual dates were 19-25 October; for voyage 26423 the log stated the voyage dates were 25 October - 1 November, however the actual dates were 26 October - 1 November.
Medical-
The medical staff were not paying particular attention to the number of crew members and the voyage dates on the AGE surveillance logs. AGE surveillance logs contained over four times the number of crew members onboard and incorrect voyage dates. For example, the logs indicated the voyages started and ended on a Friday or started and ended on a Saturday. Medical staff showed the itineraries which indicated voyages started on a Saturday and ended on a Friday.
Medical-Crew Members Reporting
A Stateroom Attendant had an onset of AGE symptoms on 22 October at 00:30, but did not report to the medical center until 16:17. His time card stated he worked 12:01-13:02. He received a written warning for late reporting.
Housekeeping-Outbreak Prevention and Response Plan
The ship's Outbreak Prevention Plan (OPP) stated in section 2.2 that when applying chemicals, the crew should ensure guests were not exposed to those chemicals. This was a statement rather than a procedure. The staff also presented a standard operating procedure (SOP) on the fogging process, but that procedure offered no information on protecting passengers from exposure to the fogging chemicals. In the previous VSP inspection it was noted that there was no procedure in the outbreak plan to protect passengers for respiratory or dermal exposure to the disinfectant products used, particularly during spraying and fogging applications.
Galley-Deck 4 - Pastry
Mango mousse was stored in the walk-in refrigerator without any covering. Also in this walk-in refrigerator there was sponge cake, a tray of individual gelatins, berry cobbler, apple pies, and individual cheese cakes all uncovered on the top shelf of their individual storage racks. All items were covered immediately during the inspection.

One live fruit fly was on the deckhead over the preparation counter beside the walk-in refrigerator. At the forward section of the area there were four more live fruit flies on the deckhead.

The interior surfaces of the counter model oven was soiled with food residue beyond one day's accumulation.

During operation of the flight-type conveyor dishwash machine, the final sanitizing rinse spray pattern was not producing a proper pattern and the far right upper nozzle was spraying and stopping frequently. In addition, there was a significant scaling and soiling visible on the stainless steel panel just after the final sanitizing spray arm. Water made contact with this panel and dripped heavily down to the surfaces, including dishware passing below. The build-up on the upper and lower sections of the panel was beyond one day's accumulation.

There was visible soil both on some of the deckhead panels, but also there were accumulated particles of debris in the crevice where two panels join. This was clearly a food source for some of the fruit flies observed on the inspection.

There was no backflow prevention device for the potable water connection for the new juice dispensing guns at the counter.
Galley-Deck 5 - Hot Galley
The handwash station beside the broiler had the waste bin installed on the flat grill side of the basin, where it was near a contact point with the grill.

There were 10 of the total 40 stainless steel serving pans stacked at the preparation counter for filling that were found visibly soiled with food residue or debris. Staff removed these pans immediately to the potwash area.
Buffet-Windjammer - Coffee Stations
There were 5 live fruit flies on the decorative deckhead and bulkhead over the back bar counter. Coffee was in production at the time of the inspection and staff were preparing coffee and trays of plastic cups for filling just before the lunch service. There were 6 live fruit flies observed on the back bar deckhead and bulkhead in the starboard side coffee station.
Other-Solarium Cafe - Service Area
The paper towel dispenser was positioned over the raised shield meant to prevent water from dripping onto the adjacent preparation counter.
Pantry-Schooner Bar
The deckhead throughout this pantry had what appeared to be an oily liquid on the panels and droplets forming in most areas of the space. Although technicians arrived and checked the overhead hatches to identify a source, none was found. When wiped, the liquid was light green in color and bar staff stated they made no cleaning using a product like this.
Medical-Acute Gastroenteritis Surveillance Logs
The acute gastroenteritis (AGE) surveillance logs from voyages 26421, 26422, and 26423 contained over four times the actual number of crew members onboard. For example, for voyage 26421 (12-18 October) the number of crew members onboard was 855, but the logs stated 3721; for voyage 26422 (19-25 October) the number of crew members onboard was 875, but the logs stated 3665; for voyage 26423 (26 October - 1 November) the number of crew members onboard was 867, but the logs stated 4383.

Other-Management and Operations
The management and operations for all the departments such as medical, housekeeping, food and beverage, and engineering were very strong in this inspection.
Medical-Crew Contacts
Medical staff routinely noted when crew members with gastrointestinal (GI) illness symptoms had asymptomatic cabin mates, but there were no notations to confirm that there were no other contacts involved with the crew GI case.
Housekeeping-Outbreak Prevention and Response Plan (OPRP)
The procedures to protect passengers from dermal and inhalation exposures to the disinfectants used, particularly when spraying or fogging were involved, was not found in the electronic OPRP.
Recreational Water Facilities-Strip Analyzer Records
The strip analyzer record paper gives units of 0-100%. According to the staff the 50% mark equates to 7 pH and the chlorine values are divided by 10 to give the actual value (30% on the chart equals 3 ppm). This made for difficulty in reviewing the charts and more when staff attempt to calibrate, which apparently requires the assistance of an electrician. Additionally, the position of the chart pen at the given moment is on the top of the rolling paper, where it's not clear what the precise value is for either pH or chlorine. There are some duplicate records under a data logging system, but it is not located on the upper decks as is the pool mechanical rooms.

