Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Trolleys and shelves with clean and soiled linens and towels, clean and soiled glasses, cleaning chemicals, and garbage taken from the staterooms during turnaround procedures were among the boxes that made the room very disorganized, difficult to clean, and offering limited protection for clean items.

No cooling logs were available for containers of mix salad and cabbage stored in the walk-in refrigerator. The internal temperature of each container was less than 41°F when measured.

The bulkhead behind the first condenser unit one the right had a significant amount of ice accumulation. No ice was observed on any stored boxes of food.

Buffet-Horizon Court

Several potentially hazardous food items, such as sandwiches and cut melons, were out on the buffet but not labeled with a discard time (the buffet was open for more than 4 hours). The discard labels were placed behind the signs with the name of the food and were not affixed to each individual tray or container of potentially hazardous foods. Also, two serving bowls of lettuce were out on the salad bar counter with no discard label. Staff stated this salad bar was closed at 3:30PM for the emergency boat drill and everything was to be discarded but the posted time control plan for the area listed the operation hours of the Horizon Court on embarkation day to be from 11:30AM to 11:00PM.

Two small bowls of cut melons with a discard time of 10:00AM were in the time controlled reach-in refrigerator unit 11071 when the inspector arrived to the area after 1:00PM. The bowls were immediately discarded.

Medical-Pantry

A wet wiping cloth was draped over the side of the chemical sanitizing solution container. The cloth was placed inside the solution by the VSP inspector.

Recreational Water Facilities-Fecal Accident

Fecal accident records for the operations of the Calypso port whirlpool on 28 December were inconsistent with the halogen/pH analyzer-chart recorder chart for that day. As a result, it was unclear if a proper fecal accident response took place. Specifically, records indicated that on 28 December a formed stool fecal accident occurred in the Calypso port whirlpool at 1815 and the facility was closed. Disinfection was recorded as being done from 1830 until 0030 of 29 December with halogen levels of 50+ ppm and pH levels of 7.2 - 7.5. However, the chart for that period of time indicated halogen levels between 6.0 and 7.0 ppm and pH levels around 7.8.

The hair and lint strainers and hair and lint strainer housings for the 17 RWFs were not cleaned and disinfected weekly as required. Specifically, records indicated that the hair and lint strainers and hair and lint strainer housings were cleaned and disinfected for the 6 pools and 11 whirlpools on 5 January, 1, 6, & 24 December, 1, 13, & 24 November, 9 & 20 October.

The halogen levels of this open facility were measured by the VSP inspector and the staff at above 10 ppm several times from different places of the RWF. The facility was closed.

The analyzer-chart recorder was inaccurate, measuring halogen levels between 4.9 and 5.1 ppm while manual tests done from before the analyzer probe by the staff indicated halogen levels of above 10 ppm. Staff corrected this by replacing the probe.

Galley-Cafe Caribe - Dishwashing

Heavy condensate was noted on the inside of the canopy at the soiled end of the machine. This condensate was dripping back to the soiled area of the dishmachine. A ventilation exhaust had been added at this end of the machine during the recent dry dock. Today's humidity appeared to be originating in the steam rising from the under conveyor drip rinsing system that was operating at 130°F that was not being captured by the exhaust fan.

The soiled area of the dishwashing was heavily overloaded with racks and stacks of dirty utensils. A rack of coffee carafes lids was found stored on the deck. Other racks were stored on the tops of garbage cans. The Horizon Court dishwashing machine was malfunctioning which rerouted extra dishware to this small dishwashing area and may have contributed to this overload.

Medical-Crew Member Reporting

On 1 July, a Laundry Steward had an onset of acute gastroenteritis (AGE) symptoms at 18:30, but did not report to the medical center until 2 July at 09:40. His time sheet indicated he worked on 1 July from 20:30 - 23:00 and on 2 July from 01:00 - 03:30 and 04:30 - 08:30. This crew member received disciplinary action for late reporting while symptomatic.On 22 June, a Buffet Steward had an onset of AGE symptoms at 11:30 and reported to the medical center at 12:45. His time sheet indicated he worked from 09:00 - 13:00. This crew member did not receive disciplinary action.On 14 June, a Buffet Steward had an onset of AGE symptoms at 13:45 and reported to the medical center at 15:50. Her time sheet indicated she worked from 14:00 - 16:00. This crew member did not receive disciplinary action.

