Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Potable Water-Production

Two potable water hoses, connected to a potable water supply, affixed above the chemical tanks for potable water production did not have backflow protection. The hoses were able to reach into the chemical tanks. The hoses were removed, and the potable water line was shortened and closed off.

The handwash station near the emergency exit was missing paper towels. This was corrected during the inspection.

Galley-Deck 2 Galley - Pinnacle Section

Some of the previously cleaned espresso cups and white china spoons on the clean storage rack were visibly soiled with food debris and food residue.

Galley-Deck 2 Galley - Pinnacle Section

A continuous water leak was noted from a deckhead access hatch near the telephone at the forward bulkhead. Water was accumulating on the deck below, but no equipment or food was located beneath the leak.

Galley-Deck 3 Galley - Hot Galley

Significant corrosion was visible across most of the top surface of the previously cleaned range top.

Galley-Deck 3 Galley - Pot Wash

An excessive amount of soiled plastic bins, stacked and on deck stands, was in the pot wash area. These stacks of bins were also stored along the bulkhead directly opposite to and less than one meter away from the #2 and #3 combination ovens, which were in active cooking at the time. The #3 combination oven door could not be opened without impacting the stacked soiled plastic food bins opposite.

Other-Explorations Cafe

The deckhead slot ventilation supply air vent was heavily soiled in the section over the front bar counter near the handwash station.

Bar-Sea View

Rough surfaces were noted, from both peeling and corrosion, on the black painted squares on the deckhead, which made cleaning difficult. The sections over the front bar counter were in the worst condition.

In one corner a branch of leaves from the artificial plant was in contact with a bowl of oranges.

Buffet-Port Side Omelet Station

A pan of cooked but not crispy bacon was on the top shelf in the hot warming unit had a time control discard color dot for 8AM when the inspector arrived at 9AM. This pan was immediately discarded. Another pan of cooked but not crispy bacon was on the bottom shelf but did not have a time control discard label. Staff stated this was a fresh pan of bacon that just arrived but was not labeled yet. A yellow sticker indicating a 11AM discard time was placed on the this pan of bacon.

Buffet-Aft Beverage Station

Artificial plants and bakery items were set-up as decorations in all four corners of the of the buffet and beverage station. These items were not easily cleanable and were on the back of the counter behind the sneeze shield.

Galley-Lido Warewashing

A tray of clean espresso cups had several cups with soil on the exterior side of the cups. The tray was immediately sent to be rewashed.

The dough sheeter belt was missing small sections in several areas that appeared to had been ripped off. A small hole was also present in the center of the dough sheeter belt.

There were four slotted fasteners in the food contact area of the mechanical potato peeler.

Medical-Crew Member Reporting

A Hairdresser had an onset of acute gastroenteritis (AGE) symptoms on 2 October at 05:00, but did not report to the medical center until 14:00. She received appropriate training for AGE reporting, but stated she thought her symptoms were related to motion sickness. She did not work during these hours, and was terminated following her isolation. A Cast Member had an onset of AGE symptoms 31 August at 08:30, but did not report to the medical center until 15:15. He received appropriate training for AGE reporting, but stated he did not report sooner because he was advised to first notify his company manager. He received a written warning for late reporting.

Potable Water-Public Toilets and Showers

The list of backflow prevention devices for the public passenger and crew toilets and showers could not be located.

Children Area-Club HAL

The toilet room for 3-7 year olds did not have a child-sized toilet or toilet seat.

The rim around the ice dispensing chute on the ice/water machine was soiled with a dark material. This was corrected.

A container of whipped cream was not adequately protected by a sneeze shield. It was not for self service, but was located within one meter of where a consumer could stand. According to the staff, side shields had been ordered for this area. The container was immediately moved.

There were two small holes in the deckhead tile behind the ice cream machine. This was corrected.

Buffet-Neptune's Lounge

Grapes were available for self-service in whole bunches, which would require consumers to handle the grapes in order to obtain a single serving.

Potable Water-Far Point Analyzer Chart

No chlorine reading was on the far point analyzer chart on 21 December 2013 from 1100-1430. No notations were made on the chart to explain the absent reading.

Recreational Water Facilities-Hair and Lint Strainer and Housing

From 27 January 2014 to 22 February 2014 no documentation was made for the cleaning and sanitizing of the hair and lint strainers and housings for the 2 pools, 5 jacuzzis, and 1 hydropool. From the last inspection on 11 November 2013 to 26 January 2014 the only notation made on the charts was 'pump inlet strainer cleaned and sanitized.'

