Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Galley-Deck 6 Main Galley - Garde Manger

The previously cleaned counter model slicer had a visible food debris wedged below and protruding out of the sealant along the lower seam of the backplate, where food contacts. The sealant at that position was removed to reveal an accumulation of debris under the sealant. Staff removed it from service to clean again.

In the starboard side, forward ice machines #1 and #3 the interior surfaces of the metal upper level cuber panels were not smooth and the metal appeared pitted. In the #1 machine (most forward) there was also a pink to purple color on the surface of the upper metal cuber panel. A brand new cuber panel was brought in to compare the metal surface, and staff stated the change evident in the used panels was typical.

The insulation on the coolant piping which was under the different refrigerators below the long preparation counter was torn and damaged to a point that it could not be cleaned. Staff repaired the torn insulation during the inspection.

Nearly all the loaf pans set on sheet pans and stacked on the soiled racks had loose and peeling non-stick surface material, particularly so along the entire perimeter of the top interior edge of each loaf pan. In some cases it was evident that lower interior surfaces were also peeling.

The deckhead mounted round AC supply vent cover was dripping water directly onto previously cleaned plate covers which were stacked and air-drying on the counter below. All the contaminated covers were immediately removed for re-washing.

There was very good documentation of the asymptomatic cabin mate or immediate contacts of the crew gastrointestinal (GI) illness cases in the medical logs. The form was left blank if there was no cabin mate or no other immediate contacts for the crew GI cases.

Children Area-Child Activity Center - Toilet Rooms

The sensor type faucets for the two sinks in the children's toilet room provided only about 3-5 seconds of water when activated, and after that period they could not be activated again for several seconds, with only another 3-5 seconds of running water.

Potable Water-Cross-Connection Control Logs

The electronic log of inspection for the non-testable backflow preventers indicated the date and that inspection was carried out, but there was no result written unless there was some form of failure or repair made to a device.

The deckhead panels over the front bar area were very rough and difficult to clean due to excessive paint coating onto each of the square panels. It appears the metal underlayer may have corroded and the paint was meant to control that deterioration.

There was one live fruit fly in the bar pantry which was destroyed immediately and one on the deckhead in the front bar.

The serving utensil for the hard boiled eggs was touching the food and submerged in the water used to keep the eggs warm. The dish of eggs was removed and a new dish of hard boiled eggs was placed on the buffet line. The serving utensil was not replaced at the time the dish was put on the service line and a passenger was observed using their bare hands to pick up a hard boiled egg. A serving utensil was immediately provided for the hard boiled eggs.

Dead fruit flies were observed in the shieldings which covered the large circular light fixtures. One of the shields was removed to verfiy there were dead fruit flies, Three dead fruit flies were noted on the starboard middle light shielding.

The drain for the dishwasher dirty water was clogged. Used dishwater had accumulated in the drain area, and the drain line was completely submerged in the dishwater resulting in no air break.

Two waiter stations provided less than 220 lux of artificial light. Management was aware of the lighting issue and had assessed the light intensity throughout the ship. Staff showed the inspector the purchase order form for new lighting.

Management was aware of the issue and stated there was a leaking pipe. Staff reported they were in the process of addressing numerous water leaks on the ship. Staff explained this preparation table was not used due to the water leak.

Excessive condensation was observed on the deckhead. Management was aware of the issue and a repair team was on board and in the process of addressing this issue.

The posted time control plan states, 'food will be discarded within 4 hours and at the end of service.' The service period for lunch is longer than four hours.

The discard labels on the buffet line were above the hot food items. Staff explained the food containers were not labeled because the items were stored on the bain marie and were worried this would may be a fire safety issue.

This was a sprinkler valve room that was not properly constructed to store food or food service articles. The deckhead was open exposing sprinkler piping and the bulkheads and decks were not smooth, durable, and easily cleanable.

The time control plan for port days indicated a discard time of 10:30 AM for each side of the buffet. However, all the food was labeled for discard at 11:30 AM. The inspection took place on a port day.

The underside of the left counter mounted juice machine was soiled near the dispensing nozzles. This soil was black and appeared to have been there longer than from that day's service.

Three separate technical hatches were leaking in this area. A deckhead seam in front of the crushed ice machine was also leaking. These leaks were noticed by staff during the breakfast service and all work in the area was stopped. Maintenance was notified of the leaks at this time. When the inspector arrived in this area shortly after noon the leaks were still active and galley staff were mopping the floor were the leaks were dripping onto the deck. Maintenance then arrived to investigate the issue and it was determined a hot water pipe in the deckhead was leaking which caused water to accumulate in the deckhead and drip from the technical hatches.

A box of single-use plastic spoons and several boxes of food handing gloves were stored in this locker with cleaning equipment and supplies.