Medical-Crew Members Working While Ill

Between 14 June and 15 July, a review of the crew members AGE symptom onsets and time of reporting to the medical center, three non-supervisory food workers knowingly worked while symptomatic. One of these crew member received disciplinary action. Food workers receive extensive training about AGE symptoms and proper reporting procedures.

Potable Water-Mineralizer

The water line was striped to indicate potable water from the mineralizer to the production chlorine and pH injections points. Staff stated this had been corrected later during the inspection, but was not verified by the inspector.

Food Service General-Time Control Plans

The time control plans stated 'all potentially hazardous food items under time control are consumed or discarded within 4 hours and at the end of service.' The plans did not indicate potentially hazardous food items would be labeled to indicate the time 4 hours past when the foods were removed from temperature control. For many food areas, dinner service periods were longer than 4 hours.

Many coffee mugs out for passenger service were wet with condensation and dripping onto their storage trays. It appeared the mugs were not allowed to thoroughly air dry after being washed.

The light intensity could not be raised to 220 lux at all of the waiter stations. This was written on the last two inspection reports. Staff stated new light fixtures were received in November 2013, but they were too large. The correct light fixtures were received about one week prior to the inspection. Over the past week, electricians began installing additional lights in the Palm Dining Room. The entire project was scheduled to be completed by mid-March.

Water continuously dripped from the outside of the machine hood overhang between the machine and the clean storage area. It appeared the water was coming from a seam underneath the sealant used on the overhang. Also, water dripped from a seam between two deckhead panels above the clean storage shelf behind the potwash machine. No clean items were impacted.

During inspection of the in-use potwash machines, the pressure gauges fluctuated between 20-30 psi during use. The manufacturer's data plates indicated the rinse water pressure to be 20 psi, but did not indicate a range.

At the starboard (#1) beverage station, the inside of the juice machine dispensing ports were soiled with old juice residue. On the ice/water machine, the inside of the ice chute was soiled with a black material. These machines were not in operation. At the port (#2) beverage station, on the ice/water machine, the inside of the ice chute was soiled with a black material. This machine was in operation.

Buffet-Officer's Mess

Plates of ready-to-eat apples and pears were out for officer self-service, but did not have a serving utensil.

Medical-Employee Reporting

During the 1-8 February voyage, a Waiter had three episodes of diarrhea beginning at 08:30 and did not report to the medical center until 10:00. Employee work records indicated the crew member worked during this time. During the 25 January to 1 February voyage, an Accommodations Attendant had two episodes of diarrhea beginning at 13:00 on 27 January and did not report to the medical center until 17:30. Work records indicated this crew member was not working during this time. A salon department worker had three episodes of diarrhea beginning at 11:00 on 29 January and did not report to the medical center until 16:45. Work records indicated this crew member worked during this time. During the 18-25 January voyage, a Photo worker had two episodes of diarrhea beginning at 11:00 on 21 January and did not report to the medical center until 22 January at 16:30. Work records indicated this crew member worked during this time. A Pastry Chef had four episodes of diarrhea beginning at 15:00 on 22 January and did not report to the medical center until 19:00. Work records indicated this crew member did not work during this time. Documentation for disciplinary actions was observed for all of these instances. These findings were similar to the last USPHS inspection in which there were three instances of late reporting by crew members with acute gastroenteritis (AGE) symptoms.

Medical-AGE Surveillance Log

The AGE surveillance log for voyage 8-15 February had two crew members listed that were carry-overs from the previous voyage, but there was no documentation in the notes or underlying illness column that they were from the previous voyage. The crew members were listed as non-reportable AGE cases and were still in medical isolation.