Pastry reach-in cooler #2.1.4 had a loose door seal which allowed warm moist air to enter and condense on the upper part of the cooler. Water was noted dripping onto an empty cooler shelf.

An extensive condensate problem was noted in the Soup / Sauce station. Water was leaking from the hood's light fixture frames onto the tops of the closed high pressure cookers underneath.

A single house fly was noted flying through the cold pantry preparation area. No food was impacted during this time because all of it was covered at the time of this area's inspection.

In an active pot washing machine, the final rinse temperature measured in separate trials using a USPHS Thermocouple / and the (Ship's Maximum Registering Thermistor)]: 1. 144°F / (156°F); 2. 158°F / (156° F); 3. 148°F / (154°F); and 4. 143°F / (153°F). The installed final rinse temperature gauge was reading 168°F or less during each of these trials. The crew repaired the pot washing machine and the manifold temperature measured over 180° by the USPHS thermolabel prior to the inspection team leaving the area.

The two ice makers examined had a black residue in the bottoms of all four recirculating chilled water baths.

The Windjammer 'time as a public health control' plan indicated the embarkation day breakfast set up and discard times as 0500 and 0830 respectively. The embarkation day lunch set up and discard times were listed as 1000 and 1600 respectively on the plan. Labels listing the set up and discard times were indicated on the plan for food items placed in the areas such as embarkation lunch period exceeded four hours.Food on a Hot Galley time holding counter was not conforming to the posted ship's 'time as a public health' control plan. At 0855, 25 minutes past the expiration of the embarkation plan and over an hour before the posted embarkation lunch set up period, the following foods were found: mixed condiment tray including sour cream and rico, a sour cream cucumber mix marked as 0815 - 12:15; bag of sour cream marked as 0600 - 1000; breakfast sausage gravy had no label; a gravy marked as 0600 - 1000. Staff discarded all products on the counter.

The sous chef in charge of this galley could not explain to the USPHS inspector and executive chef where these items fit into the time control plan for embarkation day and how he was effectively managing this plan.

The deep fat fryer A and B were missing the hinge area end caps. Electrical wires and difficult to clean crevices were noted in these areas. The missing end caps were found by the staff.

Four very rough cutting boards were noted in the final sanitizing compartment of an active dishwashing operation. The knife scores and grooves made the boards difficult to clean.
Preparation Room-Deck 2 - Fish Preparation
The handwashing sink was blocked by a rolling trash can and a three bucket trolley in the inactive prep room. The hot and cold water supplies had been turned off at the valves below the sink rather than at the sink level. It was initially explained that these supply lines would freeze because the room was refrigerated, but the lines between the valves and the bulkhead were in this same environment. The bucket filling valve was found leaking when the water supply was restored.

A house fly was noted in the area of the crew beverage service line.
Galley-Deck 5 - Ice Machine
The ice maker examined had a black residue in the bottom of two recirculating chilled water baths.
Galley-Deck 11 - Windjammer / Dishwashing
The final rinse manifold was not firmly seated in the machine's sanitizing rinse supply resulting in a large amount of water being discharged through the loose joint. This reduced the flow of the sanitizing rinse through the manifold nozzles.
Bar-Deck 4 - Lobby Bar
The bartender indicated when questioned that he had washed his hands about two minutes prior to the inspection team entering the bar. He pointed to a utility sink in the bar counter as the place he washed his hands. The handwashing sink is installed at the opposite end of the bar. It took over three minutes for warm water to reach this handwashing fixture when the USPHS inspector was washing his hands. The bartender needs instruction on where, when and how to wash his hands.
Recreational Water Facilities-Automated Free Halogen Residual
Electronic data loggers for the Slide Pool and Whirlpool #2 were installed, but were not ready for use. The staff stated that the new data loggers were installed in August 2012, but the installer has yet to return to finish the install. Staff pointed out that they have been in contact with the company, but there are no dates set for completion. In addition, strip recorders were being used to monitor the automated free halogen residuals at the Children's Pool, Whirlpool #1, Whirlpool #3, and at both swimming pools, but they were not being initialed and dated daily. Also, there were no range values on the strip chart to identify the free chlorine residual and pH value being recorded, nor did staff know how to clearly read the charts.
Recreational Water Facilities-
Staff were unaware of the automated free halogen residual testing requirements for the RWFs. Staff did not know how to work or calibrate the newly installed electronic data loggers nor could they identify if they had certified data security features. Also, they did not know how to extrapolate the data from the data loggers when needed or requested. In addition, staff did not know how to properly read the strip recorders or for how long to retain the charts. Staff were also not aware of proper turnover rates for the Children's Pool.
Potable Water-Far Point Records
There were no records to indicate that the far point analyzer values were verified manually or calibrated from 6 November 2012 to 2 January 2013.
Potable Water-Electronic Test Kit
There were no mid-range secondary standards for calibrating the electronic test kit used for calibrating the far point analyzer. High-range secondary standards were available to ensure accuracy between 2.0ppm and 6.0ppm.
Potable Water-Cross-Connection Control Program Log
There was no indication in the log that the vacuum air breakers installed on crew and passenger toilets and showers were being inspected periodically. Also, there were no test results recorded for the reduced pressure assembly installed in the medical room for dialysis machine hook-up. This device was installed in September 2012.

