Issue:
Based on staff interview and review of facility documents, the facility failed to prevent the unauthorized disclosure of Patient A ' s health information (PHI) when a facility employee faxed a lab test result for Patient A to a local resident instead of Patient A ' s doctor. Findings:On 2/10/12 at 2 p.m., the facility ' s investigation of the incident was reviewed with the Compliance Officer (CO). A review of the investigation showed that on 3/30/09, a facility employee was going to fax Patient A ' s test results to the patient ' s physician. Documentation showed that the employee copied the fax number wrong and the test results were faxed to a private home instead. The resident at the home contacted the facility and destroyed the faxed copy. On 2/10/12 at 2:10 p.m., the CO was interviewed about the incident and she stated that the involved employee self-reported the incident as soon as she was made aware of the error by the caller. Per the CO, the employee was given a disciplinary document that reinforced the importance of verifying all fax numbers prior to faxing any documents.
