Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The Aft Pool had not open for the day when it was inspected. The water was very cloudy and the bottom and drains were not visible. Staff said the pool would not be opened until problem was abated by draining the pool.

The sanitizer in the bucket for wiping the counters was below the required amount. NOTE: It was discovered that were two lot numbers of test strips from the supplier were being used. The color standard markings on the side greatly differed in on the two bottles which may have contributed to an incorrect reading. Supplier was contacted.

Dining Room-Deck 2 Main

The new sanitizer bucket for wiping the counters was below the required amount. See remarks in Item 2 above.

Preparation Room-Bamboo Skewers

Skewers used in food preparation were stored incorrectly with office materials in a cabinet.

A dirty phone cord was draped in front of the soap dispenser making it difficult to access.

The back flow assembly behind the newly installed juice machine had a slight leak.

Preparation Room-Deck B - Fish Preparation

An insect bait station was found on the top of the soap dispenser at the handwashing sink. Another one was on the piping cover of the prewash station of the dirty counter. The adhesive strip where they had been originally placed somewhere on the bulkhead was very small and they had been knocked off during the area cleaning operation. These bait stations were discarded and the area where they were found was cleaned.

Galley-Deck 2 - Main Galley Beverage Station

The plastic inner liner near the door hinge of the counter-mounted milk cooler for the coffee machine was severely cracked making it difficult to clean. Staff had already ordered a new one and it is due to be delivered on 03 Feb.

Galley-Deck 2 - Main Galley - Grill Line

A counter-mounted fryer that had been previously cleaned, had old grease splash on the electrical cable.

Buffet-Deck 9 - Cheese Station / Salad Station

Regular small dinner forks were found placed in the buffet line for passengers serving themselves the cheese cubes on at least four platters. The fork handles were not long enough to prevent the cheese from being touched by the passenger hands. Small tongs which provided more distance for adequate food protection were placed on these platters. The tongs for the lettuce bowl had been placed by staff where the part of the handle that would be touched by the passenger hands was in the salad. The passengers had not started through the line at the time these areas were inspected so no food needed to be discarded.

Buffet-Deck 9 - Dessert Station

A wiping cloth solution was soiled with a grease film on the water. The water was changed immediately.

Bar-Deck 3 - Ocean Bar

The recessed screw holes in the bottom of the shutter track on the back bar had old food soil in them. Cleaning and sealing the holes started immediately.

Medical-Routine Acute Gastroenteritis (AGE) Reporting

The review of the last 7 cruises found underreporting at the time of submitting 24-hour reports, failure to submit 4-hour report updates, and failure to submit a routine AGE report up arrival to a U.S. port from a foreign port. Specifically: (1) On the voyage of 28 December to 4 January, a 24-hour report was submitted on 2 January at 12:17 pm for arrival at Key West at 8:00 am on 3 January from Cozumel, Mexico; it reported 2 passenger reportable cases and 0 reportable crew cases but at the time of that report, the AGE log documented 5 passenger reportable cases and 3 reportable crew cases. (2) On the voyage of 28 December to 4 January, a 4-hour report was not submitted to update the count with an additional passenger reportable case from 2 January at 2:05 pm prior to arrival to Key West on 3 January at at 8:00 am. (3) On the voyage of 21-28 December, a 24-hour report was submitted on 23 December at 8:29 am with 0 passenger reportable cases and 1 reportable crew case, while at the time of this report the AGE log documented 0 passenger reportable cases and 1 reportable crew case; a second 24-hour report was submitted on 26 December at 7:56 pm with 0 passenger reportable cases and 0 reportable crew cases, and a third 24-hour report was submitted on 27 December at 8:57 pm with 0 passenger reportable cases and 1 reportable crew case, all three reports prior to arrival to Fort Lauderdale on 28 December at 7:00 am. These three reports underreported cases at the time of submission, being the correct count of reportable cases, per the AGE log, at the time of the report submitted on 26 December to be 2 passenger reportable cases and 2 reportable crew cases.(4) On the voyage of 7-14 December, a 24-hour report submitted on 9 December at 12:26 pm prior to arrival to San Juan indicated 2 passenger reportable cases and 1 reportable crew case, while the AGE log documented at the time of that submission 4 passenger reportable cases and 1 reportable crew case.(5) On the voyage of 7-12 December, no 24-hour or 4-hour reports were submitted prior to arrival to Fort Lauderdale on 14 December at 7:00 am from Bahamas. The AGE log for the cruise documented 5 passenger reportable cases out of 1857 total passengers and 2 reportable crew cases out of 864 total crew members.