Potable Water-Monthly Micobiological Sampling

The monthly sampling since November 2013 included seven samples from Deck 9 and five samples from Deck 2. This does not provide a representative overview of the ship.

Integrated Pest Management-Follow-up Documentation

According to the active surveillance log fruit flies were observed in the main galley pot wash on 1 December 2013, and the main galley, main galley coffee station and main galley dishwash on 8 December 2013. No follow-up documentation was found.

There was an entire rack of gelatin in individual serving bowls uncovered inside pastry walk-in refrigerator #26. Staff corrected this during the inspection.

There was a significant accumulation of a grey colored granular debris in the bottom of the cuber water bath for the in-use ice machine #2.

Dining Room-Tamarind Restaurant - Menu's

Due to the page formatting and the decorative corner covers on each of the individual menu's, it was not clear to see the asterisk ( * ) written at the beginning of the consumer advisory near the bottom of the page to explain what the asterisk's next to specific undercooked fish and seafood items was for.

Galley-Lido Galley - Beverage Station

The three filled juice concentrate containers inside the juice dispenser did not have the plastic lids on to cover. Staff found the container lids and covered each during the inspection.

Buffet-Lido Buffet - Dessert Section

The top trays inside the reach-in refrigerators entering the space had trays of dessert items on the top shelf with no covering. Staff re-organize both refrigerators during the inspection.

Children Area-Club HAL - Child Activity Center

Several of the previously cleaned Lego block toys were found stored in the clean bin and still wet in the void spaces. Staff removed them to air-dry immediately after the inspection.

Food Service General-Culinary Arts Center and Club Hal - Variance Pending

The ship conducts hands-on and demonstration-style food preparation activities for both adult and young guests. A variance was submitted to VSP for these procedures, which is undergoing review.

*General Comment-Records and Operations

The records and operations for the medical center, potable water, recreational water, pest management, housekeeping, and ventilation were very strong.

Galley-Deck 2 Hot and Cold Sections

In the hot section, working containers of diced ham, diced tomatoes, and grilled salmon were not labeled with their 4-hour discard time. In the cold section, working containers of cut watermelon, cut honeydew melon, and cut cantaloupe were not labeled with their 4-hour discard time. The posted time control plans for these sections indicated breakfast service set-up to discard times were 06:00 - 10:00. Staff showed e-mail communication between VSP and the corporate office indicating the time control plans were under review.

Galley-Deck 2 Pinnacle Grill

The milk compartment for the coffee machine was labeled with a blue sticker. According to the time control plan for this milk, the period for the blue sticker was 05:00 - 09:00. At 09:05 the milk was still labeled with a blue sticker. This milk was immediately discarded.

Other-Culinary Arts Center and Club Hal

Staff stated they conduct 1-2 classes per voyage where a maximum of 12 passengers participate in hands-on food preparation activities. These participants were not screened for current or recent respiratory or acute gastroenteritis illnesses. All age groups in the Club Hal actively participate in pizza making, cookie decorating, and mocktails classes. No documentation or procedures were given to the inspector to verify proper public health procedures were being followed during passenger participation in these activities.

Buffet-Lido Starboard Omelet Station

A decorative palm tree was over the omelet station. This tree was moved to an appropriate location.

Preparation Room-Room A.1.01

A trolley of bread was outside the proofing oven. Bread on the top tray of this trolley was uncovered. Staff immediately covered this bread with paper.

Potable Water-Production Charts

The production chart for 5 September indicated water was produced from 02:00 until 08:00 at the correct free residual halogen and pH levels, but after 08:00 the free residual halogen levels dropped to between 1.25 ppm and 1.75 ppm from 10:00 until 02:00 the next day. During this time, the pH levels were between 8.2 and 8.4 with no notations on the chart. According to the documentation provided, the ship was underway the entire day. Staff stated no water was produced, but the water already in the evaporator was switched from potable water to technical water at 10:00.

Potable Water-Cross-Connection Control Log

The cross-connection control log indicated three Watts N9C devices were installed on the deck 9 Green House Spa steam room shower hoses. Upon inspection of this area, all three devices were an Oras type. Also, the cross-connection control log indicated one Watts AP809DV was installed on the bin washing hose in the garbage room. Upon inspection of this area, there were two Watts 9D devices installed on the hot and cold water line to the bin washing hose.