The light intensity was less than 110 lux behind and to the left of the ice machine.

The backflow prevention device for the pre-wash spray hose was leaking at the intermediate vent. The device was replaced during the inspection.

Preparation Room-

The water temperature at both handwashing stations was measured at 90°F. The temperature was raised during the inspection.

Bar-Calypso

The front panel of the orange juicing machine was cracked in several places. It was not in use during the inspection.

These suction fittings were dual or multiple fittings greater than 1 meter apart, but not all of these fittings had antientrapment drain covers.

The safety signs for the RWFs did not meet all of the requirements of the 2011 VSP Operations Manual.

Integrated Pest Management-

The combined logs for active and passive monitoring instructed the user to not record zero findings on the log. Because only locations where pests were found were recorded on the logs, it was not known what other areas were inspected and when they were inspected. In addition, the IPM plan did not provide guidance on the times for inspections, including some at night or during periods of minimal activity. This was written on the previous inspection.

Integrated Pest Management-Loading Inspections

The logs for loading only indicated that food and beverage deliveries were inspected. Other nonfood items, such as other hotel stores and technical stores, did not have inspection records. According to the staff, these deliveries were inspected.

The new plan also did not provide guidance on the times for inspections, including some at night or during periods of minimal activity. There was also no passive device monitoring records.

On January 19, 2014, at approximately 1645 local time, the 949 foot long, Bermuda-flagged cruise ship GRAND PRINCESS was docked at pier 40 in San Francisco, California, when a passenger operating an electric scooter struck another passenger with her scooter. The passenger that was struck fell as a result of being struck by the scooter. The scooter operator proceeded to run over the fallen passengers right leg and hip area which resulted in a fracture of the fallen passenger's left hip. The unidentified scooter operator did not stop; she kept on going (hit-and-run) and was subsequently never positively identified. A determination was made by the Sector San Francisco Investigations Division Chief that there would be little or no value gained from conducting an Informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

Propulsion was restored within seven minutes; however, the casualty was not reported to the San Francisco Vessel Traffic Service or to Coast Guard Sector San Francisco as required by regulation.

On January 1, 2014, at approximately 1800 local time, a passenger onboard the 949 foot, Bermuda-flagged, cruise ship GRAND PRINCESS fell down the aft passenger stairwell on deck 11 and fractured a bone in her left leg. The vessel was en route San Francisco, California, from Lahaina, Hawaii, when the incident occurred. A determination was made by the Investigations Division Chief that there would be no value gained from conducting an Informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

On November 26, 2013 Marine Safety Team Maui received a report from M/V GRAND PRINCESS, a Bermuda Flag Administration cruise ship, that the vessel suffered a partial loss of propulsion while en route to Lahaina, Maui, HI, from San Francisco, CA. The partial loss lasted for approximately forty minutes before crew were able to resolve the mechanical issue. The vessel continued on its scheduled voyage. The incident occurred in international waters, approximately 577 NM southwest of San Francisco, CA. This investigation was downgraded to data collection in accordance with the Marine Safety Manual, Volume V, section B.7.A.2.b (foreign flag vessel not on U.S. navigable waters).

The cruise ship GRAND PRINCESS reported that a female passenger (U.S. Citizen) had fallen overboard approximately 656 nautical miles NE of Hawaii. The GRAND PRINCESS deployed multiple life rings and has altered course to search for the passenger but was unable to locate her. In accordance with MSM Volume V, Chapter 7 A.2.b the investigation has been downgraded to data collection only.

The passenger sustained a fractured right hip and was transported and treated in the vessel's infirmary. The passenger was later transported to a local hospital for further treatment and released.

S. citizen passenger died from natural causes. As per Chapter 5 of the Coast Guard Marine Safety Manual, Volume V, this investigation was downgraded from the Informal to Data Collection level.

While moored in Aruba, the Grand Princess had a routine underwater inspection conducted. The #2 Stern Thruster had a line stuck in it but the sealing was intact. The #3 Stern Thruster was missing numerous securing bolts that secure the struts to the axel. The repairs will be conducted in Port Everglades on March 31, 2012. The vessel will use tugs to assist in maneuvering. 31MAR 2012: Repairs to the Stern Thrusters completed by Subsea Solutions (Miami Divers), at Port Everglades. Foreign flagged vessel, condition of vessel found outside of U.S. navigable waters. Because the vessel is coming into U.S. waters with a reduced ability to maneuver a Data Collection level investigation is being conducted.

A female crewmember (SPA Receptionist) fractured the ulnar styloid of the left wrist. The crewmember had just awakened and was getting out of the top bunk in her cabin, she stepped onto a chair which in turn moved, causing her to fall to the deck. There were no alcohol/drugs involved and this incident was not a result of vessel operations.