Potable Water-Bunkering Shore-side Pretest

There was no documentation of shore-side free halogen residuals or pH values taken at the shore-side water supply before potable water bunkering dating back to November 2013. The ship bunkered potable water approximately 20 times since November 2013.

The two bunker hoses being used for active potable water bunkering were not labeled 'Potable Water Only' at the ship's connecting end.

Recreational Water Facilities-Drain Covers

A suction drain cover was missing on the side wall located near the shower at the Neptune Pool. Also a suction drain cover for the message therapy system was missing at the Spa starboard whirlpool. Both recreational water facilities were filled with water, but were closed and netted during the observation.

Food Service General-Room Service Staging Pantries

A room service staging pantry identified as Trolley Wash 8641 (deck 8 crew corridor) had an inner locker in which food service items were stored with a miscellaneous assortment of nonfood items. The food service items included boxes of packaged forks, wine glasses, unboxed service bowls that were stored on top of and in contact with boxes of hand sanitizer, wood polish, tables, dust pans, black tubs, and other cleaning equipment. A similar finding was observed in Trolley Wash 9367 (deck 9 crew corridor) where single service paper food service items such as napkins, cups, and bowls were stored with and in contact with cleaning chemicals, electrical cords, cleaning buckets, unfolded towels, and other cleaning items. Several metal spoons in wrapping were also laying on the deck.

Pantry-Calypso Bar

Two pitchers of milk kept on time control inside the milk refrigerator on the counter next to the espresso machine were not discarded as per the labeled discard label. The time of the finding was at 1:27pm and the discard label showed 1:00pm. Also the refrigerator was not physically labeled as being on time control.

Pantry-International Cafe

The light intensity was less than 220 lux at the back left counter next to the counter-mounted microwave. A portable warmer holding a container of soup was positioned in this area. A deckhead light in the center of the pantry was burnt out, which may have contributed to the light deficiency.

Two room service staging pantries identified as Trolley Wash 8641 (deck 8 crew corridor) and Trolley Wash 9367 (deck 9 crew corridor) contained food service and nonfood items, as well as poisonous materials such as hand sanitizer and wood polish.

The cooked-to-order eggs and the smoked salmon and smoked trout were not specifically identified in the consumer advisory statement. Only the general consumer advisory was posted at the buffet.

The light intensity could not be raised to a minimum 220 lux on all of the counter surfaces of the waiter stations. This is a repeat violation from the previous inspection report. Staff was able to provide documentation that light fixtures have been ordered to correct the issue. The light fixtures will arrive 5 Nov 2013 and be installed shortly thereafter.

The deck had numerous sections of missing and recessed deck tile grout or broken tiles. This was noted in the previous inspection report for every galley. Progress has been made in every other area of the vessel.

According to the blast chiller log, red pepper salsa prepared on 26 September was placed in the blast chiller at 8:00PM at 128°F and cooled to 62°F at 10:00PM and 38°F at 12:00AM. There was no written explanation for why the cooling did not start until the food was 128°F.

The cook in charge of checking the cooking temperatures of the thin pieces of fish did not have a tip-sensitive thermocouple.

Buffet-Crew Mess

The discard labels for several containers of potentially hazardous foods were placed on the outside cabinet door and not on each individual container. One discard label was also used for all three containers of chicken salad on the cold well.

A leak in the deckhead seam was dripping onto the food preparation table. The table was not in use at the time.

Condensation was accumulating on the entire length of the right bulkhead/deckhead juncture. An ice patch was present in the top right corner of the bulkhead closest to the door. This is a shared bulkhead with the poultry freezer.

Medical-Food Worker Reporting and Isolation

On 1 September, a Junior Waiter experienced one diarrhea episode at 07:20 and one vomiting episode at 07:30. She reported to the medical center at 09:00 that day. According to her time card, she worked from 06:00 - 08:00. She returned to work at 06:00 on 3 September. This was not a complete 48 hours of medical isolation. On 7 September, a Junior Waiter experienced one episode of diarrhea at 14:30 and reported to the medical center at 17:15 that day. According to his time card, he worked from 17:00 - 18:00 that day. On 12 September, a Food and Beverage Supervisor experienced four episodes of vomiting starting at 07:20. He reported to the medical center at 08:15. According to his time card, he did not work 12 and 13 September, but returned to work at 07:00 on 14 September. This was not a complete 48 hours of medical isolation.