Medical-Routine Acute Gastroenteritis (AGE) Reporting

The review of the last 7 cruises found underreporting at the time of submitting 24-hour reports, failure to submit 4-hour report updates, and failure to submit a routine AGE report up arrival to a U.S. port from a foreign port. The onboard medical staff was not aware or could provide explanations for the multiple AGE reporting deficiencies found during the inspection. Staff stated that most of the deficiencies found were done by the previous medical team.

Children Area-

Although a written guidance on symptoms of common childhood infectious illnesses was posted at the entrance of the area for 3 to 7 year olds, staff explained that this area was not always used and usually all kids from this age range were cared at the areas for kids 8 years old and up and never have to go near the 3 to 7 year old area. All areas had separate entrances and the areas for 8 years and up had no postings of the aforementioned guidance.

Children Area-

The faucet of the kids bathroom sink did not work.

Two closed beer bottles were stored on the deck and next to trash bins. These were removed from the station.

Recreational Water Facilities-Maintenance Records

The staff described their cleaning and disinfection procedures for the cartridge filters and the filter housings, but only the filter replacement and filter cleaning and disinfection were actually written in the Bubba filter and suction strainer sanitation log. Additionally, the staff described the cleaning and disinfection of the hair and lint strainers and the housings for same, but the log only listed the cleaning and disinfection of the strainer and there was no entry for the cleaning and disinfection of the strainer housing for any of the pools or whirlpool spas.

The time contol plan stated to only label containers of potentially hazardous food on time control if the service was greater than four hours.

The waiter station was not in service and the area was previously cleaned. There was visible food debris on the carpeted deck and there was no coving at the deck/bulkhead juncture. Heavy dust accumulation was observed on the back bulkhead which was constructed of a material which was difficult to clean. Food items and equipment were stored in the water station.

A tray of spongecake was uncovered on the top level of the walk-in freezer shelving units. Another tray of spongecake was partially covered on the top shelf of the same unit.

In the lido aft beverage station, the consumer advisory statements for the fried eggs and the boiled eggs cooked to order did not state that these items were cooked to order. Also, in the starboard lido benedict station, there was no consumer advisory sign or statement for the cold smoked salmon on the Scottish Benedict. The consumer advisory sign only indicated a caution for the undercooked poached egg in this dish. In this same area, there were three containers of scrambled eggs, beef, and chicken that were fully cooked with a consumer advisory sign for each container. Finally, at the Terrace Grill, there were hamburgers and cheese burgers that were cooked to order, but this was not stated on the consumer advisory sign.

Galley-Warewashing

A worker was observed carrying a tray with two stacks of clean bowls that were not covered or inverted to the dining room area. The worker was corrected by the manager and the bowls were inverted.

The vent of the backflow prevention device was heavily corroded.

Other-Food Lift #6 Deck 9 Lido

There was a small ball of cockroach gel bait at the back left corner of the deckhead/bulkhead juncture that was not covered or protected and located so that it could fall in food transported in this lift. No food was stored in this lift during the inspection.

Galley-Forward Pantry

There were a few small balls of cockroach gel bait at the upper deckhead/bulkhead junctures, especially between bulkhead seams and above wrapped containers and packages of bread and pasta and boxes of cereal. No bait was seen on any food items during the inspection.

Provisions-Deck A Liquor Room

There were a few boxes of facial spray stored in the same cabinet as several boxes of liquor, but the spray was on a different shelf. The ingredients of the facial spray were made of water and no other chemicals were indicated. According to staff, the facial spray would be sold to guests and was not used or needed in the food service operation.

The deck under the shelving units was soiled at the juncture with the bulkheads. In addition, the metal cabinet between two shelving units was directly on the deck and it was not elevated on legs or sealed to the deck. The edges of the cabinet where it met the deck were corroding in a few areas. Food and clean equipment were stored in this room.

