Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Food Service General-Decks - Main Galley and Crew Galley
The vessel had several food area decks where the deck paint was in poor repair. The paint was peeling and pitted in some areas of the buffet preparation room, main galley (decks 2 and 3), crew galley and service lines and lido service lines. Deck repairs are planned during the two-week dry-dock in April 2015.
Galley-Deck 2 - Pinnacle Grill
The soap in the dispenser at the handwashing sink in the middle of the grill counter had been diluted with water to the point that it would not produce suds.
Galley-Deck 2 and 3 - Main Galley
Absorbent foam padding was found on the outer deflector panel and covering the openings where the refrigeration lines entered the ice maker section of the machine from the compressor area. Moisture was found in this padding on two of three machines checked that could leak into the ice water bath.
Preparation Room-Deck A - Lido Prep
The underside of the back plate on a previously cleaned food slicer had old food soil on it. A black residue was found on the small nozzles in the previously cleaned salad ingredient washing machine.
Provisions-Deck A - Dry Stores
Old food debris and dirt was noted behind the shelving foot pads near the bulkhead along the back left corner of the room.
Provisions-Deck B - Dairy Cooler
A small amount of food debris was noted on the deck near the shelving foot pads along the left bulkhead.
Buffet-Deck A - Crew Mess
A grease film and soil was noted in a wiping cloth storage bucket at the end of the crew mess line. The solution was changed while the team was still in the area.
Buffet-Deck 9 - Buffet Counters
Open-woven bread baskets were being used as a direct bread-contact surface at all of the buffet counters. Accumulation of bread crumbs were found in the difficult to clean weaving.
Buffet-Deck 9 - Cheese Counter
Regular knives and forks were being used for passengers to slice bulk cheese from platters on the display. The short handles put the passengers hands in close proximity to the cheese that was being sliced. These handles were observed resting on other platters of cheese between the uses by the passengers.
Medical-Crew Gastrointestinal (GI) Illness Reporting
A cast member had onset of GI illness symptoms at 9 am on 3 March, went ashore in Costa Rica, ate food there for lunch and reported to the ship's medical facility at 5 pm. A beverage manager had onset of GI illness symptoms on 25 February at 5:30 pm, but did not report to the medical center until 26 February at 7:55 am. A bar tender had onset of GI illness symptoms at 12:30 pm on 25 February, worked from 9 am to 3pm, and ate lunch and dinner in the crew mess before reporting to the medical center at 5 pm. A dining room runner had onset of GI illness symptoms at 6 am on 14 February, ate breakfast in the crew mess, and reported the symptoms to the medical center at 10;15. A guest relations associate had onset of GI illness symptoms at 4 pm on 7 February, worked from 5 to 6:30 pm, and reported to the medical center at 7:45 pm. Each of these individuals were disciplined for their non-compliance. A similar violation was noted on the last VSP inspection on 19 December, 2014.
Medical-GI Illness Food and Activity Questionnaire
The ship uses a fleet form for the 72 hour food and activity questionnaire. This form did not have data fields specific to the place where foods were consumed on the ship for crew members, as is the case in the standardized questionnaire from the CDC Vessel Sanitation Program. In some crew food histories the place for meals was written in on the side, while others provided no information for any of the meals on where it was consumed.
Pantry-Housekeeping Pantries on Deck 1 and 8
In the deck 1 pantry (P.1.2.01), inside ice machine #40 there was a grey foam pad glued to the exterior side of the removable plastic panel covering the ice cuber panel. This foam pad was absorbent and had surface damage from cuts in the center and upper portions. Additionally, the lower left quarter of the pad was saturated with water. In the deck 8 pantry (P.8.4.02), the foam pad on the exterior of the plastic cuber panel was damaged along the soft surface and the lower half of the pad was loose from the panel with old glue residue still present on both the panel and the pad. A clear water mark at the middle of the pad was evidence of some past water saturation that likely caused it to loose adhesive quality. The pad was dry during the inspection.
Recreational Water Facilities-Deck 9 Forward Whirlpool and Starboard Whirlpool
The whirlpool spas on deck 9 were all empty of water and netted. The anti-entrapment drain covers in the center bottom of both the forward whirlpool and the starboard whirlpool were broken open from damage to the top surface. Ship staff replaced each drain cover during the inspection.
Bar-Deck 12 - Midship Lido Bar
A soft fabric circular deckhead was noted over the front bar. Staff stated this material was to be replaced with a hard, durable easy to clean material in the upcoming April dry dock.