Potable Water-Microbiological Testing

The incubator used to analyze water for E. coli contained two samples from the shoreside and potable water tank #4. These samples were collected on 25 September. The thermometer inside the machine indicated the temperature was 37.1°C, and lowered to 36.0°C. The mercury thermometer at the top of the machine remained at approximately 34.5°C. Staff stated they use the inside thermometer because it was more reliable. According to the manufacturer's specifications for analyzing E. coli, water samples were to be incubated at 35°C +/- 0.5°C. The two water samples were discarded.

Potable Water-Sand FIlter Sedimentation Test Resuts

The sand filter sedimentation test results were not clearly documented. According to the electronic work order/maintenance database, procedures for inspection of sand filter media state that 'if a measurable layer of sediment is within or on top of the filter media or fine, colored particles are suspended in the water, then the organic loading may be excessive, and the media replacement must be considered.' According to the staff, if the test results contained particles, an unscheduled work order would be made to replace the media. If the sedimentation test results were clear, nothing was documented, and only the next scheduled test date was noted.

Recreational Water Facilities-Pad 16 (Kid's Paddling Pool)

There was confusion whether or not children who were not toilet trained were allowed in this pool. According to the Youth Center Manager, when asked who used this pool, her response was kids ages 0, 1, 2, 3, 4, and 5 years old. During further discussion, she implied that kids could not use the pool if wearing diapers, yet did not disagree that kids ages 0-1 year old could go in the pool naked if accompanied by their parent. The vessel did not have a variance for allowing children in diapers or who were not toilet trained in this pool, nor was the pool properly maintained for this type of activity.

On 7 September, a Junior Waiter experienced one episode of diarrhea at 14:30 and reported to the medical center at 17:15 that day. According to his time card, he worked from 17:00 - 18:00 that day. On 12 September, a Food and Beverage Supervisor experienced four episodes of vomiting starting at 07:20. He reported to the medical center at 08:15. According to his time card, he did not work 12 and 13 September, but returned to work at 07:00 on 14 September. This was not a complete 48 hours of medical isolation.

The list of tested reduced pressure (RP) assembly backflow prevention devices along with many other types of preventers and air-gaps had future dates under the column inspection date or T date, such as 30 May, 2013, 3 September 2013, 11 October 2013, and 20 November 2013. Some of these data filled forms had the words, 'due to' handwritten into the form just below these dates, while others had no writing. In reviewing the pressure differential test results for the RP backflow preventers from the 2011 list, there were 49 RP's tested and the pressure values noted on test 1 and test 3 for every single one was an identical number to the same preventer test results for the 30 May 2013 testing. In comparison, the 2010 list of RP backflow preventers had mostly different values recorded for the same preventers under both the test 1 and test 3 than what was provided on the 2011 RP test log. Clearly the 2011 test data was simply copied over into the log form for RP backflow preventers dated 30 May 2013.

Potable Water-Records

In the backflow preventer inspection logs there were several backflow preventers listed under two log field columns titled device type and model as reduced pressure backflow preventers (RPZ). This included air-gaps and continuous pressure model backflow prevention devices, which were all titled 'RPZ' as device type and given model numbers such as RPZ - N9 or RPZ 1' under the model column.

Recreational Water Facilities-Spa Area Pool

The spa pool was open for use and the free chlorine concentration as measured by the inspector's test kit was 0.28 and 0.35 ppm for two consecutive tests. The ship used a different manual test kit and measured 0.64 and 0.76 ppm on two consecutive tests. Both tests from the ship and the inspector were from different parts of the pool. The spa pool was closed and netted immediately upon obtaining these test results.

The ship's test kit measured 1.13 ppm and 1.09 ppm on consecutive tests. Both test kits were calibration checked using the secondary standards held by the ship onboard and both kits were verified thru their respective secondary standards to be accurate.

