Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Buffet-Horizon Court Starboard Line

One of the heat lamp bulbs at the carving station had started to peel slightly. The bulb was removed. The buffet line was open, but the carving station was not in use.

The wash tank for the rack conveyor dishwash machine was measured at 147°F by the inspector. The dataplate indicated that the temperature of the wash must be 160°F.

Food Service General-Dishwash/Glasswash Machines

Several machines were out of order. The deck 5 main galley starboard conveyor dishwash machine had been out of order for less than one day. The deck 5 main galley starboard hood-type glasswash machine had been out of order since 25 October. The deck 5 main galley port hood-type dishwash machine had been out of order for three days, the deck 6 main galley potwash machine had gone out of order on the day of the inspection. The deck 6 main galley starboard hood-type glasswash machine had been out of order since 23 October. All of the machines were waiting for spare parts that had been ordered and the vessel was managing the warewashing by using other areas with working machines.

Medical-Medical Records

The acute gastroenteritis (AGE) surveillance log automatically determined if a patient was a reportable case and staff reported they were unable to override this entry in the log. On the 13-20 October voyage, a passenger experienced one episode of diarrhea and was dispensed anti-diarrheal medication. When the inspector asked the staff why this case was reportable, the medical records were not available to review. Staff explained the medical records were at the corporate office for privacy reasons and the AGE log system automatically determined the patient as a case if dispensed diarrheal medication.

Medical-4-hour report

The 4-hour report was incorrect for the 7-18 October voyage. The 4-hour stated 3 reportable passenger cases and the AGE log stated 2 reportable cases.

Medical-

An Assistant Cook signed onto the ship on 20 May in Juneau, Alaska. He documented on his 'Health Declaration for Joining Crew Members' form that he experienced acute gastroenteritis symptoms beginning 19 May at 12:00. The Declaration form was collected by the ship's Crew Office who overlooked the positive symptom response. The Assistant Cook continued with his new-hire in-processing and was not seen by medical staff until 21 May at 19:00. At that time he was placed into medical isolation. He did not experience additional symptoms and was released from isolation on 22 May at 08:30.

Other-Crew Office Staff

Members of the Crew Office overlooked a positive response to an in-processing crew member's acute gastroenteritis symptoms. On 20 May an Assistant Cook signed onto the ship. The crew member documented on his 'Health Declaration for Joining Crew Members' form that he experienced acute gastroenteritis symptoms beginning 19 May at 12:00. The Declaration form was collected by the ship's Crew Office who overlooked the positive symptom response. Members of the Crew Office received a written warning for the importance of immediately reporting positive acute gastroenteritis responses to the medical department.

Recreational Water Facilities-Lotus Pool

Between December 2013 and April 2014, a review of the halogen analyzer-chart recorder charts indicated numerous days where the Lotus Pool was chlorinated at less than 2.0 ppm for many hours to the entire day while the facility was in operation. On 2 May a notice was sent by the corporate public health department which classified this facility as an activity pool. Since that date all charts indicated appropriate chlorine levels. When turned on, the Lotus Pool produces a current that the user can swim against. The breaking of the water surface classifies the pool as an interactive/activity pool.

Integrated Pest Management-Food and Beverage Areas

When pests were found during the initial and follow-up active inspections of the food and beverage areas, the action taken was '565 sprayed at end of service. Drains cleaned with Bio Gel.' Some of the cases were still ongoing for 2-3 months.

At the forward pastry counter, the three-bucket system sanitizing solution was heavily soiled with oil and food debris floating in the water.

Room Service-Deck 7 - Bell Box

Old food soil was in the crevice between the back panel and the upper inside shelf on all four of the room service trolleys. This back panel could be removed for cleaning.

Galley-Bayou Cafe / Sabatini's Restaurant - Warewash Area

The final rinse temperature of the glasswash machine did not exceed 151°F in four trials when measured at the utensil surface. This was verified with direct measurement with the inspector's thermocouple. All dishes for this outlet were washed in this machine and it was in active use during the inspection. The adjacent dishwash machine was out of service awaiting manifold repair parts.