The plastic lid on a container of sliced potatoes was cracked in three places on the outside. The lid was immediately replaced.

Four trolleys of soiled dishes, one deck stand with a stack of plastic containers and one deck stand with several coffee dispensers were stored in the hot galley directly adjacent to the potwash soiled landing. According to the staff, these items had been brought from the lido breakfast service after it closed. There was no active food preparation or cooking in the hot galley at the time of the inspection. Also according to the staff, the only meal service from this galley was dinner, and the soiled items would be cleared from this area well before dinner service preparation began.

Two garbage bin lids were stored on the deck under one of the soiled storage shelves. The lids were removed.

Other-Culinary Arts Center

The vessel had a program for passengers to participate in a cooking class. According to the staff, all food prepared during this class was immediately discarded and not consumed. Any food for tasting by participants was prepared in the main galley and served after the class. These classes were offered for adults as well as children through the youth center. The corporate office had recently submitted procedures for this program to VSP for review.

Preparation Room-

One small fly was along the front counter near the stored cookies. The fly was removed.

Provisions-Deck A Liquor Room

A metal cabinet was directly on the deck near the entrance to the room. The cabinet was not sealed to the deck or elevated on legs. Another metal cabinet was also directly on the deck in the back, left corner of the room. The cabinet was not sealed to the deck or elevated on legs. Staff began to work on a solution immediately.

Water was dripping from a deckhead air vent to the deck in front of the citrus juicer. No clean items were impacted.

There was no coving at the deck/bulkhead junctures. Bottled beverages and a wine refrigerator were in this locker.

There were three soiled glasses in the waste receptacle of the handwashing sink.

The chart recorder chart indicated the pH was less than 7.0 when whirlpool spa was open for several hours on October 22 - 25, 2013. The free chlorine residual was within the correct range during this same time period.

The analyzer readings on the chart recorder charts indicated the pH values were less than 7.0 when the whirlpool spa was open for several hours on October 22 - 25, 2013, but the pH values recorded on the same charts measured with a test kit were above 7.0. The difference in the pH values between the analyzer reading and the manual test kit was greater than 0.2. The chart recorder charts indicated the analyzer readings and the manual test kit values for the free chlorine residual were within the proper range during this same time period. There was no documentation indicating why the pH was out of the acceptable range or why the analyzer was not properly calibrated.

Several boxes of hot cocoa mix, sugar packets, potato chips, gold fish crackers, a bottle of water, and a box of single service coffee stirrers were stored on the deck in the closet where the far point chart recorder was housed. Several boxes of hot cocoa mix, sugar packets, potato chips, gold fish crackers, a bottle of water, a box of single service coffee stirrers and a bag of disposable cups were stored on the deck in the closet where the far point chart recorder was housed. The bulkhead had several electrical wires, devices, and cables attached to the bulkhead that were not easily cleanable. All of these items were removed.

The potable water connection from the bunker line for water distribution was under the active fuel bunkering line and partially in a container with a small amount of fuel. The potable water connection was not in-use during the inspection. According to the staff, water distribution using this connection has never been done.

The staff removed the orange dot from the fridge explaining that the yellow dot indicated the correct setup and discard times for the potentially hazardous foods in the fridge.

The water pipes behind the counter-mounted coffee machine were soiled with dust.

Provisions-Parasite Destruction Record

The parasite destruction record for Capital Sales had unclear and conflicting information. The letter certified that the following raw-marinated herring products were parasite free: rollmops, gallon cream, gallon wine, 10# wine, and 10# cream. However, the letter also had a statement indicating that upon request, all other cream and wine marinated herring could be delivered with a certificate stating that the products were frozen at -4°F for at least 7 days. There was no other record that indicated that these ready-to-eat products were parasite free.

Bar-Explorations Cafe

There were two slotted fasteners in each of the food splash zones by the coffee dispensing tubes of the two espresso machines.

Potable Water-Tank Maintenance Records

Potable water storage tanks 3C, 2S, 1S, 1P, and 1C were cleaned and disinfected in April 2012. The records indicated chlorine values of less than 5 ppm before they were put back into service. However, the actual chlorine values were not recorded.