Norovirus Outbreak on Jan 25, 2014

Symptoms: vomiting, diarrhea

In response to the outbreak, Princess Cruises and the crew aboard the ship took the following actions:Increasing cleaning and disinfection procedures according to their outbreak prevention and response plan,Making announcements to both notify onboard passengers of the outbreak and encourage case reporting,Collecting stool specimens from ill passengers and crew for submission to the CDC lab. Samples tested with the vessel’s onboard rapid norovirus test were positive for norovirus. The specimens will be sent to the CDC lab for confirmatory analysis,Making multiple daily reports of gastrointestinal illness cases to the VSP,Consulting with CDC on plans for: Passenger notification procedures and the planned delayed embarkation schedule in Houston, TX on February 1, 2014,Disembarkation plans for active cases, and terminal and transport infection control procedures.Two CDC Vessel Sanitation Program environmental health officers will board the ship in Houston, TX on January 31 and February 1, 2014 to conduct an epidemiologic investigation, environmental health assessment, and evaluate the outbreak and response activities. Specimens are being collected and will be sent to the CDC lab for testing.

Personnel Casualties : 1 injury

While exiting his bathroom in his cabin the victim lost his balance and fell forward towards his right side, landed on his right knee and his head on the bulkhead. Then his whole body made contact with the deck and he twisted his right ankle and skinned his right knee. The victim said he has physical limitations, that his left leg is weak due to a stroke that happened in the past. Caribbean Princess was anchored at Princess Cays, Bahamas. The victim fractured his right distal fibula and was treated by the doctor on the vessel.

On September 30, 2013 the passenger vessel CARIBBEAN PRINCESS was underway in the Atlantic Ocean approaching their destination port of Boston when a passenger broke her arm. The passenger fell in her stateroom and the ship's doctor diagnosed her with a fractured left arm. Upon arrival at Black Falcon Terminal in Boston, local EMS transported the passenger to Tufts Medical Center.

CARIBBEAN PRINCESS (IMO 9215490): A male passenger lost his balance and fell while standing in the passageway outside of his cabin. Initially he declined medical attention but afterwards complained of hip pain. He was taken to the ship's medical center and an x-ray revealed a fractured right femur. He was medically disembarked at the next port of call in the Bahamas.

On 12/21/12, Sector Miami was notified via CG-2692 (faxed) that the CARIBBEAN PRINCESS had experienced a failure of its starboard propulsion electric motor (engine). This failure was apparently due to a faulty starboard PEM aft converter water conductivity meter. The conductivity meter was replaced with a ship's spare and tested. Lloyd's Surveyor attested to the proper operation of the starboard propulsion electric motor.

On 18 Dec 2012, Sector Miami received a faxed CG-2692 from the foreign flagged cruise ship CARIBBEAN PRINCESS notifying of a loss of propulsion on their port propulsion electric motor (engine). The ship's crew found the cause of the failure to be a faulty air winding temperature probe. Ship's crew performed a temporary fix. The generator was restored to operation within the hour. Upon arrival to port, a Classification society surveyor attended the vessel and issued an interim certificate. That same day a Certificate was issued attesting to the satisfactory operation of the port engine.

On 12 March 2012, received notification from Master onboard P/V CARIBBEAN PRINCESS that port main diesel engine suffered a lost propulsion while enroute from San Juan Harbor into Saint Maarten. After a thorough assessment by vessel's class surveyor (Lloyd's Register), it was determined that the loss of propulsion was due to a faulty ground on the port power entry module supply.

On 25 February 2012, received notification from Master onboard P/V CARIBBEAN PRINCESS that port main diesel engine suffered equipment failure while enroute from Barbados to San Juan Harbor. After a thorough assessment, it was determined that the cause of the failure was due to a steam water leak that reached the port temperature regulation valve. Subsequently, the port propulsion electric motor registered a high temperature and shutdown automatically.

Subsequently, the fall caused the passenger's left ankle to suffer an open fracture on the tibia/fibula region.

