Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The cross connection control log indicated the two reduced pressure (RP) assemblies in the infirmary were inspected 29 April 2013 and the test results were incomplete. The test 4 was not conducted/documented. Documentation was shown to verify the RP assemblies were inspected and tested in 2014.

Recreational Water Facilities-Deck 7 Pool and Whirlpool Machinery Room 7.4.017

The flow meters for the port side whirlpool spa and midship pool were not working. Staff replaced the batteries, and verified the flow meters were operational.

Potable Water-Deck 2 Hot Water Circulation Line

The air gap was not twice the diameter of the supply pipe for the deck 2 hot water circulation line in the engine room.

Children Area-

Some of the sleeping mats in the children activity center were torn exposing the foam, making them difficult to clean. Staff informed the inspector they did not use the torn mats and new ones were on order.

Five live fruit flies were observed by the SOMAT in the incinerator.

Galley-Main Galley - Soup Area

The previously cleaned large wand mixer at the bulkhead beside the steam kettle was soiled along the blade and interior blade guard with visible food residue.

A skillet with two cooked corned beef hash patties was set on the working stovetop along with other pans with red sauces. Staff initially stated the foods were on temperature control. The corned beef had internal temperatures of 114-115 °F. The other sauces were holding at or above 135 °F. Another staff member then stated these foods were on time only as a public health control. The time management plan was reviewed and there was no indication the stove top was a time control unit.

Galley-Fish Preparation and Garde' Manger

The left door (4.3A) on the three door undercounter reach-in refrigerator did not seal tightly when shut. Several folded sheets of paper could be passed from the outside of the lower section of the door to the interior of the unit without resistance. The same condition was noted in undercounter refrigerators 4.39A and 4.40A in the Garde' Manger.

Six live fruit flies were observed throughout the bakery/pastry during active service. The fruit flies were observed on the deckhead panels and particularly at the either the seams where panels joined or at the crevice along the profile strip connecting the panels.

Upon review of the manufacturer's instructions for testing these RPz, actually three tests had to be performed and documented: Test 1: tightness against reverse flow, Test 2: record pressure differential, and Test 3: recorded pressure differential had to be at least 2 psi less than the supply pressure.

Records documented that during the October 2013 dry dock, potable water pipelines were replaced for decks 1, 2, 6, and 7 and that from December 2013 to February 2014 technical water lines to the AC Stations were converted to potable water lines. During the inspection, half of the water was drained and the compartment was filled with hot water. The temperature reached 90°F. Additional chlorine was added to maintain the concentration within the appropriate range.

There were slotted fasteners near the front of each shelf inside the two deck ovens.

On the preparation counter, a knife blade was soiled with old food residue. The knife appeared to have been previously cleaned and sanitized. The area was not in use during the inspection. A crew member removed this knife, an adjacent knife, and the cutting board to be cleaned and sanitized.

The light intensity could not be raised to 220 lux at all areas of the waiter stations.

Potable Water-Medical Center

In September 2012, the double-check valve assembly backflow prevention devices (950XL Wilkins) were installed in the deckhead hot and cold potable water lines to address connections in the ward rooms for dialysis treatments. This was in response to the finding during the August 2012 VSP inspection. The model received was not a reduced pressure assembly backflow prevention device and no intermediate vent was installed. Staff ordered replacements during the inspection.

Children Area-Club HAL

The hot water temperature in the child toilet room handwash sink faucet reached 122°F and the faucet for the child handwash in the play room reached 126°F. Staff stated an order was already in place for temperature reducers for both sinks.

Potable Water-Bunkering

During active bunkering the analyzer and recording chart displayed a free residual chlorine of 2.20 ppm. The ship's manual test kit registered a free residual chlorine of 2.01 ppm and the inspector's test kit measured 1.88 ppm. On retest, the ship's manual test kit measured 1.84 ppm, 1.78 ppm, and 1.89 ppm, while the inspector's test kit measured 1.85 ppm and 1.88 ppm. Staff stated that the potable water chlorination system company specified a set point for bunker chlorination of only 2.20 ppm. The analyzer was recalibrated immediately.