*General Comment-Cooling Logs - Kudos
The vessel was placing a sticker with the corresponding discard date on each page of the cooling logs. This allowed the cooks to easily identify the correct discard date for every food item on the log, minimizing the risk of errors. This also allowed the management to easily identify items when cross-checking the prepared foods under temperature control against the cooling logs.
Food Service General-Decks
The vessel had recently painted several food area decks, but the paint was not applied properly and was peeling and pitted in some areas of the buffet preparation room, main galley (decks 2 and 3), and lido service lines. New paint had been ordered and would be reapplied. In addition, several areas were planned to be replaced with diamond plate stainless steel during the March/April 2014 dry dock.
Galley-Dishwash Area
Condensate was collected on the bulkhead, deckhead, and exhaust hood around the soiled end of the conveyor dishwash machine. Some of the condensate was also dripping from the edge of the exhaust hood to the soiled landing. The ventilation in the area was sufficient, but it was determined that the air supply was blowing too strongly on the exhaust hood and surrounding area, causing condensation when the steam from the dishwash machine contacted the exhaust hood, deckhead and bulkhead. According to the staff, a new booster had been installed in the A/C unit two days prior and the system still needed adjustment and balancing. The air supply was adjusted and the condition was improved when the inspector returned to the area.
Galley-
A deck pallet with several bags of rice was stored in between two garbage bins. The bins were not in contact with a rice. A new location for the pallet of rice was identified.
Galley-Deck 2 Hot Galley
Condensate was collected on the deckhead above one of tilting pans. Food was cooking in the pan with the lid open, generating a large amount of steam. The lid was immediately lowered and the condensate wiped off. No food was impacted.
Galley-Deck 3 Dishwash Area
The final rinse temperature gauge on the flight-type conveyor dishwash machine was reading between 210 and 220°F. The final temperature at the utensil surface was below 170°F and no steam was coming from the nozzles, indicating that the gauge was not working properly.
Buffet-Starboard Salad Station
The counter between the cold wells was cracked in several locations. According to the staff, this countertop was scheduled to be replaced during the March/April 2014 dry dock.
Buffet-Starboard Bistro Line
Water was dripping from the deckhead to the counter below. The area was not in service during the inspection. It was determined by the vessel staff that there were no pipes overhead and that the likely source of the water was condensate from the A/C pipes. The deckhead panels were wiped down.
Medical-Acute Gastroenteritis (AGE) Surveillance Log
For the 3 previous cruises, the total numbers of passengers and crew were entered as 0 on the AGE surveillance logs. The totals were found on another form in Medical. In addition, for the voyage dated 10/27 to 11/17, it was indicated that a crew member had an illness onset at 0800 on 11/16 and reported to the clinic at 0800 on 11/17. The nurse's notes indicated that the actual onset was 1800 on 11/16, not 0800.
Integrated Pest Management-Rat Guards
The 2 forward mooring lines did not have rat guards.
Medical-Crew Reporting
On 8 December a food handler (cabin steward) had an onset of AGE symptoms at 1330 but did not report to medical until 1700. The work record indicated that he worked until 1530 that day. The crew member was counseled. In addition, the vessel had a program where crew members were issued notices twice per month regarding immediate reporting of AGE symptoms and this crew member had received the notices.

