Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The interior white tube dispensing ice into the top bin had a heavy accumulation of black slime around the bottom. This was noted on both ice machines.

The 24-hour report for the cruise ending 1 November listed the total crew count as 1645. In addition, the 24-hour report for the cruise ending 28 October listed the total crew count as 1337. According to the medical staff, the total crew count is usually around 860 and there was a problem with electronic medical database system that was creating these reports.

During the inspection, the door to the pantry was pinned open. The bar was on an open deck. The door was immediately closed. The inspector returned to the bar approximately one hour later and the door was pinned open again.

According to the staff, these were only for decoration. A crew entertainer reported to medical with one diarrhea episode and was listed as a non-reportable case of AGE, but the following day experienced a vomiting episode. Staff stated that although they did note the change in condition, they were unable through the electronic medical record to change this person to a reportable case. Since this system also created the official AGE illness report to VSP, this case was not reported.
Medical-I.C.U. Ward Dialysis Connection
A continuous pressure model (9D) backflow preventer was installed on a potable water connection intended for connection by dialysis patients. Staff stated there had been no such patients visiting this ship. The backflow preventer was removed during the inspection.
Pantry-Deck 9 and 10 Ice Pantries
There was visible mold soil along the interior perimeter of the water bath below the cuber panel in the in use ice maker in the deck 10 pantry. There was pitting along the cuber panel where a tool had been used to try to open the cuber side profiles. There was black and pink mold residue in each of the pitted marks and a heavier layer along the top of the cuber. In the deck 9 pantry ice pantry, the cuber flap panel had black mold residue on most of the interior surfaces. The entire underside of the cuber panel was soiled with heavy mold residue, as was the top of the cuber panel. The water bath below the cuber was heavily soiled with a mixture of a black powder and mold. Staff immediately stopped both machines to clean and sanitize the upper ice maker sections.
Recreational Water Facilities-Forward and Aft Pools - Maintenance
The record for the cleaning and disinfection of the hair and lint strainers for the forward and aft pools was documented in the electronic ship maintenance system. This work was scheduled for and performed every three months, rather than weekly as required in section 6.2.1.2.7 of the VSP Operations Manual.
Potable Water-Crew Laundry
Among the listed backflow preventers in the non-testable backflow log maintained in Facilities was the crew laundry machines. Although backflow preventers were installed on the water lines connected to the machines these came from the laundry tank and were technical water lines.
Potable Water-Bunkering pH
There were many days during active bunkering of potable water since the last inspection where the record showed acceptable values for pH, but there was an adjustment listed to change that value. In some cases the recorded adjustment was in the wrong direction and the actual values recorded exceed the limit of 7.8. On 3 June the values began at 1200 with 7.7 and adjustment down, then at 1250 a value of 8.2 with adjustment up, then at 1340 a value of 8.3 with adjustment up, and the final value at 1430 with adjustment up. On June 7 the values began at 0900 with 7.4 and noted adjustment up, then at 0950 a value of 7.7 with adjustment up, at 1040 a value of 8.1 with adjustment down, at 1130 a value of 8.1 with adjustment down, and the final 1220 value was 7.9 with adjustment down. It is not clear if on these tests conducted every 50 minutes that staff were adjusting the pH values or that it was in the correct direction.
Potable Water-Bunkering Chlorination
During active potable water bunkering the reading on the ship's analyzer registered 2.68 ppm free chlorine, while the manual test measured only 1.91 ppm. Staff immediately adjusted the manual chlorination pump and re-calibrated the chlorine monitoring analyzer.

The handwash station was blocked by the hanging waste bin for paper towels immediately in front of the basin and a long coiled deck wash hose connected to the tap under the sink and set immediately in front of the basin.

During the cruise from May 27 to May 31, 2013, a nonfood worker was recorded as a case, but the individual with AGE symptoms was a passenger. According to the staff, they were not able to change the information already entered because they did not have access to the ESeaCare electronic reporting system. The passenger was recorded as a case in a different row on the AGE log.
Medical-MIDRS 24-hour Report
The 24-hour MIDRS report stated there were 3 sick passengers with AGE and 2 sick crew members with AGE. After reviewing the AGE log, there was only one sick crew member with AGE. According to the staff, they were not able to make this change in the AGE log due to the new ESeaCare electronic reporting system.

During the cruise from May 27 to May 31, 2013, the AGE log stated a nonfood worker reported to medical at 8:00 PM on May 27, but according to the medical notes, this person reported to medical at 3:10 p.m. This individual's symptoms started at 11:45 a.m. on the same day. According to the staff, they were not able to access the new ESeaCare reporting system during embarkation day and could not make this change on the AGE log.
Medical-4-Hour MIDRS Report
According to the medical staff, the ESeaCare system generates 4-hour reports to MIDRS even if they are not needed. This should be corrected so that the 4-hour reports are generated and sent to MIDRS only when required per the instructions of the 2011 VSP Operations Manual.
Integrated Pest Management-Active Surveillance
There were no follow-up inspection results when pests were found during active pest surveillance inspections. The action taken during the initial finding was recorded.