Children Area-Time Control Plans

There were two different time control plans in each of the children's areas including the 3-7 year olds, 8-12 year olds, and 13-17 year olds. The time control plan labeled 'Club Hal Food Service Plan' indicated the set-up and discard times for the Club Hal lunch were 11:00 and 12:30, the set-up and discard times for the kid's and tween's evening snack were 19:30 and 21:00, and the set-up and discard times for the teen's evening snack were 22:00 and 12:00. The time control plan labeled 'Food Safety Plan (daily)' indicated the set-up and discard times were 10:30 and 14:30 for lunch, 19:00 and 23:00 for the kid's/tween's evening snack, and 21:30 and 01:30 for the teen's evening snack.

Recreational Water Facilities-Operations and Maintenance

The records and operations for all of the RWFs were expertly maintained.

The sign advising users to wash their hands after using the facilities was cracked and half of the last word on the sign was missing. The sign was replaced.

The top pan of bread on a trolley by the combination ovens was uncovered. The pans on the trolley were going to be put in the ovens, but food in the ovens was still in the cooking process. The pan was covered.

Galley-Deck 2 Hot Galley Soup and Sauce Station

The piping inside the hood cleaning cabinet was leaking. A work order had already submitted and the ship's staff provided a purchase order for replacement parts.

Galley-Deck 2 Room Service Pantry

A portable table stored in this area had rough edges made of raw, unsealed wood. Staff stated that the table was used for linen folding and was always covered by a cloth before use. The table was removed.

Provisions-Dry Provisions Room 19

Three trolleys with boxed food items and two pallets of bottled water were stored in this room along with several drums, boxes, and containers of chemicals. Staff stated that the trolleys held items for a sister ship and would be offloaded the following day and the water was only stored in the room because the other areas in provisions were over-stocked. The trolleys were moved to a food-only storage area and staff stated that the pallets of water would be moved.

Pantry-Explorations

The dispensing tubes on the two bulk milk containers were longer than one inch. This was corrected.

The wiping cloth was draped over the edge of the bucket of sanitizing solution.

The chlorine concentration of the sanitizing solution in the bucket was less than 50 ppm.

Galley-Pinnacle Grill Dishwash Area

The alarm on the in-use rack-conveyor dishwash machine was flashing and none of the crew members in the area were reacting to the alarm. A crew member was loading dishes into the soiled end and another crew member was removing dishes from the clean end. A senior officer and the inspector noticed the alarm upon arrival in the area and the officer called maintenance immediately. It was determined that alarm flashed when the final sanitizing rinse temperature gauge read between 174 and 180°F. The temperature of the final rinse spray as the plate surface was greater than 160°F.

The cleaning locker was not labeled.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

The electronic AGE surveillance log did not include all the data elements and was not in the exact order and placement as the form provided in section 13.2.2 of the VSP Operations Manual 2011. Additionally, the electronic database could not generate or export the form required from section 13.2.2.

Medical-Reportable Case

The electronic database programming did not use the current definition of an acute gastroenteritis (AGE) case. The database specifically did not permit entry of reportable as a case any individual with less than 3 diarrheal episodes (loose stools) unless the vomiting symptom was marked. A test case was entered with 2 diarrheal episodes, fever, abdominal cramps, headache, muscle ache, and bloody stool and the system determined this was not a reportable AGE case. Additionally, for persons with more than 3 diarrheal episodes the system automatically determined them to be an AGE case, even if there were pre-existing medical conditions which would explain that this numbe of episodes was normal for the individual.

Medical-Case Recording

The electronic AGE surveillance log was programmed to leave a blank in the yes/no column for a symptom where the number of episodes was marked zero It also did not enter a yes value when the symptom field for diarrhea was marked 2 or 1 episodes.

Medical-AGE Surveillance Questionnaire

The questionnaire's used for AGE cases did not include many of the data elements provided and required from the VSP AGE questionnaire provided in annex 13.2 of the 2011 VSP Operations Manual. The question on knowing other persons with same symptoms and if yes please list their names was not included. Did you stay overnight in the boarding city before joining the vessel, and if so what city and what lodging was not included. Mode of travel to the boarding city was not included. Are you part of a tour group was not included. In the meal and activity portion snacks was not listed and activities were only listed as on the ship, not AM or PM, and they weren't listed for the entire 72 hour period working back from symptom onset. There was also no time entered on the meal form. This form must include all data elements provided in the version from the VSP Operations Manual or the violation will be a repeat in the next inspection.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

The written OPRP refers to use of a Virox disinfectant in a 1:16 concentration for disinfection onboard, but another product is actually used on this ship. Additionally, from the review of OPRP there were not specific methods described everywhere for the respiratory and dermal protection of passengers exposed to the disinfection chemicals used, with the exception of the fogging in shops and one case where virox was specified to be wiped on surfaces and not sprayed (super-san cleaning and disinfection).