The individual, who uses a walker and cane for mobility. Passenger had come in from the open deck to use a public rest room. The door was locked and the passenger turned away and rested and the bulkhead with his walker out in front of him when he lost balance and slid down the bulkhead and fractured his leg and wrist.

On 27 March, 2011, received notification from Master onboard M/V CARIBBEAN PRINCESS that U.S. Passenger fell, breaking her hip. Injured passenger was walking into a designated pool area when her right foot got caught in a ramp and fell onto her left side into the deck, causing the injury. Passenger was wearing prescription bifocal lenses and open-toed sandals with rubber thread. The area of the incident was determined to free of defects and with good lighting. Injured passenger was transported to local EMS ashore during port call in San Juan PR. After a thorough assessment of the incident, it was determined that the injury was caused by the passenger's lack of familiarization with the vessel's surroundings, specifically the location of the ramp.

On February 13, 2011 received notification from Senior 1st Officer onboard P/V CARIBBEAN PRINCESS that passenger suffered on left shoulder and left elbow injuries as a result of a fall. Accident occurred on deck 15 aft, starboard side Horizon Court (entrance forward buffet area). Passenger tripped on the base of the sign, fell forward on the left hand side. Passenger was wearing prescription glasses and branded sneakers with good traction. The area of the incident was inspected for any defects and none were found; visibility was determined not to be an issue.

On October 15, 2010 while underway transiting to Brooklyn, NY, a passenger on the passenger vessel CARIBBEAN PRINCESS fell and fractured her humerus on her left arm. Passenger was ambulatory but with significant pain. Passenger was evaluated by ship's crew and based on their assessment she was not medically evacuated that night. The vessel was scheduled to moor the following morning in Brooklyn, NY. Injured passenger disembarked in Brooklyn, NY without further incident and returned home for further medical treatment.

At 1645Q, on 31 Aug 2010, Caribbean Princess experienced an injury aboard their ship by a passenger who fell down a step. 01SEP10: MSD Belfast was notified via phone call and fax from Sector Northern New England. (The casualty was originally reported to Sector Boston.) The individual was walking to a small seating area when she fell down on her left side fracturing the head of her humorous and receiving abrasions on her wrist and left knee. The individual stated that she did not see the step. There was no drug or alcohol testing required due to crew members not being directly involved in the accident. The investigation was downgraded from an informal investigation to a data collection due to the Deepwater Horizon Response.

On August 8, 2010, the M/V CARIBBEAN PRINCESS was underway between Newport, RI and Boston, MA, when the #4 diesel generator/alternator failed due to a voltage problem in a terminal connection box. A network protection system operated as designed and tripped the generator circuit breaker thus shutting down the propulsion systems. Once the problem was isolated and contained, propulsion was quickly restored (within 10 minutes) and vessel passage resumed. Remaining five generators remained online during the incident and no loss of lighting was experienced in passenger or crew spaces. Onboard technical/engineering department investigated further to determine cause and extent of damage.

Received report from Senior 1st Officer onboard P/V CARIBBEAN PRINCESS that crewmember suffered first & second degree burns during the cleaning of the Lido Galley. Crewmember stated that he was lifting one of a number of oven racks that had been soaking in a water boiler. Several became caught on the rack and then fell back into the still hot water, resulting in water splashing up and over th edge of the boiler. The same landed in the gap between his PPE and clothing.

Recieved notification that passenger onboard P/V CARIBBEAN PRINCESS suffered injury on left ankle. Passenger was walking with two friends on deck 15 (Lido Deck) portside from forward to aft, in the vicinity of the Neptune Pool when she slipped, fell backwards on the deck and fracture her left ankle. Passenger was wearing rubber flip flops at the time of the incident. Passenger states that there was a "caution" sign on the other side of the incident site and saw it after the injury occurred.

Medical Officer onboard subject vessel stated that passenger was admitted to the Ships Medical Center in critical condition. Despite all critical care monitoring and emergency treatment, passenger deceased at 1430 with pulse less electrical activity. The Medical Officer ruled the loss of life incident as multi organ failure.