Preparation Room-

A rack of clean utensils was immediately adjacent and at the same height of an empty mop bucket that was almost touching it.

Galley-Deck 5 - Soup / Sauce Station

A deckhead leak was over the walking area and down a support column across from the soup kettles. Water was dripping from the seam and from the deckhead-mounted speaker. No food was impacted. Repairs were started immediately.

The final rinse temperature spray was less than 160°F on five trials when measured at the utensil surface. On the crew member's maximum registering thermometers the temperature was 150°-153°F on two separate trials. The manifold temperature gauge read a constant 221°F, but the upper manifold was less than 140°F when checked with the inspector's thermocouple at two of the far end nozzles. Repairs were started immediately.

Medical-Acute Gastrroenteritis (AGE) Reporting

The ship sent four AGE reports to VSP with incorrect information while having the correct information on the AGE surveillance logs. For instance, (1) on 22 October, a report was sent with a total count of passengers of 1756 but the actual number was 1754, (2) on 12 October, a report was sent with a total count of passengers of 1682 but the actual number was 1862, (3) on 12 September, a report was sent with zero passenger reportable cases and three crew reportable cases but the correct numbers for reportable cases were two for passengers and one for crew, (4) on 16 August, a special report was sent instead of a 24-hour report with zero passenger and crew reportable cases but the correct number of passenger reportable cases was one. In addition, the ship sent special reports on 12 October, 12 September, and 16 August, but the cumulative percentage of reportable cases entered in the AGE surveillance log did not reach 2% among passengers or 2% among crew.

Medical-Crew Acute Gastroenteritis (AGE) Reporting

The review of the AGE logs revealed that there were crew members who experienced AGE symptoms and did not report in a timely manner to the medical center. These cases included one non-food employee that continued working after his symptoms began and another non-food employee continued visiting public areas after her symptoms began. Specifically, (1) a technical worker had an onset of AGE symptoms on 3 October at 05:30, worked his morning shift, and reported to the medical center at 15:00 that day; (2) a waiter had an onset of AGE symptoms on 8 October at 03:30, did not work, and reported to the medical center at 10:38 that day; (3) a junior assistant cruise director had an onset of AGE symptoms on 12 October at 17:00, did not work, and reported to the medical center at 19:15 that day after having dinner at the officer's mess while symptomatic. These employees received written warnings.

The ship sent four AGE reports to VSP with incorrect information while having the correct information on the AGE surveillance logs. In addition, the ship sent special reports on three different times but the cumulative percentage of reportable cases entered in the AGE surveillance log did not reach 2% among passengers or 2% among crew. The ship's electronic quality management system available to the staff had instructions, consistent with the 2011 VSP Operations Manual, on when to send 24-hour reports, 4-hour update reports, and special reports, but these instructions were not always followed.

Potable Water-Medical Center

The reduced pressure assemblies for the sluice did not have shut-off valves on each side of the devices. Test results showing the pressure differences on both sides of the valves were kept by the staff but tests could not be performed without shut-off valves on both ends of the assemblies according to the technical documentation. It was unclear how the tests were performed.

Children Area-Kid's Toilet & Diaper Changing Area

There was no airtight, washable waste receptacle to dispose of excrement, soiled sanitary wipes, and soiled gloves.

In addition, one of the glass deckhead panels looked cracked and the extent of the damage could not be determined.

Two tea cups were stored on the clean storage racks with their lids on. When inspected these tea cups had standing water inside indicating they had not be properly inverted and air dried. These two tea cups were immediately removed to be re-washed.

Three separate water leaks were noted in the area. A water leak was coming from the mechanical potwash machine, a second leak was coming from the water pipes leading to the sanitizing compartment of the three compartment sink, and a third leak was coming from the water pipes leading to the wash compartment of the three compartment sink. These three leaks caused a significant amount of water to accumulate and pool on the deck in the area.

Provisions-Fruit and Vegetable Walk-In 4212

A small leak was noted in the deckhead seam adjacent to the technical hatch in the center of the room. Water was present in the seam and several water drops were formed on the deckhead but there was no evidence the water was dripping on the food or deck below. It could not be determined at the time if there was a plumbing or condensation issue.