Children Area-Illness Exclusion Sign

In the signs posted at the outer and inner entry door to Club Hal child center the statement is written that 'As this is a USPH requirement regular charges will apply for all services rendered by the medical facility'. This statement incorrectly implies that the requirement that children with signs or symptoms of infectious illness should be evaluated by medical professionals before being allowed in the facility in some way demands a fee from CDC or the U.S. Public Health Service. The decision to demand a fee for medical evaluation is solely and completely a corporate decision by Holland America Line and in no way is required by the CDC or the Vessel Sanitation Program. Reference to the U.S. government with regard to such fees should be removed from this and all such signs in the child activity center or anywhere else on the ship.

Recreational Water Facilities-Whirlpool Spas

There was no monitoring of flow rates in the whirlpool spas as there are no flow meters installed.

There were no automated pH and bromine analyzers or chart recorders installed to a sample line from the existing return lines or as a separate line at each whirlpool spa tub and before the compensation tank to monitor and record pH and halogen levels in each whirlpool spa.

The various pools, whirlpool spas, and hydro pool safety and health warning signs were missing some or all of the new elements required in sections 6.7.1.1.1 and 6.7.1.1.3 in the 2011 VSP Operations Manual. Staff stated the new signs were ordered.

Food Service General-

The nonpotable chilled water lines connected to the back of the ice machines and ice cream machines in food areas throughout the ship were not uniquely identified. (2011 VSP Operations Manual)

Other-Cushion Store L.9.2.03

Two bottles of liquor were stored directly on the deck of this locker. There were napkins soiled with food debris also stored in this locker. This area was also not finished in accordance with sections 7.7.4 and 7.7.5 of the VSP Operations Manual for food storage areas.

Buffet-Port Salad Line

There was no serving utensil for a lettuce container. This buffet line was open for service at the time of the inspection.

The dipper well was continiuosly leaking.

The handwash station located next to the scraping area where a crew member was prewashing plates was getting wet and soiled with water from this prewash. The paper towels were also wet.

There was a pan of raw ground beef stored next to a pan of ready to eat fish cake mixture in undercounter refrigerator # 3.15 A. The pans were repositioned in different refrigerators.

Dining Room-Pinnacle Grill Restaurant

A portable induction unit, a rolling cart, and a portable waiter station had rough surfaces and raw wood, making them difficult to clean.

Potable Water-Cross-Connection Control Program

The backflow prevention devices installed on the 2 Natura water purification systems onboard appeared not to be vented devices.

Galley-Deck 2

The deck drain behind the left ice machine had a few inches of pooled water and was not draining.

Buffet-Crew Mess

There was no serving utensil for a hotel pan of vegetables. This buffet line was open for service at the time of the inspection.

Food Service General-Lighting

The light intensity behind and around many pieces of deck-mounted equipment was less than 110 lux, such as behind and around the ice machine of starboard Lido pantry, behind and around the combination oven of the crew galley, and behind and around the ice machine of the pantry of the Ocean Bar. In addition, the light intensity over the handwash station of the Ocean Bar could not be raised to 110 lux. (2011 VSP Operations Manual)

Preparation Room-

The butcher was cutting beef at the time of the inspection. He had a pronunciated mustache and was wearing no facial hair restraint.

The interior of the door lid to the ice bin of the ice machine had a cracked stainless steel profile strip that created a sharp edge and a difficult to clean seam.

The milk in the milk refrigerator of the espresso machine was on time control according to the staff. Even though the door of the refrigerator was identified as a time control unit, the milk was not identified with a discard time. The time control plan for this area instructed to place a green dot on the milk container inside the refrigerator indicating that it had a set up time of 1700 and a discard time of 2100. The time control plan was not followed.

Food Service General-Consumer Advisories

The menus for the Pinnacle Grill, Le Cirque, main dining room and Tamarind had consumer advisories, but did not identify with asterisks the animal derived foods that could be served undercooked or raw. (2011 VSP Operations Manual)

The management and operations observed in all the hotel, food and beverage, facilities, housekeeping, and potable water departments were very strong today.