An atmospheric vacuum breaker backflow device was installed on the potable water supply where the dialysis machine connects in the medical center. Due to the higher health hazards associated with dialysis, a reduced pressure assembly or a pressure vacuum breaker device is required. No dialysis machines were connected or present during the inspection.
Preparation Room-Walk-In Cooler
A plastic trolley used for transporting containers of raw meat had meat residue splashed on the walls of the lower shelf compartment. The trolley was immediately removed for a more thorough cleaning and sanitization.
Preparation Room-Centralized Potwashing
A mop was stored hanging on the edge of the mop sink with its handle extending across the adjacent handwashing sink. The mop was removed immediately and taken to the room's cleaning locker.
Preparation Room-Commissary (Veg Prep) and Fish Prep
One small house fly was observed in the area of the prep table of the commissary (vegetable preparation). Another small fly was noted around the prep table in the fish prep room.

The deck near the room's back bulkhead (under the large ducts and behind the cardboard bailing machine) had food debris and a large accumulation of dust indicating that it had been a long time since the area was cleaned. The ducts above were also soiled with old food spills.

The hand soap dispenser valve was broken. Hand soap was seen in the unit through the open top. The entire dispenser was replaced before the inspection team left the area.

The 20 second final rinse cycle on the trolley potwash machine had steam emitting from the last part of the final rinse cycle rather than a hot water spray.Measurement by a thermolabel showed that the manifold temperature exceeded 180°F. The final rinse gauge showed that the temperature exceeded 200°F rather than a maximum of 194°F as specified in the VSP Operations Manual.

A small floor mounted mixer that had been stored clean had dried batter on the wire type mixing blade.

A wiping cloth sanitizing solution was found at 10 PPM. New 100 PPM solution replaced the solution in the bucket.

The bain marie was at a high boil which caused steam to condense on the bottom of the shelf above it. Food in covered pans in the bain marie was not impacted.

Two carbonator backflow preventers had water leaking from the air relief holes. Rust had stained the chrome valves indicating this condition had existed for a long time. Maintenance was called immediately.

Potable Water-Bunkering
The ship was bunkering potable water at the time of the inspection and the potable water was not continuously halogenated to at least 2.0 mg/L free residual chlorine. The free residual chlorine was significantly less than 2.0 mg/L as indicated by the halogen analyzer reading approximately one hour after bunkering started. The amount of free residual chlorine was adjusted by the staff and after several minutes the halogen analyzer measured 6.24 mg/L, the staff's test kit measured 9.7 mg/L, and the inspector's test kit measured 3.97 mg/L. After further adjustment by the staff, the free residual chlorine measured 1.78 mg/L on the halogen analyzer, 1.09 mg/L on the staff's test kit, and 0.76 mg/L on the inspector's test kit. It was also noted during this time that the staff was using the high range level of their test kit when the user's manual stated to use the low range of the test kit when free residual chlorine levels are expected to be 2.0 mg/L or below. After further adjusting by the staff and more than 30 minutes later, the free residual chlorine was measured at 1.28 mg/L on the staff's test kit, 1.19 mg/L on the inspector's test kit, and 1.81 mg/L on the halogen analyzer. At this point, bunkering had been taking place for almost 3 hours. Eventually, after further adjustment and several minutes later, the free residual chlorine level increased to above 2.0 mg/L.

An adult male passenger was found by his spouse in their cabin, unresponsive and with no pulse. An Alpha Code was announced and the ship's medical team responded. They performed CPR, and attempted to revive the pax with an AED, and an IV Line. The passenger briefly regained consciousness and was transported to the medical center. He was later declared deceased due to Cardio respiratory Arrest. The deceased passenger was escorted by his spouse to Funenaria Silva in Ponta Delgada and repatriated to London. Downgraded to Data Collection due to death contributed from Pre-Existing Medical Conditions and Natural Causes.