There was no procedures in the OPRP on how the crew would be informed of an outbreak.

There were no procedures on how the passengers would be protected from dermal or respiratory exposures when chemicals were used. This was noted during the previous inspection. According to the staff, the proper procedures were followed to protect the passengers from these potential exposures, but they could not be found in the OPRP. Procedures on how to protect crew from exposure to disinfectants were included in the OPRP.
Children Area-Time Control Plan
Food such as pizza and desserts that needed to on time or temperature control were served in the children's center, but there was no time control plan at this food outlet.

Both of the Dora kitchens used by kids 3-5 years old were soiled with a heavy dirt and dust residue. The right Dora kitchen toy also had two pieces of masking tape on it. According to the staff, these were not in use.

Galley-Deck 3 Potwash
There was an excessive amount of soiled pots, pans, baking sheets, and loaf pans staged throughout the soiled landing counters. Upon inspection, over half were items which were previously cleaned which were nested together and stacked on those counters. Surrounding the counters were actually soiled items which were on trolleys, deck stands, and even on top of various waste bins. These items were soiled and surrounded the potwash area to the point of being within one meter of the clean racks at the clean end of the potwash section.

The toilet room was converted to a semi-storage room with stacked cleaning buckets stored on the same deck stand as clean plastic food storage containers. All plumbing related fixtures were removed from the space sometime prior and the door was locked.

There were several entries on the cross-connection control program managed by the facilities department that were missing information about the device type and quantity. For example, the entry for the isolation and changing room only listed "shower/toilet." The entry for provision area only listed "water taps."

Condensate was observed above the tilting pans along the backsplash and in the ventilation hood. Condensate was not observed dripping. Sausages were in the left tilting pan, while the right tilting pan only had water. This item was also noted on the previous inspection.

Material Failure (Vessels)
On 7Nov 2011, Sector Miami was notified via CG-2692 that on 7 November 2011 at approximately 0408, the cruise ship MAJESTY OF THE SEAS has experienced a reduction in main propulsion due to main engine #1's clutch disconnecting. Cause of the clutch failure was due to a broken air pipe. At 0845, the air pipe was replaced and main engine #1 clutch was tested and found to be in satisfactory working condition. Class Society (DNV) was not notified of this incident.

Smoke was reported coming from the port side windlass and an abnormal noise was heard while operating. The windlass was taken out of service until inspection and repairs could be made.

Vessel Maneuverability
30 Sep 2011: Sector Miami IOs contacted Duty person for Royal Caribbean Cruise lines because it was observed that the M/V MAKESTY OF THE SEAS came into port with a tug escort (not normal procedure). IOs confirmed that the vessel had experienced a failure of its starboard shaft bearing which caused the stbd shaft to seize. The incident took place at approximately 0345. Vessel still had 2 rudders, port shaft, and thrusters for maneuverability when coming into port. Arrangements were made to get a tug assist but immediate notification was not made to the Coast Guard. SIO conducted education with the Duty manager regarding the non-compliance of making immediate notification to the CG. Marine inspectors and Chief, Inspections Division attended the vessel and conducted education with the vessel master (and several RCCL personnel) about making immediate notification after dealing with resultant safety concerns of the marine casualty. No Letter of Warning or NOV issued.

Majesty of the Seas was moored in Little Stirrup Cay in the Bahamas for a port visit. An elderly male passenger in his cabin was found not breathing and unresponsive by a member of the ship's housekeeping crew. The medical department was notified and responded. Several attempts to revive the passenger were unsuccessful. The passenger was declared dead by the ship's physician, and the cause of death was from Cardiac Arrest. Downgraded to Data Collection due to cause of death from natural causes.

The M/V MAJESTY OF THE SEAS discharged approx 1 pint of fuel oil into Port of Miami Government Cut a tributary of the Atlantic Ocean. The oil was discharged from the rescue boat when it collapsed during launch. The discharge created a sheen and was immediately cleaned by the responsible party. The rescue boat was then removed from the water and place on land for repairs to be made. The M/V MAJESTY OF THE SEAS was in the prcoess of lowering the port side rescue boat to the water when there was a failure of one of the two of the aft bolts that secure the cage to the rescue boat's hull. The increased strain caused the failure of the second bolt on the aft portion of the cage, which lead to the rescue boat tipping nose up vertically in the air. The crewmember in the rescue boat fell from the stern of the rescue boat to the water, a distance of approximately 3 stories. The crewmember was retreived from the water, taken to the ship's hospital and found to have sustained a few minor scratches to his lower leg. The M/V MAJESTY OF THE SEAS received consent from Class to transit without the port rescue boat. Marine Casualty investigation closed.