Several holes were made on the light shield to allow proper air flow to the exhaust vent above the light bulb. These holes would allow pieces of glass to fall through if the light bulb would break.

A significant amount of dust was accumulated on the light bulb and around the exhaust vent in the light fixture. This dust was not contained in the light fixture because of the several holes that were made on the light shield to allow proper air flow to the exhaust vent above the light bulb.
Galley-Mid-Ship Food Transportation Lift
The bulkhead ledge around the lower half of the lift had an accumulation of soil. Soil was also noted around the emergency telephone box.
Galley-Lido Warewashing
A crew member reached over the center table with a squeegee to grab an empty rack from another crew member. The table was covered with clean plates and utensils. The chief steward noticed this action and immediately removed all plates and utensils that were under the squeegee to be re-washed. The tables was also cleaned and sanitized before being used again.
Galley-Deck 2 Garde Manger Cold Room 27
A shrimp mixture that was combined with a scallop mixture to create a shrimp and scallop cocktail was not listed on the cooling log. The scallop mixture and the final cocktail product were listed on the cooling log.
Galley-Ice Machines 2.00/2.01/3.00/3.01
The far end of the ice cubers by the water dispensing nozzles were heavily soiled with an accumulation of a black residue.
Galley-Deck 2/3 Flight Type Dishwasher
The attached data plate for both machines listed the recommended water temperature for Wash Tank 1 to be 150°F. The actual recommended water temperature for Wash Tank 1 was 130°F. Both machines also did not have a conveyer speed listed on the data plates.
Galley-Crew Pot Wash Handwash sink
The flanges around the hot and cold water lines were loose and not sealed against the bulkhead exposing gaps and seams.

The deck in front of the deep fryers was cracked, chipped, and peeling in several location.

Medical-Crew Acute Gastroenteritis (AGE) Reporting
A guest relations attendant (nonfood worker) had an onset of AGE illness symptoms on 4 January 2014 at 0700 but did not report to the medical center until 6 January at 0830. A review of the crew member's work record indicated that they worked while symptomatic. A shop staff (nonfood worker) had an onset of AGE illness symptoms on 25 January 2014 at 0900 but did not report to the medical center until 26 January at 0840. A review of the crew member's work record indicated that they worked while symptomatic. The vessel had specific training informing crew members to report to the medical center after their first AGE symptom. These two crew members received disciplinary action.
Recreational Water Facilities-Fecal/Vomit Accident Plan
The fecal/vomit accident plan indicated that the pH should be maintained between 7.0-7.8 instead of between 7.2-7.5.
Children Area-Toilet Room
The toilet was greater than 11 inches from the deck and the handwashing station was greater than 22 inches from the deck. There was one step stool in the toilet room instead of two.

Medical-Passenger Medical Records
The medical log/records for passengers with AGE from cruise 480 (6 July - 13 July) and earlier were not readily available for review on board. These records were eventually retrieved from the ship's company by the end of the inspection. The AGE surveillance logs and other required records for these cruises were available.
Recreational Water Facilities-Turnover Rate
The aft starboard whirlpool did not meet the required maximum turnover rate of 0.5 hours. The ship's calculated turnover rate for this whirlpool was 0.76 hours.

The counter-mounted coffee cream dispenser had not been used in at least two years. According to staff, this equipment was not needed for the operation.

