Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Cheese and several containers of milk were stored in this refrigerator and were below 41°F. It was later determined that the compartment was mislabeled and was on temperature control.

Staff reported water was produced in the Amazon River during the 26 February-4 March and 30 November-9 December voyages. Water production records were reviewed during the inspection and the production during the six day roundtrip cruise into the Amazon River was verified. This was noted on the previous inspection.

Medical-4-Hour Acute Gastroenteritis (AGE) Report
A passenger reported to medical on 25 November with 2 episodes of vomiting plus a fever and was deemed a reportable case. The ship was returning to Port Everglades on 26 November and had already submitted the required 24-hour AGE report. However, a 4-hour report was not submitted, even though the case reported greater than four hours before the ship arrived in port.
Potable Water-Spa Foot Bath
No backflow prevention device was found on the potable water supply line to the foot bath.

On the previous voyage the ship sailed through the Amazon River from 5 December to 11 December and produced water every day. The cruise began in Ft. Lauderdale, FL on 26 November and ended in Ft. Lauderdale, FL on 22 December.

The lido whirlpool spa and the port aft whirlpool spa were shocked on May 8 and 13, 2013, but the halogen residual levels were not documented at the beginning or end. The documentation stated a halogen residual of 10 ppm during the shocking of both RWFs.
Integrated Pest Management-Active Surveillance Log
The active surveillance log did not indicate what the follow-up inspection results were for pests that were found during an inspection. For example, fruit flies were found in the garbage room on April 14, 2013, but only the follow-up date was recorded as May 2, 2013 and the action taken stated monitoring. According to the staff, no fruit flies were found during the follow-up inspection, but this was not documented.

Finally, the light attached to the deckhead above part of the ice machine had several dead small flies inside the light cover and the light was not properly sealed to the deckhead.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)
The OPRP did not state how to notify crew there was an outbreak onboard. The documentation provided to the inspector was only for guests.
Food Service General-Light Intensity in Food Areas
The following areas had light intensities less than 110 lux behind or around mounted equipment: bakery behind the stack oven, deck 4 center hot galley between the two combination ovens and around the single Rational combination oven, deck 7 hot galley behind the two combination ovens, lido hot galley behind the Rational combination oven, behind the ice machines in the lido bar pantry and java cafe pantry, and behind the espresso machine on the back bar counter on the java cafe bar. There was an ongoing light installation which was not complete in all areas.

There were new deck tiles in place or renewed decks in some of the food areas, but the following places had recessed or missing tile grout, or broken or cracked deck tiles: deck 5 galley pastry, potwash, and butcher preparation room.

Several small holes were noted in all galley deckheads, particularly where rivets connected panels or profile strips were missing.

The deck tiles were cracked and damaged in nearly all food preparation areas. This included the bakery/pastry, deck 4 galley, deck 7 galley, and bar pantries. Work had been done in some areas to begin installing new deck tiles. In the deck 7 galley hot section, most of the newly installed deck tiles were cracked.

In the pots and pans soaking bin, there was a grill brush with an open ended handle, which made cleaning difficult.

The heating stones were cracked inside the second to top unit, making cleaning difficult. A job order had been placed on 19 December 2012. Also, there were slotted fasteners inside all units of the deck oven and some of the fasteners were missing. Work was to be completed to replace the slotted fasteners by March 2013.

The top of the vertical bulkhead profile strip was damaged and loose on the left side of upright refrigerator #7.32.

Potable Water-Microbiological Testing
The vessel had expired color comparators for analyzing the results of the potable water testing method Colitag for microbiological monitoring.
Potable Water-Tank Maintenance Records
Forward potable water tanks 320 and 321 were touched-up with an approved coating system in December 2012, however, there was no written documentation that the manufacturer's recommendations were followed for application, drying, and curing. In addition, aft potable water tank 1410 was inspected in November 2012, however, there was no written documentation on the free halogen residual value before the tank was put back in service.
Potable Water-Cross-Connection Control Program
The cross-connection control program incorrectly described the Watts N9 backflow prevention devices as pressure vacuum breakers. This was also noted in the past two inspections.