Recreational Water Facilities-Whirlpool Safety Signs

The whirlpool safety signs stated that 'children under 14 require adult supervision.' However, the clarifications page to the 2011 Operations Manual indicated that those under 16 years of age are considered children. This was also noted in the previous inspection.

Medical-Crew Acute Gastroenteritis (AGE) Reporting

During the 2-9 December cruise, a cadet had an onset of AGE symptoms on 2 December at 1200, but did not report to the medical center until 3 December at 0730. The time card for this crew member indicated that he worked on 2 December from 1300 to 1400 while symptomatic.

Integrated Pest Management-Mooring Lines

The rodent guards on the mooring lines were ineffective in preventing rodents from entering the ship. The guards did not completely surround the lines and some of the guards had blown off due to the wind.

The potable water line and electrical cord from the espresso machine were draped across the counter, making the counter difficult to clean.

There was excessive and peeling sealant at the backsplash/bulkhead juncture of the prep sink counter nearest the hand sink.

Buffet-Deck 9 - Aft Beverage Station

Two trays of uncovered cups of ice water were stored unprotected on the counter of the beverage station. The inspector was informed that these glasses of water are only served by the beverage attendant and were not for passenger self-service. During this time, however, a passenger was observed serving themself.

Food Service General-Ice Machines

An accumulation of sand-like granules were observed along the length of the plastic water return trays below the ice cubers in multiple deck mounted ice machines. This was observed in the ice machines at the following locations: Lido Galley corridor, Tamarind Galley, both ice machines located on deck 3 Main Galley Aft, and the Explorations Cafe Pantry. A heavier amount of sand-like granules were observed in the Lido Galley corridor unit then any of the others.

Food Service General-Glasswash Dish Machines

Rack-type glasswash machines located in the Lido dishwash, Tamarind dishwash, and deck 2 Main Galley dishwash were out of order. Soiled glassware in these areas were sent to nearby working machines.

Food Service General-Consumer Advisories

The menu's for the Manhattan Dining Room, Tamarind Restaurant, and Pinnacle Grill had consumer advisories indicating that consuming raw or undercooked animal derived foods may increase the risk of foodborne illness, especially to consumers with certain medical conditions. However, the animal derived products that could be ordered raw or undercooked were not identified with an asterisk.

Other-Concierge Lounge

Dust and coffee debris of more than a day's accumulation was observed underneath both of the counter mounted coffee machines. This was corrected during the inspection.

The deck drain for the coffee machines located inside the pantry technical compartment was full of mold and coffee debris. The amount of mold and debris observed indicated that this area is rarely cleaned. This area was cleaned during the inspection.

Staff stated that the hair and lint strainers and hair and lint strainer housings on the recreational water facilities were cleaned and disinfected but there was no documentation regarding the frequency of cleaning, disinfectant concentration, or contact time.

Recreational Water Facilities-Safety Signs

The safety sign posted at the spa pool did not contain all elements of the 2011 VSP Operations Manual sign requirements. The signs did not include bather load or caution against use by individuals who are immunocompromised, on medication, or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure. In addition, the safety signs for the whirlpool spas stated that children under the age of 14 are required to have adult supervision instead of cautioning against the use by children.

Recreational Water Facilities-Automated Monitoring

The whirlpools did not have chart recorders or electronic data loggers installed on sample lines directly from the recreational water facilities or on the return lines from the recreational water facilities before the compensation tanks.

The shepherd's hooks at the two swimming pools were telescoping.

Potable Water-Cross-Connection Control Program

The cross-connection control program listed the non-testable backflow prevention devices with an inspection date column where the staff wrote N/A for not applicable. All air gaps, double check valves with intermediate vents, and atmospheric vacuum breakers were not included in a periodic inspection schedule. In addition, the listing described the Watts N9C as pressure vacuum breaker for non-continuous pressure applications; the Watts N9C is a double check valve with intermediate vents for continuous pressure applications.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

The OPRP did not include procedures to protect the passengers from exposure to disinfectants to minimize respiratory and dermal exposures.

