Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Galley-Grill Station
The automatic hood cleaning system was filling the food preparation counters with water and soap that was also dripping on the deck. After the finding, maintenance staff tried to find the root cause of the malfunction.
Medical-Immediate Contacts of Symptomatic Crew
Staff stated that crew members could share a bathroom with up to 14 other crew members in the crew corridors. Staff explained that when a crew member experienced acute gastroenteritis (AGE) symptoms, they only followed up with significant others and cabin mates. The current inspection found no symptomatic crew members in the past 22-day crossing voyage from Europe. The day of this inspection was the first U.S. port since departing from Europe.
Potable Water-Striping
The water lines were striped blue/green/blue, indicating potable water, after the reduced pressure zone backflow preventers for the pool filling connection in the engine room, the supply lines in the forward pool machinery room, and the water supply for the garbage room.
Ventilation-AC Station 4.4
A layer of water was noted in the condensate collection pan of AC unit 4.4-5, and was unable to drain. Staff said this could be due to a clog in the drain line. The condensate collection pan of AC unit 4.4-2 was not visible through the sight window and equipment access for evaluation was impractical.

Staff stated that food employees could go to their cabins but the crew cabins were located several decks below. Staff added that in case of a real need, food employees could use passenger toilet rooms which were not that far away from the galley.

A hotel pan with exposed meat was on a nearby counter and appeared that is was not impacted. Even though no employees were at this station at that point in time, many employees, including area supervisors, were at other stations immediately next to this one showing no concern. After the finding by the VSP inspector, the employees moved the meat to another section and closed off the area. During the final meeting, maintenance staff stated that the root cause of the water and soap on the counters was still unclear.

Medical-Crew Member Acute Gastroenteritis (AGE) Reporting
A beautician had an onset of AGE symptoms on 13 May at 1100 but did not report to the medical center until 14 May at 0750. This crew member worked while symptomatic. The crew member was given a written warning. On the previous inspection an assistant steward worked while symptomatic with AGE symptoms.
Medical-Special Report
The vessel did not submit a special 2% report to the Vessel Sanitation Program (VSP) when the vessel reached 2% of reportable case passengers with acute gastroenteritis on the cruise dated 2-17 May. Although the cruise ended in a foreign port, the vessel was within 15 days of a US port at the time the vessel reached 2%. A telephone notification to VSP was also not made.
Potable Water-Production in Amazon River
The vessel produced potable water while sailing in the Amazon River every day between 13 and 20 March 2014. This was confirmed by cross-referencing the production records and vessel itinerary.

The coving at the deck/bulkhead and deck/cabinet junctures were missing or broken in several locations. In addition, there was a piece of broken bulkhead tile near the deck under the utility sink.

During active use of the glasswash machine, the final rinse gauge indicated the temperature was 140-143°F (60-62°C). Staff stated the sensor was positioned away from the nozzle. This was corrected. The inspector and crew member obtained appropriate manual readings of the final rinse temperature.

There was excess condensation on the inside final exhaust compartment of the in-use dishwash machine. No condensation was observed dripping onto clean dishes as they exited the machine. A crew member wiped off the condensation and allowed the machines to continue operation to see if additional condensation built up. Within a few minutes, condensation was beginning to form again.

At the three-compartment sink, the faucet for the rinse and sanitizing compartments was leaking.

Staff discarded the food as a precaution, the entire cooking line was closed down, and technical staff was immediately called to repair the area.

Medical-Crew Reporting Acute Gastroenteritis (AGE)
During Voyage #773, a state room attendant reported an onset of AGE symptoms on 9/30/2013 at 2300, but did not report the illness to the medical staff until 10/03/2013 at 1620. The attendant worked while symptomatic. The attendant was released from the company because of this lapse in reporting.

A tray of soiled dishes collected from the passenger cabins was found on the deck in front of the dishwashing machine. The soiled dish counter was completely full.