The pressure vacuum breaker installed at the bin wash in the garbage room was not included on the testable device list. The staff was unaware that the pressure vacuum breaker was a testable device and only visual checks were performed.

The turnover rates and bather loads for all recreational water facilities were calculated using the manufacturer's flow rate of the pumps and not actual flow rates measured by flow meters. This was noted in the previous inspection. Work was to be completed by March 2013.

The granular filter media of the whirlpool sand filters was last changed on 12 November 2011, more than 13 months ago.

The distillate water lines from the evaporators to the potable water halogenation station had sections where they were either painted completely blue or painted blue with blue/gray/blue stripes. This identification method is not in accordance with ISO 14726 standards.

The whirlpool safety signs stated that children under 14 require adult supervision. However, the clarifications page to the 2011 Operations Manual indicated that those under 16 years of age are considered children.

This was written on the previous inspection. According to the staff, equipment had been ordered for the deck 4 galley and electricians were developing a plan to install additional wiring. In addition, a light meter had been ordered and a lighting survey will be conducted.

The person in charge of the RWFs had a difficult time answering basic public health questions. Staff told the inspector they used all sand filters, but upon inspection only two sand filters were used and the rest were cartridge. Also, staff did not know the purpose of collecting the monthly sedimentation samples from the sand filters or how to read the results. Finally, staff had a hard time finding the sample lines and cross-connections in the pool machinery rooms.

There was no case identification number on the AGE surveillance log and the items on the log were not in the exact order as required in Annex 13.2.2 of the 2011 VSP Operations Manual. Also, the reportable definition for diarrhea episodes did not include what is above normal for the individual. In addition, if more than 3 episodes of diarrhea were recorded in the AGE surveillance log, a reportable case was automatically entered; medical staff were unable to change this during the inspection.

The cross-connection control program listed all Watts N9 backflow preventers as pressure vacuum breakers, but upon inspection they were dual check backflow preventers. In the garbage room, the program listed three Watts 800NC pressure vacuum breakers installed at the bin wash/micro spray area, but only one was installed; the log indicated three were checked in April 2011. These were testable devices, but there were no records documenting the testing. Also, the program listed three Watts N9 installed at the bin wash/micro spray under the equipment column, but two were installed and these backflow preventers were only protecting the handwash station. Additionally, the pressure vacuum breaker is a testable device but there were no records documenting its testing. In the spa, the two shower hoses had ORAS type backflow preventers installed, but they were not indicated on the list. Also, the list of backflow preventers indicated two Watts N9 installed in the sauna technical space, but only one was installed.

The electronic standardized GI illness surveillance log is programmed to automatically populate the symptom fields for diarrhea, vomiting, and fever with no answers. Under diarrhea there are individuals with 1 and 2 diarrheal episodes listed and the field under this symptom remains marked as no diarrhea. According to staff this cannot be manually corrected and the answer only changes to yes when 3 diarrheal episodes are recorded.

On the voyage of 5 thru 31 January a storekeeper with GI illness had symptom onset at 10 am on the 27th and first reported to the medical center at 9:30 am on the 28th. On the 15 April to 13 May voyage a casino dealer with GI illness had symptom onset at 7 am on 26 April and first reported to the medical center at 9am on the 27th.
Pantry-Deck 11 Pantry
There was no data plate affixed on the Electrolux undercounter single rack dishware machine. This was a new machine(installed one week ago) with 3 different warewash cycles which could be selected. It was unclear if this machine was equipped with an alarm or LED code for a low hot water final rinse temperature. The manufacturer's operating manual was reviewed and it did not indicate such an alarm or code indication of the low hot water final rinse temperature.

The halogen analyzer chart recorder for the deck 9 whirlpool had been out of order since October 27th. Manual samples were taken, but not every hour. While the whirlpool was open, samples were taken and recorded approximately every two hours. On most days, there was also a four to five hour span prior to the whirlpool being closed where no manual samples were taken. When asked, the staff indicated that the person in charge of sampling ended his shift at 1830, and no one else was responsible for sampling between then and when the whirlpool was closed at 2200.