The time control plans for three separate areas (Bell Box, Garde Manger Deck 6, and Sabatini) listed refrigeration units that were not constantly under time control. For example, fridge #092424 and #092425 in the bell box were listed on the plan as under time control from 6:30 AM to 10:30 AM. The physical units were labeled as being under time control.

Buffet-Deck 14 - Horizon Court - Pastry Counter

Five bowls were found in the clean stack of tableware being dispensed to passengers that had dried food residue on the front or back of the bowl. The soiled bowls were removed immediately for re-washing.

Galley-Deck 14 - Horizon Court - Beverage Pantry

A new bulk milk carton had been placed in the bulk milk dispenser #T631 without the dispensing tube being trimmed to 1-inch. This was corrected immediately by trimming the tube at a 45° angle to less than 1-inch.

Galley-Deck 6 - Hot Galley

The current time control plan specified the set up and discard time for the food items being held under the plan to be 1700 - 2230, respectively. The plan did not describe how the food time in the danger zone would be controlled, i.e. labeled batches, since the service period was more than four hours. A new time control plan for this area is under development.

Galley-Deck 6 - Dishwashing

Heavy condensate was dripping from the bulkhead and deckhead from the soiled landing area to the mid-section of the dishwasher. A section of the canopy had been cut out to allow the control panel door to be open and steam was escaping, condensing and dripping from the deckhead in this area to the deck where personnel were walking from the dirty to the clean areas. Moist air was also escaping at the dirty entrance to the machine. The mid-machine exhaust also had a positive pressure at an opening that allowed moist air to escape the exhaust system.

Galley-Deck 5 - Cold Pantry

A small amount of dried food soil was found on the recessed area of the backplate of a food slicer.

Bar-Deck 6 - International Cafe

The air relief vent on the left carbonator backflow preventer in cabinet #065202-T651 was closed with a screw-type plug that was sealed in place with joint tape. The backflow preventer on water supply line to the carbonator system was also continually leaking from the air relief vent indicating a possible failure. Staff immediately replaced these devices.

Provisions-Deck 4 - Cold Rooms

The forward fruit and vegetable walk-in cooler had mildew on the evaporator unit fins closest to the door. A heavy buildup of ice was noted on the top section of the other units' evaporator fins and also on units #4211, #4209, #4212 and #4201.

A water leak from an unknown source was noted along about 8 feet of the mid cooler deckhead seam in walk-in cooler #4209.

The leak described above in walk-in cooler #4209 exposed the top layer of a pallet of honey dew melons in open cartons to potential contamination. The staff immediately moved the pallets and washed the affected melons.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

On August 8, 2013, a crew member had two episodes of diarrhea, abdominal cramps, headache, and muscle cramps, but was not recorded as a reportable case on the AGE log, but upon discussion with staff, this person should have been a reportable case. The pre-programmed AGE log cannot be altered or over-ridden by staff once it is decided whether an individual is a case or not. Even if the underlying illness column states a reason for an individual to not be reportable, this information is not taken into account when the computer program is deciding whether an individual is a case or not.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

During the cruise from July 31 to August 7, 2013, a crew member with AGE symptoms was counted as a reportable case even though they were already counted as a reportable case on the previous cruise. This individual had an illness onset on July 30 with three episodes of diarrhea. The underlying illness column stated this individual was a carryover case, but this information was not factored into the computer program.

Medical-Crew Reporting

On August 10 a food worker had an illness onset of two episodes of diarrhea, two episodes of vomiting, and abdominal cramps at 2200 and did not report to medical until August 11 at 1730. According to their work record, this individual worked on August 10 from 1800-2400 and on August 11 from 1100 to 1600. Also, on August 10 a food worker had an illness onset of two episodes of diarrhea, two episodes of vomiting, and abdominal cramps at 2200 and did not report to medical until August 11 at 1730. According to their work record, this individual worked on August 10 from 1800-2400 and on August 11 from 1100 to 1600.