The light intensity behind and around the milk dispenser in the pantry next to the bistro line was less than 110 lux. In this same area, the light intensity in the area between the bulkhead and side of ice machine was less than 110 lux.

The poured epoxy decks in the main galley were pitted and rough in several areas. These decks had been renovated during the April 2011 dry dock and were in the process of were being repaired. The decks had been repaired in the deck 2 garde manger, potwash, and hot galley, and the rest of the galley decks were scheduled to be repaired during the upcoming cruise.

The menus for the Pinnacle Grill and Le Cirque did not have asterisks at the animal-derived foods that could be served raw or undercooked and a footnote stating: 'consuming raw or undercooked meats, seafood, shellfish, eggs, milk, or poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.' The menus for the main dining room and Tamarind had the footnote, but did not have asterisks.

Several cases of bottled water and individual bottles of water were stored on the deck in the office outside of the deck 2 main dining room entrance. According to the staff, this water was for sale during meals.

Medical-72-Hour Questionnaires

Several 72-hour self administered questionnaires on file for passengers and crew included information for breakfast, lunch and dinner, but did not include or specify meals on the day of illness onset, the day before illness onset, two days before illness onset or three days before illness onset.

There was gray powder along the length of the black plastic water return tray immediately below the ice cubers of the ice machines in the pantries near cabins 6091 and 6063.

The manufacturer-provided letter from Culligan indicated that the main drain covers for the two main pools conformed with ASME A112.19.8-2007. However, there was not enough information in the letter to determine if the drain covers were custom constructed/field fabricated or if the covers were manufactured drain covers tested by an internationally recognized testing laboratory and stamped according to the standard. These pools had gravity drainage and multiple drains more than 3 feet apart.

A number of soy sauce dipping dishes and sauce bowls that were stacked together were still wet and some dripping.

Galley-Bakery / Pastry Walk-In Cooler

There were water drops dripping from the deckhead panel next to a sprinkler. Trays of cups with pudding were underneath and some of the puddings were impacted by the dripping. The trays were moved to another cooler and food products in the impacted tray were discarded.

Galley-Pinnacle Restaurant

The water in the sanitation bucket had a concentration of free halogen that was less than 50 ppm. This area was in operation during the time of inspection. The water was changed.

A number of clean plates that were stacked together were still wet and some dripping

A hotel pan with cut peppers had no serving utensil. This was corrected.

Provisions-Dry Stores - Liquor

The deck under the metal racks was soiled with dust and pieces of wood. The staff stated that the pieces of wood came from wood pallets taken inside during provisioning.

A syrup-like liquid was spilled on the deck next to fresh pineapples. The spill was cleaned.

Provisions-Dairy Cooler

There was a milk spill below the pallet having boxes of half pint milk containers. The staff identified the box causing the spill and removed the damaged container. The spill was also cleaned.

Preparation Room-Butcher Room

There was water dripping from the deckhead intake vent to the passageway next to a preparation table. The area was not in use at the time of inspection. The staff explained that this issue was going to be corrected in the next dry dock.

There was water dripping from the deckhead. No food was impacted. The staff explained that this issue was going to be corrected in the next dry dock.

Provisions-Parasite Destruction Records

The letter declaring that the pickled herring was parasite free did not state that it was frozen at a certain temperature for a specified period of time enough to claim parasite destruction. This issue was noted in the last inspection but for another herring product from a different supplier.

Medical-Gastrointestinal Illness Records

The GI illness surveillance log for the last 5 cruises had a number of passenger entries for over the counter anti-diarrheal illness sales with empty data (no yes or no completed) for all symptoms other than diarrhea, vomiting, and fever, and for those symptoms the logs entries for those passengers was no. The field for pre-existing medical conditions had one of the following comments: OTC sale only refused to be seen, or declined medical treatment, refused exam. Staff stated that the questions specific to GI illness symptoms were not asked of these passengers per company directive, and the no answers in the log form under diarrhea, vomiting, and fever were automatically populated in the electronic program for the form completion.

Potable Water-Microbiological Analysis

Staff were not following the procedures for adjustment of the test water samples for microbiological analysis of potable water in bunkering and distribution, and the company log form did not specify these procedures. The new fecal coliform test kit (Colilert-18) specifies adjustment of the test water sample to a temperature of 33-38 °C using either a water bath of 35 °C for 20 minutes or 44.5 °C for 7-10 minutes. Following that the sample was to be incubated for 18 hours. The company form specifies incubation for 24 hours under point 6 in the method, but does not address adjustment of the water sample temperature.