There was no backflow prevention device or air gap on the fill line for the laundry tank. Laundry water was produced by the evaporators and the fill line was connected to the potable water system. There was only a non-return valve installed on the line.

Pantry-Deck 9 Pantry
There were brooms located in the back corner behind the ice machine. There was no designated locker for these items.

There was insufficient lighting above the clean/soiled rack in all the housekeeping pantries.
Provisions-Bulkheads
In the walk-in refrigeration units and dry storage rooms, there were small holes in the bulkheads where screws had been removed, loose profile strips, seams, and bent metal in the threshold areas of entrances. The vessel had contractors working in this area and the work was not completed at the time of the inspection.
Provisions-Entrance Curtains
Several of the entrance curtains were soiled. The curtains had recently been re-installed following drydock and had not yet been cleaned. Work in the area by contractors was not completed at the time of the inspection.
Provisions-Transportation Corridor Deck
The decks in the transportation corridor between the food storage areas were damaged and covered with debris. The ship had just left drydock and work in this area was not completed. Staff stated that the decks would be refinished.

Food Service General-Decks
The decks throughout the food preparation, food storage, food service, and warewash areas had chipped deck tiles and damaged grout. There were also gaps between the coved tiles and the stainless steel bulkheads in several locations.

The threshold material was damaged at the entrance to the dry stores room.

The non-food contact surfaces of equipment had missing fasteners, excess sealant, loose profile strips, seams, and other difficult to clean features.
Buffet-Officer's Mess
This area had new deck tiles installed. Below the counter, the deck/bulkhead juncture was not coved and the grout appeared to be recessed between the tiles at this juncture.
Galley-Pinnacle Grill Beverage Station
In the technical compartment below the coffee machine, the back left area of the compartment was open to the void space behind the counter. This made cleaning difficult and could allow for pest harborage.
Galley-Cold Galley
In pass-through refrigeration unit 7.16.C, the side of the gasket by the latch was missing. In upright refrigeration unit 7.25A, the gasket was cut near the latch. The latching mechanism was very loose on undercounter refrigeration unit 7.19.B.
Galley-Hot Galley
The water fountain did not have sufficient water flow. Staff stated that the fountain was scheduled to be replaced.
Galley-Dishwash
The data plate was worn and the final rinse cycle time was not readable on the single-compartment glasswash machine.
Dining Room-
The deck/cabinet junctures of the waiter stations were not coved.
Buffet-Starboard Beverage Station
In the juice machine technical compartment, the pipes were too close to the floor of the compartment to allow for easy cleaning. The Vitality technician was onboard to install new counter-mounted dispensers and the equipment in the technical compartment was to be removed.

The deck/cabinet juncture was not coved on the island buffet counter, on the bread station on the starboard side, and on the beverage station and the bread/fruit station on the port side.
Galley-
The door latch was broken on the single bulk milk dispenser.
Galley-Cold Preparation Counter
On the left side of the cold preparation counter, the electrical conduit was separating from the junction to the counter.
Bar-Crow's Nest
There was no handwash station in the bar. The handwash station in the pantry was more than 8 meters from the working areas in the bar.
Bar-Java Bar
The deck/bulkhead and deck/cabinet junctures were not coved in the bar or in the pantry. Below the point-of-sale machines, the vinyl deck material was bubbling at the bulkhead juncture. At the entrance, at the bottom edge of the counter, the new carpet was not secured.

Personnel Casualties : 1 injury
On 1/4/09 at approximately 1736 local time, the C/S PRINSENDAM while departing Port Everglades contacted the CG via VHF requesting assistance. A contractor on board the vessel was suffering from a head injury and needed to be transported to shore. Drug testing was not conducted as this individual was a contractor transiting with the vessel to the shipyard for drydock purposes. The report states that alcohol testing was conducted by ship's crew, however there is no evidence to substantiate this claim.