Medical-Crew Reporting

On August 8, 2013 a food worker had an illness onset of two episodes of diarrhea and abdominal cramps at 0700 and was a reportable case. According to their work record, this individual worked on August 8 from 0730 until 1130, but did not report to medical until 1300 the same day. This was noted on the previous inspection.

Medical-Acute Gastroenteritis (AGE) Log

A passenger with an illness onset on August 3 at 2200 was entered into the AGE log. Several columns of the AGE log were left blank including the passenger meal seating and any symptom information for episodes of diarrhea, vomiting, abdominal cramps, and headache. The reportable case column was also left blank and there was not enough information about this case to determine if it was reportable or not. There were no passengers documented as reportable on the MIDRS confirmation report during the cruise from July 31 - August 7, 2013.

Medical-Acute Gastronenteritis (AGE) Log

The AGE log stated there were six reportable cases, but the total reportable cases in the medical staff documentation indicated there were five reportable cases. It was unknown which of these numbers were correct because there was no documentation confirming the 24-hour MIDRS report was sent.

The underlying illness column for the last five cruises included information such as whether passengers and/or crew members are on shore, drank on shore, took shore tours, the location of the shore tour, and also included general comments. This made it very confusing to determine if the information in this column was supposed to be used to indicated whether a case was reportable or not. For example, a passenger had four episodes of diarrhea and no other symptoms, but the underlying illness column stated they were positive for influenza with no other documentation. According to staff, this individual has both AGE and Influenza A, but this could not be determined by the documentation.

Potable Water-Backflow Prevention

There was not a proper air gap between the pressure relief vent and the drain for the vent on the reduced pressure assembly on the potable water line to the HI-FOG system.

The six potable water hoses in the hose storage locker did not have readable labels at each connecting end stating 'Potable Water Only'.

Potable Water-Starboard Bunker Station

The reduced pressure assembly for the international shore connection had a tag indicating 'disconnected' on it and the fire protection line above the device had caution tape around it and a plastic bag full of tools. According to the staff, the device was working properly and the tag, tape, and bag were removed.

The backflow prevention device on the shower hose was not on the cross-connection control log.

Potable Water-Mineralizers

There was no air gap between the combined drain line and the drain.

Recreational Water Facilities-Sand and Cartridge Filters

The sand was replaced for Spas D and E on March 3 and 23, 2013 respectively, but there were no records indicating the filter housing was cleaned, rinsed, and disinfected. Also, documentation indicated the cartridge filter had been changed several times for the Lotus pool, but there was no documentation indicating the housing was cleaned, rinsed, and disinfected.

Recreational Water Facilities-Sand Filters

According to staff, monthly inspections and samples were done and collected for all of the spas and pools that had sand filters, but there was no documentation. Staff had a box of the samples collected for August 2013, but no record of the results. According to staff, samples are kept for one month and then discarded.

Recreational Water Facilities-Hair and Lint Strainers

There was no documentation indicating the hair and lint strainers or their housings were cleaned, rinsed, and disinfected on a weekly basis for any of the RWFs.

Housekeeping-Outbreak Prevention and Response Plan (OPRP)

During red level sanitation when at least 2% of passengers or crew had AGE, the OPRP stated that public areas should be cleaned both continuously and then only frequently in the same section. The language was confusing and even the staff could not determine how often these areas should be cleaned.

There were no procedures to notify crew of an outbreak while onboard or how to notify passengers of a cruise that had a previous outbreak on the last voyage. Also, there were no procedures stating how to protect passengers from exposure to chemicals and disinfectants used onboard.

There were two food workers that worked while sick and did not report to medical immediately. For example, on August 8, 2013 a food worker had an illness onset of two episodes of diarrhea and abdominal cramps at 0700 and was a reportable case. According to their work record, this individual worked on August 8 from 0730 until 1130, but did not report to medical until 1300 the same day.

After the inspector discussed the issue with the staff, the curtains were removed and properly cleaned and sanitized before the machine was put back into service.