Atmospheric vacuum breakers were installed on the potable water supply lines where dialysis machines connect in the medical center. Due to the higher health hazards associated with dialysis, a reduced pressure assembly or a pressure vacuum breaker device is required. In addition, the consumer advisories posted at the buffet lines were posted at waist line level and in small font, making them difficult to read. At the Neptune Lounge buffet dome, there was a consumer advisory posted but it was not specific to the cold smoked salmon offered in that station. At the Pinnacle Grill, the lunch menu had no consumer advisory and beef could be cooked to order; the dinner menu had a consumer advisory but chicken breast was identified as a cook to order item. At Le Cirque, the menu had a consumer advisory but beef and lamb could be cooked to order and they were not identified as such. At the Rotterdam Dining Room, the breakfast menu had a consumer advisory but eggs could be cooked to order and were not identified as such; the lunch menu had a consumer advisory but identified turkey as a cook to order item; the dinner menu had a consumer advisory but identified grilled chicken as a cook to order item. The room service menu had a consumer advisory but the steak was not identified as a cook to order item and the cold smoked salmon was identified as a fully cooked item; the breakfast door hanger had a consumer advisory but eggs were not identified as cook to order items.

Food debris and liquid stains soiled the carpet immediately below the half-wall style counters used as soiled plate/tray returns.

Food Service General-Decks

The decks were pitted, worn, and had recessed deck grout in several areas, including: lido galley, lido starboard breakfast buffet, crew galley, butcher shop, vegetable preparation room, pinnacle grill, and lido hamburger grill. In the main galley there was recessed grout surrounding the stainless steel scuppers in several areas and on the bulkhead/deck juncture in front of the beverage station. According to staff, most of these decks will be repaired during the next dry dock in 2012.

Galley-Port Side Juice Station

There was one small black fly behind the juice station. Flies were noted in this area during the previous inspection; however there were significantly fewer flies noted during this inspection. According to staff, several measures were put in place to decrease pests in several areas of the ship, including the lido juice station.

Galley-Starboard Forward

There were no test strips readily available to check the concentration of the HB Quat solution in front of the waiter station. Eventually staff were able to obtain an unopened test kit to measure the HB Quat sanitizing solution.

The inside of the ice chute of the ice/water machine was soiled.

Three stainless steel pans of pasta and two stainless steel pans of vegetables were not completely covered in walk-in cooler #25. In addition, one stainless steel pan of sliced ham was not covered in the fish prep freezer.

Serving utensils were not provided for 3 containers of sliced cheese, sliced turkey, and sliced ham on the cold buffet self-serve line. In addition, no serving utensil was provided for one container of fish on the hot buffet self-serve line. This was corrected during the inspection.

Galley-Warewashing

The plate surface of the in-use conveyor rack type warewash machine could not be measured above 152 °F by the inspector nor staff after two attempts using both a thermalabel and a stem thermometer. A thermalabel attached to the sanitizer spray nozzle rod measured over 180 °F by the inspector; however, the warewashing final rinse temperature gauge measured above 200 °F. In addition, steam was coming from the nozzles on the sanitizer spray rod. The warewash machine was taken out of service until it could be properly repaired.

Preparation Room-

One in-use cutting board was scored and stained.

The visible insulation on the inside of the water/ice machine behind the ice chute was heavily soiled.

Potable Water-Potable Water Bunkering Records / Pre-chlorination

The free halogen residual charts dating from 14 September 2010 to 23 October 2010 had recordings of free halogen residual levels continuously changing from 0 ppm to 5 ppm during the bunkering process. According to the staff and email correspondence, these dates matched the time that a new monitoring system was installed and it was hard to stabilize. No manual recordings were conducted. The malfunctioning device was corrected.

Potable Water-Bunker Stataion / Starboard

The top potable water connection was facing upwards.

Potable Water-

The annual test of all 16 testable backflow prevention devices were past due, with due dates dating as far back as August 2010. According to documentation in AMOS, a new test kit was identified on 5 February 2011 to initiate a purchase order.

Food Service General-Main Galley, Pinnacle Grill and Crew Galley

The deck surface was pitted in various areas, making cleaning difficult. The new deck prototype was installed in other galley areas and appeared to be in good condition. The staff provided details on the plan to install the new epoxy deck system in the remaining areas of the galleys. Some of these areas were noted in the previous VSP inspection report.

Galley-Dishwash

The in-use #1 dishwashing machine displayed a rinse tank temperature of 163°F, while the manual temperature of the rinse water was measured at 154-155°F on both the inspector's thermometer and that of the staff.

The two large in-use ice machines had an accumulation of a small white and gray colored granular material in the interior water bath surfaces.

Two unopened bags of cut raw spinach were inside the undercounter reach-in refrigerator with a food temperature of 47°F. They were removed and iced down immediately.

The underside upper stainless steel panel for the ice and water dispenser had large gaps around the dispensing chutes for both ice and water.

Galley-Dishwash

The area was not in use at the time, but water was leaking from the deckhead onto a small deck stand positioned immediately beside the clean dish landing table for the conveyor dishwashing machine.

Preparation Room-Provisions

The potato peeler was posted out of order on 31 July 2010 and staff stated that they were still awaiting the spare parts needed to repair this machine.