Potable Water-Engine - Incubator
The shore potable water sample was collected as part of the microbiological test for potable water bunkering and found inside the incubator. The incubator thermometer reading was 50.9 °C and the control knob was turned far past the marked set point. Staff manually adjusted the incubator to 35 °C as required by the test kit manufacturer.
Potable Water-Tank Maintenance
On 11 July, 2013 potable water tank 3 starboard underwent maintenance, which included cleaning/disinfection and coating. In the written record there was no free chlorine residual value when the tank was refilled following disinfection and there was no drying/curing times listed. There was no statement that the tank coating was applied, dried, and cured according to the specifications of the manufacturer.

The comprehensive list and inspection results for the non-testable backflow prevention devices noted a non-continuous pressure device on the potable water supply pipe to the ice machine in the Explorations Cafe', but when inspected it was found to be a continous pressure model.

In both pools (Lido and Aft) the mounted floatation devices did not have a length of rope attached.

The #3 ice bin on the port side of the front bar has been in disrepair and not working at all since 5 March, 2013. There is a repair order for the unit, which is used for holding juices for mixing according to staff.

The glass sneeze shield near the double range and above the cold well was chipped in several areas as well as an approximate 5 inch crack that appeared to be splintering. This area was directly above food display and was not easily cleanable.

Galley-Dishwash
A constant water drip was coming from a corner of the deckhead head hatch near the clean landing of the rack-typed dishwash machine. The hatch was opened and an accumulation of rust build-up was observed on the hatch where the water was dripping from the pipe above. According to staff, this leak was caused by poor insulation around the chiller lines. The amount of rust build-up on the hatch indicated that the issues has been ongoing.
Food Service General-Decks
Deck tiles throughout the food service areas had missing or recessed deck grouting, broken deck tiles, or missing deck tiles. This was especially observed in the Lido dishwash, Garde Manger, Pastry/Bakery, Main Galley potwash, Main Galley dishwash, and Main Hot Galley.

The plastic lid to the port side ice machine bin had an approximate 3 inch lengthwise crack that prohibited the area from being easy to clean. Sealant had been used to fill in the open crack, but most of it had been worn away. According to staff, a new lid was on order.

Light intensity to the right of the newly installed counter mounted juice machine was less than the required 110 lux of light.

A constant water leak was coming from the corner of the deckhead hatch 3.8.1 and landing on the deck. Staff were unsure where the water was coming from after an initial inspection inside the hatch. No clean items or food was impacted.

A cardboard box of sole fish stored in the back left corner had an approximate 1/4' layer of ice on the top surface. The deckhead above where the box of fish was stored, had a heavy build-up of frozen water droplets indicating that the area was above temperature for an extended period of time. The box of fish was discarded.

The far left final rinse spray nozzle to the in-use conveyor glasswash was not producing an effective spray pattern.

The reduced pressure assembly for the hood cleaning system in cabinet MK-16 had a steady water leak coming from the relief vent indicating possible malfunction.

Galley-Main Galley - Breakfast Pantry
Condensate water was dripping from the evaporator pan from both the left side and the right side onto shelving and bottom of the reach-in cooler #703. No food was stored in the unit at the time of the inspection. It was found that the sliding doors on the pantry side were both not on the track and the gasket on the right door was apparently misaligned. It was found during the immediate repair that the wrong gasket had been installed. The passage side left sliding door also had an improperly-seated gasket. Moist air from the galley was entering the unit through the loose door seals and condensing on the top of the unit above where the food would be later stored.

The milk units 1 and 2 on the coffee machine were listed on the time control plan. The units were not labeled 'Time Control'.

The electrical cable to the top grill unit on the large grooved grill had rubber cover missing on the lower portion of the cable exposing braded metal shielding which created a difficult to clean surface. Replacement of this cable was instituted while the inspection was still in progress.

More than 10 inches of profile strip was loose at the bottom of the bulkhead panel below the flat grill that created a difficult to clean area.

