Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Crew Late Reporting

A nonfood worker had a gastrointestinal illness (GI) onset on 9 April at 1930, but did not report to medical until 10 April at 1932. The crew member worked on the 10 April from 0650 to 745 and ate breakfast in the mess. The crew member was disciplined and this was documented. A similar finding was noted on the last inspection.

Medical-Acute Gastroenteritis (AGE) Report

A crew member was listed on the AGE surveillance log as a reportable case, but upon evaluation of the crew member's medical record it appears this crew member was a mistake and did not report to medical for AGE symptoms. The medical staff informed the inspector a staff member accidentally entered the AGE case in the electronic reporting system under the crew member's name when the AGE case information was reported and experienced by a passenger. This was all indicated in the documentation. A separate AGE entry was recorded for the passenger, but staff were unable to be remove the crew member entry from the AGE log. A similar finding was noted on the last inspection.

Potable Water-Engine Room - Pump Room #2

The water line was striped blue green blue after the reduced pressure assemblies for the laundry filling from the evaporator and forward transfer pumps.

Water was dripping from a deckhead mounted fire detector onto the preparation counter near the conveyor toaster oven. Water drops were noted on the counter and the area was not in service. Another water drip was observed from the aft starboard canopy hood, near the front edge and beside the vegetable preparation sink. Staff investigated and determined the leaks were from recent cleaning.

Several of the soda dispensing units at both stations had exposed slotted fasteners on the underside by the dispensing nozzles.

The first three final rinse temperatures measured by the inspector and crew on the in-use flight-type dishwash machine were 155°F, 154°F, and 157°F, and 160°F, 157°F, and 153°F, respectfully. A 180°F maximum registering thermolabel was placed on the final rinse manifold and it did change color indicating the temperature at the final rinse manifold was met. The final rinse spray pattern from the upper manifold was evaluated, and it did not produce a strong, effective spray pattern. The water was coming out of the manifold in a pulsing rhythm. Staff replaced the final rinse nozzles on the upper manifold and a stronger, more effective spray pattern was produced, resulting in plate surface temperatures 160°F and above.

A portable bain marie with 5 wells was stored by the entrance to the crew mess. Staff stated this unit had not been used in several months, and was not needed for the operation. It could not be determined if the bain marie unit needed any repairs.

Medical-VSP Acute Gastroenteritis (AGE) Report

For the cruise dated 27 July to 3 August, a crew member was listed on the AGE surveillance log as reportable but his only symptom was a headache. According to the staff, this crew member was actually a varicella case and they could not remove the entry from the log. The 24-hour report to VSP for reportable AGE cases included this case. Therefore, the report was incorrect.

A large container was placed below to catch the leaking water. The area was not in operation during the inspection and was only open during the morning on sea and port days.

There were large deckhead ventilation plenum spaces at the pass-through windows from the show galley to the dining area. Servers picked up food from the counter below the plenum spaces to take to passenger tables. This was a new addition after the January 2014 dry dock.

After the mineralizer and prior to the chlorine injection point, the water line was striped to indicate both potable and distillate/permeate water. After the mineralizer and prior to the pH injection point, the water line was striped blue/grey/blue, indicating distillate/permeate water. After the pH injection point and prior to the chlorine injection point, the water line was striped blue/green/blue, indicating potable water. The water line striping was immediately corrected to indicate distillate/permeate water between the mineralizer and the chlorine injection point.

Medical-Reportable Case Definition

On 21 February, a nonfood worker presented to the medical center with 2 episodes of diarrhea which was more than normal for the individual, was administered antidiarrheal medication, but was marked as non-reportable on the acute gastroenteritis (AGE) log. A similar finding was found on 19 January when a non-food worker presented to the medical center with 2 episodes of diarrhea which was more than normal for the individual, was administered antidiarrheal medication, but was also marked as non-reportable on the AGE log. According to medical staff, these cases were incorrectly entered into the ship's electronic medical record system and were unable to be changed once they realized a mistake had been made. No notations were made in the log that identified them as reportable AGE cases. Both cases were isolated for the required 24 hours from the last AGE symptom.

Medical-Crew Member Reporting

A Pool Attendant who experienced 4 episodes of diarrhea on 7 February beginning at 04:00 did not report to the medical center until 11:00. Time records indicated this individual worked while symptomatic.

The dinner menu had a consumer advisory, but was not visible. The advisory was tucked in the menu cover.

A hole was noted in the damaged bulkhead near the deck next to fire door 1.7.3.0 that created a difficult to clean area and an access for vermin in the provisions area.

The time control plan shows set up and discard time for food as follows: Snack - 0930 - 1030; Lunch 1000 - 1400. Hot Holding Cabinet - HH-02-02 - was identified as a Time Control Unit on the plan. At 0900, several pans of cooked rice was found in this unit. The rice had been cooked at 0800 and placed in this Time Control Unit.

The drain valve and drain assembly of a previously cleaned tilting kettle still had food soil on it. An old bay leaf fell from the drain when it was opened for inspection.

The data plates for these machines have not been upgraded to provide the conveyor speed to the manufacturer's specified conveyor speed in feet per minute or minimum transit time for belt conveyor or rack conveyor machines. The current data plates only indicate a throughput of a maximum rack capacity per hour.

Housekeeping-Public Restrooms

Restrooms located in rooms 1724 and 1725 had toilets with sinks in the rooms to the right of the main passage way. The exterior doors had the hands free exit buttons and signs. The smaller rooms with toilets and sinks did not have the hands free exit or sign on the doors. These signs were placed on these doors during the inspection.