Galley-Deck 2 Main Galley - Saucier
One stock pot half-filled with beef stock was at a holding temperature of 115-117°F on the range top. A larger full stock pot of white sauce was on the range top holding at a temperature of 122-132 °F. Both were transferred at 7:30 this morning from the larger steam kettles where they were reducing overnight, according to staff. Other stocks on the same range were above 135 °F. The beef stock and white sauce were reheated and re-checked, and both were over 170 °F.
Galley-Deck 2 Main Galley - Pastry
The two corner shelf sections opposite the evaporative condenser in the freezer inside the cold room were below the minimum light level of 110 lux.
Galley-Deck 2 Main Galley - Pastry
With the doors closed, there was a gap large enough to pass folded papers thru where the lower door joined the frame on the three door undercounter reach-in refrigerator (doors 2.19A and 2.19B).
Galley-Deck 2 Main Galley - Pinnacle Dishwash
The in-use conveyor dishwashing machine was producing erratic final sanitizing temperatures at the dish level. Some were below 155 °F and a few exceeded 160 °F. Further investigation revealed only one of the 4 lower spray arm nozzles was spraying in the final rinse, and one of three upper arm spray nozzles was spraying. The machine was repaired and re-tested later in the inspection.
Galley-Deck 2 Main Galley - Pinnacle Dishwash
The in-use conveyor dishwash machine was not spraying water from one of the three upper spray arm rinse nozzles, one of three upper spray arm final rinse nozzles, and three of four lower spray arm final rinse nozzles. The machine was later repaired.
Galley-Deck 2 Main Galley - Coffee Station
The previously cleaned #2 espresso machine milk container had a sour odor and was soiled in the milk refrigerator attached. The container was inverted, still wet inside and the base of the refrigerator was wet. There was also a milk and water liquid residue present inside the milk transfer tube, which was also previously cleaned. The milk container and the transfer tube were removed and re-cleaned.
Galley-Deck 2 Main Galley - Coffee Station
The utility cables beneath the #1 espresso machine were coiled on the counter below the machine, making cleaning difficult.
Galley-Beverage Stations and Room Service - Deck 2
The power cables to the various counter model toaster ovens in the beverage stations and room service pantry were draped on the counter top, making cleaning difficult.
Galley-Deck 3 Main Galley - Entremetier
With the doors closed, there was a gap large enough to pass folded papers thru where the lower door joined the frame on the three door undercounter reach-in refrigerator (doors 3.21B and 3.21C).
Galley-Galley Dish and Pot Wash Machines
The following warewashing machines were in disrepair during this inspection: main galley deck 2 and deck 3 pot wash machines, main galley deck 3 conveyor glasswash machine, and lido conveyor glasswash machine.
Food Service General-Conveyor Rack-Type Machines
The dataplates on the rack conveyor machines were posted with speeds in baskets per minute. There was no statement of minimum transit time as required in 7.5.1.2.3 of the 2011 VSP Operations Manual.
Galley-Deck 3 Main Galley - Soup Station
The 4 bracing metal rods used to protect pans from moving on the range top were stored beside the range with the ends in contact with the deck. Staff immediately removed them for cleaning.

The counter model reach-in refrigerator on the back bar counter was mounted only 1/2 inch above the counter making cleaning difficult.

One live fruit fly was observed inside the front counter dessert display refrigerator, which was filled with individually plated servings of various dessert items.
Recreational Water Facilities-Aft and Mid Pools
Each swimming pool had two gravity drains greater than one meter (three feet) apart constructed of a standard design and no alarms were installed. A local purchase order was placed on 5 February for ASME A112.19.8 compliant drain covers. The letter for the standard drain covers did not indicate the ship's name or the recreational water facilities which it pertained to.
Potable Water-
The ship was using Tank Guard 412 for coating the interior of some of their potable water storage tanks, however there was no written documentation of approval from the certification organization. The manufacturer indicated the product was NSF Standard 61 certified, but it could not be found on the NSF website. Staff contacted the manufacturer to for more information on obtaining the written documentation. This product has not been used during the most recent coatings of potable water storage tanks.
Potable Water-Far Point
For the manual testing of the far point, the staff was using a Hach meter with Orbeco reagents. Staff stated new Orbeco meters were ordered.
*General Comment-
Management of the medical, engineering, housekeeping, pest control, and youth center were very knowledgeable and had strong recordkeeping practices.

The chilled water lines for the ice machines were not uniquely identified.