A large deck-mounted vertical food processor had a large amount of food soil in the interior around the shaft, on the top shaft threads and on interior cover. The staff reported that the unit had been out of service for a long period of time and was due to be removed from the ship. Galley crew cleaned the unit immediately.

Two platters of cut fruit including cantaloupe, honey dew melon, and watermelon were found in a large time control reach-in cooler at 10:50 AM. The staff explained that the fruit was going to be used in the officers mess that has a time control plan that starts at 10:30 AM, but does not specify this unit as a part of that time control plan. The measured starting temperature of these fruits was 49°-51°F or about 10° F higher than allowable for cold potentially hazardous foods entering the time control period. This food was immediately discarded by the staff.

Port-side conveyor dishwashing machine had one blocked final rinse spray nozzle on the upper manifold that limited the exposure of the clean ware passing under it to the final rinse spray.

The milk unit on the coffee machine was listed on the time control plan. The unit was not labeled 'Time Control'.

The menu posted outside the restaurant did not include the asterisk to indicate the potentially hazardous food that can be ordered or prepared undercooked. This item was noted on the previous inspection report.

A leak was noted from a deckhead access panel frame inboard from the clean landing of the conveyor dishwasher. No clean dishware was being impacted at the time of the inspection.

A profile strip at the bottom of the bulkhead behind was loose that created an approximately 18-inch difficult to clean gap.

An uncovered partial tray of desserts was stored on top of an out of service ice cream dispenser adjacent to the service counter. An uncovered rack of other desserts was in the adjacent passageway/pantry. Soiled service items were also stored in this area.

Handwashing sink on the outside service counter did not have a towel dispenser or a waste receptacle. Crew were having to go into pantry to get a paper towel and use the receptacle there for disposing of towels before returning through the closed pantry door to the service counter.

Access to the handwashing sink was blocked by the open door. The door to this area was open prior to the inspection team arriving at the bar.

Hood cleaning system backflow preventer intermediate atmospheric vent was plumbed through a fitting into a small copper pipe which discharged into a flexible tube that went through a hole in the bottom of the technical cabinet. This installation did not allow determination of the failure of the backflow preventer and could actually lead to a potential backsiphonage problem. Rubber hose was pulled up into the bottom of the cabinet re-establishing an air break, as a temporary fix of this problem.

Several bunkering records during the month of October and September did not indicate the time bunkering started, and therefore, it wasn't clear whether the free halogen residual reached 2 ppm within 30 minutes from the start of bunkering.

During active potable water bunkering, an active sewage discharge hose was resting on top of the active bunkering water hose. Both process were being conducted at the Port Aft bunker station.

The backflow prevention devices in the deck 4 housekeeping chemical locker were not listed on the cross-connection control program. A continuous pressure backflow device and approximately 12 chemical dispensing units with internal air gaps were located in this room. Also, the plan indicated that the RPZ on the potable water supply line to the garbage room needed to be replaced. The log indicated that a test completed on 1 September 2012 yielded a failing pressure differential, yet the RPZ was still in place at the time of the inspection. According to staff, a new RPZ has been ordered.

Recreational Water Facilities-Antientrapment Drain Covers
The Lido Center Pool had 2 suction drains that did not meet antientrapment requirements. The suction drains were less than 3 feet apart, only 1 of the drain covers was ASME A112.19.8 compliant, and there was no safety vacuum release system or automatic pump shut-off system. Also, one of the suction drains at the Aft pool did not meet ASME A112.19.8 compliance. These suctions drains were greater than 3 feet apart, and therefore, no secondary antientrapment requirement is necessary.

In reviewing the gastrointestinal (GI) illness logs since the previous VSP inspection, there was an automatically populated no answer in the fields for diarrhea, vomiting, and fever from the electronic program used for recording these illness reports. The form populates a no answer under diarrhea even when one or two episodes of diarrhea are reported. According to staff the diarrhea field only changes to yes for that symptom when the number of episodes reached is 3 or higher, and they cannot manually change the no to yes.

