Issue:
Based on staff interview and review of facility documents, the facility failed to prevent the disclosure of Patient A's health information (PHI) while in the "Rapid Care" department (RCD), when another RCD patient (Patient B) received Patient A's discharge instructions and prescriptions.Findings:On 5/9/12 at 11:40 a.m., the facility's investigation of the incident was reviewed with the Compliance Officer (CO) over the telephone. A review of the investigation showed that on 11/28/09 an RCD nurse gave Patient A's discharge instructions and prescriptions to Patient B. Patient B called the RCD about getting the wrong discharge instructions and prescriptions and he returned them to the facility and received the correct paperwork instructions.