DO NOT USE (whirlpool/spa)-Aft Lido Spa

The manual tests of the whirlpool spa revealed free bromine residuals of 2.54 and 2.63 ppm (mg/L) on the inspector's test kit and 3.15 and 2.7 ppm on the ship's test kit in multiple tests. A passenger was using the spa just before the testing was conducted. The spa was closed immediately to adjust the bromine residual.

A staff member was observed taking a trolley of various breakfast foods out of the galley with all containers or foods covered except the small creamer pitchers. Additionally, several prepared and ready-to-eat breakfast foods were found uncovered inside the pass-thru glass door refrigerators.

the in-use conveyor dishwashing machine had a displayed rinse tank temperature of 163 °F, while the manual test of the rinse water was only 152 °F. The manufacturer's dataplate specified a minimum rinse tank temperature of 160 °F.

Some of the previously cleaned bowls stacked on the clean storage shelves were found soiled with food particles and food residue. One of the cleaned plates stored beside was also soiled with food residue.

There were small piles of a grey granular material present in the ice maker water bath on both Scotsman ice machines. It was unclear where this product was coming from or what it was.

The letter declaring that the pickled herring (roll mops) were parasite free listed processing at temperatures from 30 °C to -30 °C.

The diamond plate stainless steel deck in the butcher shop walk-in refrigerator and the fish preparation room flex significantl when walked upon and may later break at the welded joints if not better supported from below.

Food Service General-All Beverage Stations with Ice and Water Dispensers

The Scotsman model ice and water dispensers have a serious of open voids on the underside front panel at the water dispenser and across to the ice unit which make cleaning difficult and provide insect access/harborage.

Preparation Room-Butcher Shop

The center deckhead light fixture at the air vent was collecting water, with some dripping down to the deck below.

There was an absorbent raw wood panel on the underside of the back bar counter at the neurtral counter.

The manufacturer's dataplate specified a minimum wash tank temperature of 150 °F. Additionally, it was unclear if the machine was meeting the required 11 second final rinse consistently.

Galley-Lido Dishwash Area

The in-use conveyor dishwash machine had a rinse tank temperature of only 155 °F, and displayed 163 °F on the machine. The final rinse manifold temperature displayed was 209 °F, while the plate level was 160 °F after several attempts. Additionally, a thermal label used on the final rinse spray arm did not fully reach the 180 °F temperature, indicating the thermometer was malfunctioning.

Housekeeping-Spa Changing Rooms

The changing rooms had toilet rooms inside. There was no means to exit the outer doors of the changing rooms without touching the door handle with bare hands. There were tissues and a waste receptacle inside the toilet room, but not for the outside doors.

Personnel Casualties : 1 injury

26 Oct 14: Employee was working on a lifeboat from a raised catwalk. While conducting work, crew member lost situational awareness/miss stepped, and fell through a catwalk opening for a ladder to the deck below. Person fell 3.55 meters and was wearing appropriate protected equipment for his work at the time. Employee was taken by ambulance to local hospital for x-rays. X-rays determined that the employee had fractured 3 ribs. Employee was provided pain medication and placed off duty until 28OCT14.

6 March 2012: Sector Miami received a CG-2692 notifying of a U.S. passenger on board the MV EURODAM who, on 4 March 2012 (09:45) fell and sustained a fracture of her left upper arm. Passenger was medically disembarked in Grand Turk on 5 March 2012. No post casualty drug or alcohol tests were conducted on the ship's crew as no actions or inaction by the crew was found to have contributed to the occurrence of this casualty (non-marine operation).

Personnel Casualties : 1 injury

4 March 2012: A CG-2692 was emailed to Sector Miami notifying of a passenger injury on board the MS EURODAM. This passenger injury occurred at approximately 1400 on 3 March 2012 when the 76 YOM passenger was descending a staircase from Deck 2 to Deck 1. Three steps from Deck 1, the passenger fell and landed on his left knee and as a result, fractured his left femur. Passenger was medically disembarked from the vessel prior to its departure (in Port Everglades, FL). No drug or alcohol testing was conducted on the crew as no action or inaction by the crew contributed to the occurrence of this marine casualty.