Other-Terrace Grill

Dust was accumulating and hanging from the open style deckhead above the buffet counter.

Four small black live flies were on the deckhead over and the bulkhead behind the juice dispensers and the fresh orange juice machine. This buffet station was not in service at the time of the inspection, but the clean equipment was present and subject to contamination.

There were three small black live flies circulating at the bulkhead behind the chest ice cream freezer and the clean stacked bowls on the counter immediately beside the bulkhead. This area was not in service at the time of the inspection.

The soft serve ice cream machine was posted out of order from 8 May 2010. Staff stated they are waiting on spare parts.

Galley-Potwash

There were 12 small black live flies on the deckhead over the clean pot racks and one small black live fly on the bulkhead at the entry door to the potwash. Staff members were cleaning soiled pots at the time and the clean racks were filled with clean pots and utensils.

Potable Water-Far Point Analyzer

The far point analyzer reading was 2.97 ppm. The inspector's test kit reading was 1.27 ppm and the operator's test kit reading was 1.28 ppm. This analyzer-chart recorder was newly installed approximately 10 days prior to the inspection and the engineering staff was working to get the system stabilized.

Potable Water-Far Point Analyzer

On 4/9, 4/20, 4/25, and 4/27 the far point analyzer charts overlapped by > than 6 hours.

Potable Water-Tank 50C

This tank was inspected on 6/25. The AMOS system log showed that the tank was disinfected at 50 ppm halogen, however there was no contact time recorded. In addition, there was no mention of flushing to less than 5 ppm.

Potable Water-

There were a few dates in the bunkering and production logs where it took more than 30 minutes to reach 2.0 ppm after the start of bunkering. On a few of those dates it took up to 4 hours to reach 2.0 ppm.

Food Service General-

The decks in the lido galley, pinnacle grill, and crew galley were difficult to clean due to worn deck finishes, rough surfaces and cracked deck materials.

The drain under the preparation sink was clogged.

There was soiled water under the technical compartment of the buffet station.

There were several stations where the passengers lined up within one meter of the foods on display and there were no side shields.

Per the staff, these products were cooled on ice and that was why there was no entry in the "blast chiller log" for these foods.

The cooling log entry for the grilled vegetable had a start temperature of 47°F. There was no log for the cooling of this food from hot temperatures.

There was no toilet tissue in the stall.

There was a yellow dot on the bottom of a milk pitcher and an orange dot on the side. Each color corresponded with a different discard time. According to the worker in this area the yellow dot was not removed from the previous 4-hour time period. Additionally, the worker stated that the pitcher was just rinsed with water between the 4-hour time periods.

There was a loose flange around the cold water line at the handwash station creating an opening in the bulkhead.

There were several dead flies in the covered recessed lights above the braising pan and the pressure cooker.

The reduced pressure assemblies for the whirlpool fill lines were located on deck 7. However, the cross connection control program listed them on deck 9.

Material Failure (Vessels)

The passenger vessel ZAANDAM discharged hydraulic oil while crew members were preparing a life boat to be lowered. One of the valves of the system was left closed in the wrong position, causing over pressure in the hydraulic line and a safety glass to burst. The hydraulic oil spilled from the ruptured safety glass and spilled onto the deck and a estimated 1/4 gallon discharged overboard creating a 20 to 30 ft. rainbow colored sheen on the water’s surface. The vessel crew immediately shut down the hydraulic pump and stopped the source of discharge and used absorbent pads to clean the remainder of the spill off of the deck.

SCC received a call from the staff director of the P/V Zandaam that there were two injuries onboard to report. The second report was of a femaile who was getting out of the jacuzzi and slipped which resulted in a fracture in her lower spine. On board medical team sent the xray to a shoreside medical team who determined the injury could have been preexisting. Their next port of call is Hilo, HI ETA 14MAR13 at appx 1130W. Injured passenger will be evaluated at the hospital upon arrival into Hilo.

A medical emergency was raised for a guest in stateroom 7083 on the vessel Zaandam. Medical proceeded to the scene and after a brief evaluation of the situation, commenced CPR to an unresponsive passenger. The 'crash cart' arrived shortly after, however after the efforts of both the doctors and nurses, they were not able to resuscitate the guest.

SCC received a phone call from M/V ZAANDAM, reporting a crewmember injury. The crewmember injury occurred in Haines while the crew were conducting a "routine light boat drill." The boat driver looked down to observe a gauge and the hardhat he was wearing impaired his vision. As this occurred, his face was struck by the "master link of the full block," which bruised his left cheekbone. The ship's medical staff x-rayed his face and it appeared fine. Crewmember had medical follow up the next day in Juneau and discovered additional fractures, resulting in medical discharge from vessel.
