On Jan. 8, 2015 a 22-year-old fell off the Royal Caribbean's Oasis of the Seas and was in the water for several hours before being rescued by the Disney Magic.

A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Recreational Water Facilities-Solarium Pool & Aqua Theater Pool

During record review, it was found that the halogen levels for these interactive RWFs were consistently above 5.0 ppm. These facilities were considered by the ship to be interactive RWFs but halogenated to levels of 3.0-10.0 ppm. Most of the readings were between 4.0 and 8.0. The Solarium pool had spray features and the Aqua Theater pool had bubbling effects. On the day of inspection, the Solarium pool had halogen levels just below 5.0 ppm when tested by the inspector and the Aqua Theater pool was closed and not tested.

The new letter from a registered design professional stating that the antientrapment drain cover for the single drain installation was acceptable did not have the name of the ship.

Integrated Pest Management-Active and Passive Monitoring Records

Izumi Restaurant on deck 4 was added during the most recent dry-dock of October 2014 and started operations in November 2014 according to staff. IPM records did not document active or passive monitoring for this new food area in November 2014, December 2014, and January 2015.

Izumi Restaurant on deck 4 was added during the most recent dry-dock of October 2014 and started operations in November 2014 according to staff. The food area had several backflow prevention devices installed but they were not on the device list of the cross-connection control program. Specifically, the backflow prevention devices on the combination oven, the spray hose in the warewashing area, and the hood cleaning cabinet were not added to the list.

Potable Water-Aqua Suites

New suites were added during the most recent dry-dock of October 2014. There were 6 new suites on decks 11, 12, and 14 which had 2 shower hoses in each bath tub and had no backflow prevention devices installed. All these hoses could be submerged in the tubs. There were vacuum breakers installed for the toilets but it was unclear if they were added to the list of backflow prevention devices since the list grouped these devices by the hundreds on each deck.

Medical-Late Reporting

During the 7-14 February voyage, two food handlers reported late to medical after experiencing acute gastroenteritis (AGE) symptoms. On 12 February, a food handler had an onset of symptoms at 1100 and reported to medical at 1645. This food handler ate lunch at the crew mess at 1300. On 8 February, a food handler had an onset of symptoms at 1245 and reported to medical at 1545. This food handler did not work or went to public areas. Both crew members received disciplinary actions.

Recreational Water Facilities-Solarium Spa Pool - Antientrapment Cover

The documentation for the antientrapment cover in the Solarium spa pool indicated that it was to be used for multiple drains only, but the drain in the pool was a single drain. In addition, the documentation stated that the product should only be used when the application and design was by a registered design professional, however it was not clear on the signed installation certificate if the signatory was a registered design professional. The vessel was aware of these issues and provided documentation about actions being taken.

There was a blender full of ice plugged into the bulkhead on the counter and the drain for the blender was 35 inches above the scupper and was dripping down the front of the neutral cabinet with condiments and then onto the deck.

There were several deep cuts and scratches on the nonfood contact surfaces around the rim of the ice bin and also on the top of the bin used to store ice, making cleaning difficult.

A crew member was readmitted to the medical system on 06/06/2014. The reporting time as compared to the symptom onset time appeared to be late due to an electronic medical system record error. This error was documented on the individual treatment record.

A food worker during the voyage of 5/17/2014 did not have any documentation in the medical records of an interview of close contacts or whether this individual had no close contacts.

Water was leaking from the seam and light fixtures in the middle of the back section of the freezer. A few cartons of food on pallets under them had evidence of splash on the outside of carton, but no food was directly impacted.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

Two crew members, one on the14-21 December voyage and one on the 21-28 December voyage, experienced one episode of diarrhea, abdominal cramps, were dispensed antidiarrheal medication, and had no underlying illnesses. Even though the medical staff isolated the crew members because they determined that the diarrhea symptoms were above normal to these individuals, they were not included as reportable cases on the AGE logs.

In the dining room locker, several boxes of galley equipment and utensils and at least 10 lexan boxes were not protected and placed adjacent to two uncovered openings to bilge water tanks filled with bilge water along with the piping and pumps associated with these systems. This and related deficiencies were noted on the last and other previous inspections.

Even though the staff had made some attempts of correction, numerous pieces of new and used equipment were stored adjacent to or under areas that could contaminate equipment and utensils. Staff stated that some work is planned to be done in these areas during the next dry-dock scheduled for September 2014.

The documentation and procedures in potable water, recreational water, child activity center, pest management, ventilation, and housekeeping accurate, complete and thorough. In addition, the staff was knowledgeable and competent.

The toilet stall was equipped with a handwash facility in addition to the main handwash station in the toilet room. The toilet stall did not have a sign advising users to use a paper towel to open the stall door.

The gauge was measuring 187°F. Around 15 minutes later the machine was re-inspected and the final rinse temperatures were corrected. The staff explained that the temperatures were adjusted.