Potable Water-Disinfection Documentation

Large and small sections of pipe throughout different areas of the potable water distribution system have recently been replaced with new plastic piping. According to staff, these new sections of pipe were disinfected and flushed prior to being put in service, however, there is no documentation available to confirm these procedures.

The chopper had been out of order since 8/9/2008. According to the staff, the manufacturer was out of business and they had difficulty acquiring spare parts.

There was condensate dripping from the deckhead panel above the right tilting pan to the control box. The condensate did not drip directly into the pan, but liquid was splashing from the top of the control box into the pan. Condensate was also collecting on other parts of the panel, including directly above the pan, but it was not observed dripping. There was no food in the pan, but it had been previously used and was full of water for cleaning. A work order had been submitted.

There was condensate dripping from the edge of the deckhead exhaust hood to the stove top across from the soup kettles. According to the staff, this stove top was not being used. A work order had been submitted.

There was condensate dripping from the deckhead sprinkler head in front of the flat griddles near the dishwash area to the deck below. This was reported to maintenance.

Potable Water-Tank Maintenance Records

Potable water tank 3C had a maintenance job on 3 April 2011 and potable water tank 2P had a maintenance job on 20 March 2011. The maintenance job included disinfection of the tanks. There was no indication in the records that the potable water tanks were flushed until the free halogen residuals were less than or equal to 5.00 ppm before the tanks were put back into service.

Potable Water-Machinery Room - Whirpool/Spa #1 and #2

The reduced pressure assembly device (RPZ) for the compensation tank of the whirlpool/spa #1 was leaking through the gasket. In addition, both of the RPZs in this area had a funnel under the vents with an air break connected to a drain line. However, the drain line was also connected to a line labeled 'return line from showers.' When the valve on this line was opened, water bubbled out of the funnel on the right. Therefore, an air gaps is needed at each of the funnels.

The backflow prevention assembly attached to the spray hose was weeping.

Several gaskets on swinging doors that led to food preparation areas and bars had openings at the top and bottom. Several gaskets were deteriorated and difficult to clean.

The previously cleaned fryer heating elements were soiled with dark deposits of grease and old food residue, especially along the top and sides where the elements coiled.

One large plastic bin of unpackaged white grapes were stored uncovered on the top rack of a trolley. There were two other plastic bins of unpackaged and uncovered whole pears and apples on the preparation counter. All were covered immediately.

The cooling logs did not have enough detail with respect to the food name. There was difficulty in matching the cooling log with the foods in cold storage. In the sauce area walk-in refrigeration unit there were pans that were labeled "brown sauce". The chef for this area stated that the sauce was lamb sauce. The cooling log for that date did not list brown sauce or lamb sauce. There was an entry for lamb which the chef stated was the "brown sauce".

Food Service General-

There were no log entries for the pasta (3/17) in the deck 5 buffet area refrigeration unit. There were no entries in the cooling log for the duck sauce (3/14) or garlic sauce (3/17) that were in the walk-in refrigeration unit. The log entries (3/17) for the pit ham and sausage links showed cooling starting at 89°F and 100°F, respectively.

Medical-

For a waitress who reported to medical with a gi illness, the last symptom was reported as March 11 at 1900. This individual was cleared to return to work on March 12 at 2044 hours, lees than the required 48 hour isolation period after last symptom..

Medical-

There was an ill crew member who had 15 episodes of diarrhea in a 24-hour period who was listed as a non-reportable case in the standardized gastrointestinal illness log. The individual was reported as a case in the 24-hour prior to arrival report.

The recirculation lines and drain lines were striped blue indicating potable water.

Per staff, food and supplies were inspected for pests. However, the log only indicated that food was being inspected for pests.

Housekeeping-Outbreak Prevention and Response Plan

The plan stated that corridor hand rails, elevator buttons and frequently touched areas were to be sanitized on a nightly basis during an outbreak. In addition, the OPP Enhanced Sanitation Protocol document stated that frequently touched surfaces and areas were to be sanitized 3 times a day during an outbreak. Per staff, frequently touched areas were to be sanitized on a continuous basis.

NAVIGATOR OF THE SEAS (O.N. 9000051): On 14Mar2012, elderly male pronounced dead from acute cardiogenic pulmonary edema after CPR efforts undertaken. Deceased disembarked at Port Everglades. Next of Kin notified. Note: Incident investigation treated as a Data Collection due to the injury resulting from an apparent pre-existing medical condition and not from a marine operation. Note: Though this marine casualty meets the definition of Serious Marine Incident, no drug or alcohol tests were conducted on the crewmembers as no action or inaction of the crew contributed to the passenger's condition and subsequent injury.

Material Failure (Vessels)

On 19 March 2011, the Coast Guard received notification via CG-2692 that the M/V NAVIGATOR OF THE SEAS had experienced a marine casualty to their starboard azipod (servomotor M1). After troubleshooting and conducting initial repairs, the vessel is able to operate the stbd azipod but without redundancy. The port azipod is functioning as required. Vessel arrived in Port Everglades on 21 March 2011 at approximately 0700 without incident. Coast Guard contacted vessel's classification society (DNV) to determine corrective course of action.

Personnel Casualties : 1 injury

On March 13, 2011 Sector Key West received notification that a passenger from the cruise ship NAVIGATOR OF THE SEAS was jumping on a bed, fell and received a head injury. The passenger was MEDEVAC'd to Lower Keys Medical Center, diagnosed with a concussion, treated and released.