The outbreak prevention and response plan did not include procedures for informing current and embarking passengers and crew members of an outbreak or procedures for returning the vessel to normal operating conditions after an outbreak. In addition, there was no information on the required contact time for chlorine solutions to be used in food areas.

The time control plan did not provide detailed instructions on how the operations were actually conducted using alternating soft serve ice cream machines. Each of the two ice cream machines was set-up, operated for four hours, emptied, cleaned and sanitized in four hour blocks of time. The cycle began with refill from the temperature-control back-up supply. Labeling or other specificity on where each of the machines was in its cycle was not included in the time control plan.

There were four bags of cut fresh spinach stacked in a hotel pan and wrapped with film in walk-in refrigerator #5201. The spinach was at a temperature of 48 °F. It was immediately removed and ice applied to cool.

Two sheet pans filled with egg salad on croissant sandwiches had a food temperature of 52-53 °F in walk-in refrigerator #5218. Staff stated that the food was prepared earlier in the morning (approximately 40 minutes)and would be served at lunch. There was no cooling log entry for these made sandwiches and they were overwrapped with food film. The sandwiches were uncovered immediately and entered into the cooling log.

Galley-Deck 6 Garde' Manger

A bowl of egg salad was found inside undercounter reach-in refrigerator #052326OD with a food temperature of 44 °F. Staff stated it was used earlier to make sandwiches. The bowl was covered and not entered on the cooling log. Staff unwrapped the bowl and blast chilled the egg salad immediately. The egg salad sandwiches were held in the walk-in refrigerator at 52-53 °F after preparation some 40 minutes prior, according to staff, but were also not listed on the cooling log.

Galley-Deck 6 Garde' Manger

One of every 5 previously cleaned plates inspected at the aft bulkhead mounted plate storage section were found visibly soiled with food residue. The plates were immediately removed for cleaning and all plates in this area reinspected.

0999: The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. There are several heating pipes due to pinhole leaks, and soft patches throughout the engine room. 74 SOLAS (09) I/11; 50(a)(c)

Operations/Management

2025: If a ship is fitted with a marine evacuation system, drills shall include exercising of the procedures required for the deployment of such a system up to the point immediately proceeding actual deployment of the system. Personnel

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. The lifeboat crew for multiple lifeboats were lacking in duty familiarity.

Accommodation/Occupational Safety

Satisfactory means for safe passage... shall be provided for the protection of the crew in getting to and from thier quarters... and any other spacew sued in the essential operation of the ship. Crew members responsible for lifeboat operations, in stowage position, are put in an unsafe postition.

Fire Fighting

Exhaust ducts from galley ranges in which grease or fat is likely to accumulate shall suitably located for cleaning. AC station on deck 12 with Bayou Galley exhaust has substantial grease/ fat saturation.

Furniture in stairways enclosures shall be limited to seating. It shall be fixed, shall be of restricted fire risk, and shall not restrict the passenger escape route. Roll away table set ups and non fixed signs were staged in deck 14 midship stairs.

Code: 0610. At all times during the voyage all lifesaving appliances shall be in working order and available for immediate use. Lifeboat No. 2 fuel injection system was inoperable rendering the lifeboat not available for use. Parts unavailable. Vessel is restricted to carry 3000 total persons based on the reduced lifeboat capacity.

Code: 0710. Ignition sources shall be restricted. Several power strips and extension cords were found connected in series on Deck 6 Explorers Lounge backstage which may be a potential source of ignition.

Every candidate for a certificate of proficiency in fast rescue boats shall be the holder of a certificate of proficiency in survival craft and rescue boats, attended an approved training course, & meet the standard of competence for certs of proficiency in fast rescue boats. Boat captains for fast rescue boats do not meet the requirements for operation of fast rescue boats. (STCW 2011 VI/2.2)

For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are categorized according to their fire risk. During exam of electrician's workshop, flammable storage exceeded what is considered daily use. Excess flammable storage should be stored in a cat 14 space. (74SOLAS (01) II-2/26.2.2)

Accidents with life-saving appliances

One crewmember has died and another injured in an accident involving a rescue boat onboard passenger cruise ship CORAL PRINCESS in Colon, Panama on Friday, 24 October 2014. The rescue boat was being raised back onboard the ship, one of the cables that raises and lowers the boat parted, and the boat dropped back into the water with two crew members inside.The crewmembers were carrying out maintenance work on the hull inside of one of the rescue boats.