In pantry P.1.2.01, there was a pink colored material on the side covers of the cuber panel of the ice machine and sand-like granules in the water bath under the cuber. In pantry P.8.4.02, there was a heavier accumulation of the same sand-like granules in the water bath under the cuber of the ice machine. These machines were last cleaned in October 2011.

There was also a toy spoon on the range top of the child-sized kitchen that had not been cleaned and sanitized with all the other toys.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

The AGE surveillance log did not meet the new requirements. For example, there was no column for patient ID, only one column for name, and two columns to indicate passenger and crew. There were also several extra columns, including isolation cabin, dinner table, elaspsed time from onset, date out and time out. Finally, several of the columns were not in the exact order of the template in Annex 13.2.2.

Recreational Water Facilities-Turnover Rates and Bather Loads

There was no documentation for the turnover rates for the whirlpools. There was no documentation for the bather loads for each RWF.

Recreational Water Facilities-Antientrapment

There were no antientrapment drain covers or alarms on the main and aft pools. The pools had multiple drains more than 3 feet apart. According to the staff, antientrapment drain covers had been ordered. The three midship whirlpools had multiple suction fittings less than three feet apart. The fittings had antientrapment drain covers, but there were no suction vacuum release systems or automatic pump shut-off systems installed.

Recreational Water Facilities-Safety Signs

The safety signs for the two swimming pools, five whirlpools and hydropool did not include the bather load number. The safety sign for the hydropool did not include a caution against use by individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure. Finally, the safety signs for the whirlpools stated that children under the age of 14 required adult supervision.

Recreational Water Facilities-Automated Monitoring

The whirlpools did not have chart recorders or electronic data loggers installed on sample lines from the RWFs or on the return lines from the RWFs before the compensation tanks.

Medical-Crew Member Acute Gastroenteritis (AGE) Reporting

On 16 November at 09:00, a kitchen assistant reported to the medical clinic with AGE symptoms. The symptoms began on 15 November at 06:00. On 17 November at 14:15, a casino dealer reported to medical with AGE symptoms. The symptoms began on 14 November at 16:00. Both crew members worked after having AGE symptoms. Disciplinary action was immediately taken by the ship's staff. In addition, the areas where each crew member worked were sanitized and passengers and crew members were monitored to see if they developed AGE symptoms from these areas.

Medical-Public Toilet Room

There was no sign in the toilet room advising users to use a tissue or paper towel to open the door. The door was not hands free, and this toilet was available to passengers and crew in the medical facility. This was corrected.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

The OPRP did not include contact times for the disinfectants. According to the staff, this information was included in the training material, but not in the written plan. In addition, the OPRP did not include procedures for returning the vessel to normal operating conditions after an outbreak. According to the staff, the procedures were decided on a case by case basis in coordination with the corporate office and medical department.

Food Service General-Consumer Advisories

There was no consumer advisory at the Lido buffet service line where eggs could be ordered undercooked. In addition, the menus for the Pinnacle Grill and the Tamarind Restaurant had consumer advisories indicating that consuming raw or undercooked animal-derived foods may increase the risk for foodborne illness, especially to consumers with certain medical conditions. However, the animal-derived products in the menu that could be ordered undercooked were not identified with an asterisk.

Food Service General-Tip-Sensitive Temperature-Measuing Devices (TMDs)

None of the staff in the cooking areas were equipped with tip-sensitive TMDs.

Other-Concierge Lounge

A broom and dust pan were stored against the deck stand of the general storage locker.

Three plate storage carts were scratched and scored at the plate contact surfaces.

During active use of the flight-type conveyor dishwash machine, the gauge for the auxiliary rinse temperature indicated 164°F. The data plate affixed to the machine indicated a minimum required auxiliary rinse temperature of 180°F.

The decks under the pallets in dry store rooms #16, #19, and #20 were soiled with accumulated dust.

The power cable to the bulk milk dispenser was draped on the counter.

A large hotel pan of cut tomatoes had temperatures that ranged from 55°F to 58°F. The tomatoes had a discard date of 8 December 2011. The tomatoes were discarded.

The bottom panels of the CCool 9 multiflow systems were heavily corroded, making cleaning difficult. This was observed in multiflow systems in the pantries for the Ocean, Crow's Nest, and Pool bars.