There was no visible backflow prevention device on the potable water pipe supplying the bedpan washing unit in the handicap toilet. Additionally, the ward room 1and 2 are used for hemodialysis patients and a backflow preventer was installed on the potable water supply pipe for ward room 2. That preventer was not a type sufficient for the health hazard presented. Ward room 1 had only a preventer for the flexible shower hose at the bath tub.

There was no airtight, washable waste receptacle in the child toilet room for disposing of excrement, soiled sanitary wipes, and gloves which cannot be disposed in the toilet.

This is not protective against the anticipated pathogens for loose stool fecal accidents. The correct value is now 15,300. The ship was given a copy of the CDC Healthy Swimming Guidelines on this change.

Potwash and dishwash machines were erratic in operation after potable water system was restored after major water system leak was repaired. Monitor all machines closely during this voyage to assure proper operation and temperatures.

During major water system leak repair, limited food preparation activities were initially continuing without plans for providing handwashing facilities. When water outage continued longer than initially planned, the lack of alternative handwashing facilities became a critical food safety issue. Upon recommendation of the USPHS EHO inspection team, beverage dispensers filled with warm bottled potable water were temporarily set up adjacent to the installed handwash facilities in food preparation areas that were being used. Food service operations were suspended until the water system was repaired.

According to the medical staff, a part-time doctor was on board during this cruise.

There were times when the pH levels were above 7.8 on manual tests and on the analyzer tests. There was no action taken to lower the pH or close the whirlpool spa.

There were numerous occasions when a calibration test was performed and the analyzer levels were not adjusted to match the manual test result. On these occasions the analyzer was showing levels below 4.0 ppm free residual bromine and the manual tests showed levels above 4.0 ppm.

There were occasions when the readings on the charts showed less than 1.0 ppm free residual halogen. Calibration checks were performed which showed levels above 1.0 ppm but no adjustment was made to the analyzer.

The reading on the analyzer-chart recorder was maxed out at 10 ppm on 5/30. Only two manual tests were conducted between 9 a.m. and 10 p.m. On 5/12 the reading on the analyzer-chart recorder was maxed out at 10 ppm between 8 a.m. and 5 p.m. Manual tests were performed but there were no levels recorded for these tests.

There was no free halogen recording on the chart on 4/7 and no manual tests were performed.
Housekeeping-Outbreak Prevention and Response Plan (NLV Protocol)
There were public areas in the plan that were to be sanitized on a less than continuous basis when the cumulative proportion of gastrointestinal illness cases in passengers or crew reaches 2%. The passenger self-service laundry was to be sanitized every hour. The handrails, handles, elevators and landings were to be sanitized every 2 hours. The public rooms were to be sanitized 4 times per day. The public phones were to be sanitized every 2 hours. The crew laundry facilities were to be sanitized 4 times per day. the crew handrails and handles were to be sanitized every 3 hours. There was no frequency specified for disinfecting/sanitizing crew public toilet rooms. The crew internet cafe was not addressed in the plan.

Lauderdale, FL on March 28, 2014, including providing additional cleaning crew to complete a thorough public and accommodation super-sanitization cleaning and disinfection,planning staged disembarkation for active cases to limit the opportunity of illness transmission to well guests, andplanning for sanitation of terminal and transport infection control procedures.Three CDC Vessel Sanitation Program environmental health officers will board the ship on arrival in Ft. Lauderdale on March 28, 2014 to conduct an environmental health assessment and evaluate the outbreak and response activities. Stool specimens are being collected and will be shipped to the CDC lab for testing.

On 17AUG14 an off-duty crewmember, working for the hotel and entertainment staff, tripped and fell while intoxicated injuring themselves on the cruise vessel MAASDAM. The vessel was underway in the Gulf of Maine at the time of the incident en route to Bar Harbor, ME. The crewmember was treated on the vessel and later taken to the hospital when the vessel entered port in Bar Harbor, ME.