Numerous stacks of clean plates on the clean storage table and plate trolley were not covered or inverted. A crew member inverted the top plate of each stack.
Dining Room-Pinnacle Grill Menus
There was a consumer advisory statement at the bottom of the menus, but there was no asterisk beside all of the animal-derived menu items to clearly identify them as the foods which the advisory applied to.
Buffet-Crew Mess
The milk inside the bulk milk dispenser was measured at 44°F using the inspector's thermometer. The milk was discarded.
Preparation Room-Out of Order Dishwash Machines
Numerous dishwash machines were out of order in the preparation rooms. This included the bakery hood-type dishwash machine, the lido preparation undercounter dishwash machine, the fish preparation undercounter dishwash machine, and the butcher undercounter dishwash machine. Due to these out of order dishwash machines, excess items were in the deck 2 main galley dirty storage area.
Recreational Water Facilities-Chart Recorders
Not all of the recreational water facilities were equipped with chart recorders or electronic data loggers with security features to monitor the pH and free residual halogen. Staff stated a plan was in place to complete the installation over the next year.
Recreational Water Facilities-Aft Whirlpool
Two of the small suction antientrapment drain covers and the large antientrapment drain cover were not ASME A112.19.8 compliant. A safety vacuum release system was installed. In addition, the large drain cover was loose on one side because a fastener was missing and one of the two small drain covers was mounted upside down. According to the staff, an inventory of drain covers would be completed and new compliant covers will be ordered as needed. This whirlpool was not open at the time of the inspection.
Ventilation-Air Handling Units
Technical water was used for cleaning the air handling units. According to the staff, potable water lines will be installed during the December 2012 dry dock.

Fire Fighting
Sprinklers in fixed fire extinguishing systems shall be corrosion-resistant to marine atmospheres. Several sprinkler heads in forward sewage room was found to be ineffective due to excessive corrosion.
Construction/Loadline
The means for securing and maintaining weathertightness shall ensure that tightness can be maintained in any sea condition. Deck 4 forward weathertight doors did not secure to maintain weather tightness.

Passenger ships carrying more than 36 passengers shall be equipped with fire detection of an approved type for accomodation spaces. Fire detector 116.203 in passenger cabin would not alarm when smoke spray was applied.
Lifesaving
Passenger ships engaged on international voyages shall carry totally enclosed lifeboats complying with 4.6 and 4.4 of the code. One of lifeboat 12's two engines has not been operable in several months. The lifeboat is equipped with two separate engines with separate propellers, however one has been out of operation for seven months despite the ship's crew repeatedly requesting new parts.

Passenger ships of 4000 GT and upwards shall be equipped with three independently driven fire pumps. During a test of the fire main system, the #1 fire pump could not operate due to excessive leaking of water from the packing glands.

Vessel reported damage to life boat davit. The damaged part on the davit is the securing hook on the aft davit of lifeboat 10, the bracket it hooks onto is bent out of shape, causing a dent in the davit arm, and the davit arm bulges out on the side.

Chief Officer of Motor Vessel ZUIDERDAM contacted Sector Command Center reporting a passenger death on board. Female was found unresponsive in her cabin at 0600, medical personnel were dispatched, and she was pronounced deceased at 0610.

Material Failure (Vessels)
On July 8th, 2010 the P/V Zuiderdam's #3 liferaft davit failed during a drill. The scissor arm failed to lift upwards and bent. The vessel had enough lifesaving capacity for all personnel onboard and re-assigned individuals from raft #3 to other available rafts. Class issued amended PSSC for reduced lifesaving capacity until fixed. 15JULY2010 - CG MSD Ketchikan attended vessel and witnessed operational test of fixed davit. Davit failed originally due to corroded pin in the davit arm. Replaced corroded piece and replaced bent davit arm. Class is meeting vessel on 16 July 2010 in Vancouver, Canada to clear discrepancy and re-issue full PSSC. 20JULY2010 - MSD Ketchikan received class report of completed repairs.