In these areas, hot hold drawers, hot hold cabinets, and induction warmers were on time control. These units were listed in the outlet specific plans. Additionally, in the outlets that were open for more than 4 hours, the outlet specific plan did not require the use of 4-hour discard labels. The general plan did mention this requirement for outlets open for more than 4 hours. It was not clear that the time as a public health control plans in all areas included these two different plans.

The close contact for this individual had three interviews that were all recorded as June 1 at 18:30. There were two close contacts for two different crew members who were classified as cases of gastrointestinal illness. The close contacts for these ill crew members did not have daily interviews recorded. One was missing the 48-hour interview and the other was missing the daily interviews after the initial interview.

A crew member with gastrointestinal illness (Commissary 2 position) had a follow-up on June 3. The notation for that visit stated that the crew member still had symptoms. There was an accumulation of a red substance on the plastic sensor piece in the upper compartment of the ice machine. The sensor was located in the food contact zone. This item was corrected.

Galley-Deck 4 - Warewashing

The two vertical rubber strips located in the flight-type warewash machine, located after the final rinse were severely cracked and lightly soiled with a black substance.

There was a stainless steel profile strip that was buckled along the bulkhead/equipment juncture near the tilting kettles and pans. The bent strip created a small opening that could allow splash to enter a void space, and it was difficult to clean.

A hatch to potable water tank number 1 was located in the technical space of the Garden Crew Galley. It was not labeled with the words "POTABLE WATER."

Other-Laundry

Clean sheets were sorted thru using a clean tarp on the deck in front of the flat roller/iron machines. Staff stand and walk on these clean tarps and in some cases the clean sheets made contact with the deck surrounding the tarps, and the deck area around the machines was soiled. Additionally, staff did not wear shoe covers, and the clean towels, napkins and odd items sorted out of the piles of clean were set on the soiled deck, rather than in some bin or container to protect them.

Other-Deck 12 Port/Aft Outside Balcony

Approximately 195 plastic buckets used to chill wine and champagne in cabins were stored in an outside balcony space directly on the deck. These buckets were in nested stacks and wrapped in plastic bags.

Other-Pilot Station at Sr. Officer Cabin #49214

The corridor of cabins near 49214 had a strong noxious odor which appeared to be coming from the pilot station beside this cabin.

Recreational Water Facilities-Deck 5 Amphitheater Pool

There was a 1/2 inch to 3 inch gap present along the outside perimeter of the amphitheater pool platform deck and the side wall of the pool surrounding. This gap represents a significant entrapment hazard. The pool is not in use as a public pool with the platform at a swim depth, but if such a change is ever made the entrapment hazard presented in this pool should be professionally evaluated and corrected.

There was an open hole on the locking mechanism on door 8.4.3.

There were two dispensing containers of vinegar and olive oil displayed in the dining room. The deck/bulkhead juncture in this area was not coved.

However, the time control plan for Johnny Rockets did not mention the Boardwalk Bar.

For several dates in December and January the provisions inspections log sheet only had a few types of food pallets recorded as having been inspected. For example, the log for December 19 stated that produce pallets were inspected but did not mention any other items. According to the staff, all of the food and non-food items are inspected before entering the ship, and then the provisions master spot checks a few pallets once they are aboard. The spot checks from the provisions master are then recorded on the log, but the initial inspections are not.

Dining Room-Deck 5 - Departure Lounge

There were several tables preset with dinnerware. Passengers waiting to disembark were sitting at these tables. Per staff, a few unset tables are provided for disembarking passengers, however on this date, there were excess passengers and not all of the tables were cleared.

Dining Room-Deck 5 - Wait Station

There were small vases of artificial flowers stored in the cabinet with the clean coffee and creamer urns.

Galley-Deck 5 - The Cage

The deck was soiled below the deck stands.

Food Service General-Vitality Juice Machines

There were holes on the interior front panel directly below the juice concentrate containers. In addition, there was an open seam around the interior of the door frame, exposing the insulation behind.

Material Failure (Vessels)

OASIS OF THE SEAS's ship manager contacted the Coast Guard to notify that the vessel which was currently moored up in Port Everglades had its starboard azipod OOC. Vessel's rep stated that vessel's amidships and port azipods were in proper operating condition. Root cause of stbd azipod failure unknown, but ship's crew believes it has to do with the unit's uninterrupted power source (UPS) as it had been maintenanced that morning. Classification Society (DNV) notified and attested that vessel could maneuver appropriately with only two working azipods. DNV issued a condition of class for the vessel to repair the stbd azipod. Vessel cleared to leave port at request time of 1700. 8 February 2011, DNV attested that starboard azipod was in proper operating condition. Condition of class deleted.

Emergency generator suffered damage. While doing weekly checks the engine was making noises and it was discovered that Cylinder A1 crankshaft was loose and hitting the crankcase cover. After DNV survey, there was a class condition issued to have machinery repaired prior to 08/08/2010.