The loading inspection forms stated "food and beverage items loaded today have been inspected for signs of pest infestation." There was no indication that plants, flowers, and other non-food items were inspected prior to loading.

The daily check log of the refrigeration unit had the unit at 36°F. The food was discarded.There was a hotel pan of shrimp salad with a production date of November 8, 2010 in refrigerator 9.02B. There was a mounted stainless steel shelf that had openings on the top of one support leg and each of the round removable safety bars in the front were hallow. The edges where the shelves attached to the supports had gaps that made cleaning difficult. The shelves and points where the safety bars attached were lightly soiled.

A food worker was observed removing a raw frozen hamburger patty from a freezer with tongs and placing the patty on a plate on the work counter. The worker was then observed removing the butcher paper from the back of the patty and then the worker took the patty by tongs over to the grill, crossing over a clean counter and clean plates with ready-to-eat lettuce. Small particles of ice were observed falling from the patty onto the clean work surface near the plating operation. The plates of lettuce were discarded, and the plating operation for cooked and ready-to-eat products were moved to another work counter.

Buffet-Crew Mess

The soda vending machine was on legs only 1 inch (25 mm) above the deck. The deck beneath was heavily soiled with dirt and food debris.

Galley-Crew Galley - Dishwash Area

The stationary rack glasswash machine was posted out of order. Per staff the unit has been out of order since 30 October, 2010.

Galley-Deck 2 Main Galley - Garde' Manger

There was a water dripping from the piping in deckhead access hatch at the upright pass thru refrigerator #2.13B. The water was dripping down to the deck below. The water appeared to be from condensation forming on the piping.

Galley-Deck 2 Main Galley - Garde' Manger

Three sheet pans of individually stacked cut tomato and mozzarella cheese (caprese appetizer) were found inside walk-in refrigerator 27 with food temperatures ranging from 43-46 °F on the inspector's thermometer and 41-43 on two of the ship's staff thermometers. All thermometers were calibration checked using an ice bath and the staff thermometers registered 2-3 degrees lower (29-30 °F) while the inspector's thermometer registered 1 degree (33 °F) higher than expected. The pans were removed and blast chilled immediately.

Galley-Deck 2 Main Galley - Pastry

Two sheet pans of éclairs and chocolate mousse were found on a trolley inside pastry walk-in refrigerator #26 with Monday stickers attached. This indicates the food was prepared on Tuesday and should be discarded on the following Monday, according to the ship's 7 day discard system. The cooling log for both items indicated they were both prepared today and the sticker should have indicated discard on Tuesday. Staff confirmed these items were prepared today.

Galley-Deck 2 Main Galley - Pastry

One sheet pan filled with individual cups of rice pudding was found inside pastry walk-in refrigerator #26 with internal temperatures of 48-50 °F on both the inspector's and the ship's staff thermometers. The rice pudding was prepared yesterday so it was discarded immediately. A sheet pan filled with éclairs and another filled with individual chocolate mousse desserts were also in this walk-in on a plastic enclosed trolley with food temperatures of 48 °F. Three sheet pans filled with individual serving containers of crème Brule' were found on an open trolley in the walk-in with food temperatures of 44 °F. The cooling logs confirmed these products were prepared today and were out of temperature for less than 4 hours, so they were immediately removed and blast chilled.

Galley-Deck 2 Main Galley - Pastry

It was not clear if the potentially hazardous ready-to-eat foods inside pastry walk-in #26 were out of temperature due to the unit or the amount of trolleys inside the unit, or the use of heavy plastic zip covers used on some of the trolleys. The unit was locked shut for over 20 minutes and the air temperature inside was at 5.5 °C and dropping slowly. The signs posted on the door stated that the temperature inside must be maintained at 2 °C (36 °F).

Galley-Deck 3 Main Galley - Hot Galley

There was an excessive amount of soiled pots and pans staged along the preparation area of the hot galley surrounding the pot wash. One of the trolleys holding soiled pots/pans was within one meter of the preparation counter, two trolleys filled with soiled pans were on two sides of the in-use combination oven, and one trolley of soiled was in contact with the handwash station in the preparation area. There was active cooking in the combination ovens during this time, as the potwash was also in active use.