A male passenger was disembarking the vessel while moored at berth 2 in Port Everglades. He was descending the midship passenger stairs from deck 7 to deck 6. He misplaced his foot (skipped 2 steps) when stepping from the stairs to the landing on deck 6 and injured his right knee. He was carrying a suitcase in each hand and this prevented him from grabbing the handrails to possibly prevent the fall. The ship physician diagnosed the injury as a ruptured right knee tendon. The area of the accident was inspected and found to be clear of hazards. The passenger was taken to a local hospital for evaluation and treatment. He was cleared to fly and returned to his home in Maryland to seek further medical treatment with an Orthopedic Surgeon.

Engineer twisted his knee while performing maintenance on Aug 4th. He did not report any injury until Aug 13th. The engineer slipped while entering the crank case of the main engine for a visual inspection. The case is very tight quarters and was very slippery with oil. No chemical testing done due to period of time between occurrence and report.

Material Failure (Vessels)

On May 5, 2013, at approximately 2330 local time, the Cruise Ship CORAL PRINCESS experienced a loss of its bow and stern thrusters while transiting inbound the port of Los Angeles, CA. The vessel safely moored at the Los Angeles Cruise Ship Terminal with the assistance of 2 tugs. A technician from Siemens found a glitch on the Automation Power Management System resulting from the re-commissioning of a Diesel Generator. Repairs were made and the bow and stern thrusters were tested and found to be operating satisfactorily.

An elderly female passenger tripped and fell when she got out of her bed to answer a wake-up call on the telephone. After the fall, she could not move and notified her companion who found her laying on the floor in the dark. She was taken to the medical center and X-rays revealed a fracture to the neck of her right femor. She was immobilized, given analgesics, and taken by ambulance to a local hospital (Cleveland Clinic) for further treatment.

Injury on the Coral Princess Resulting Injury: Fracture distal radius - Right - Wrist splint applied. Cause: Patient stated she was in the Explorers Lounge line dancing on the dance floor, she lost her balance and fell backward. Another passenger attempted to catch her but she landed on her outstretched right hand. She was able to get back up and carry on dancing. The following day she felt a dull ache in the right wrist, but presented to medical center 2 days later as the pain had not subsided.

On 27 Mar 2012, Sector Miami received a CG-2692 from the cruise ship CORAL PRINCESS documenting a passenger injury. The Adult Female embarked the vessel and shortly after her arrival on board, she was walking through the sliding glass doors from the Open deck starboard side to the Lotus Pool area when she stumbled on a raised threshold and fell forward landing on her right side. She sustained injuries (fracture of right humerus) but remained on board the vessel. No drug or alcohol tests were conducted for the ship's crew as no action or inaction contributed to this non-maritime operation related casualty.

On December 20, 2011, a passenger on board the C/S CORAL PRINCESS fell resulting in fractured ribs while the vessel was moored in Long Beach, CA. The passenger was walking on Deck 14 toward the Horizon Court Buffet coming from the open deck. As he opened the door to the foyer area he lost his balance and fell. He felt discomfort in his rib cage that resulted from the fall but waited 2 days to report the injury to the vessel's Medical Center. The Ship's Senior Doctor determined that the passenger sustained a "Non-displaced left posterior fracture of ribs 6-8." The foyer area where the passenger fell was inspected and there is a slight step of approximately 1 inch high, which is the door threshold from the open deck to the indoor foyer. The threshold is marked with high visibility tape, was well lit and free from any hazards.

Alarm on the ships turbine oil system, causing the vessel master to anchor and complete troubleshooting. 19 August 2011: It was determined after troubleshooting that the turbine had small metal particles within the lube oil system indicating an internal failure. Turbine was secured and vessel switched to diesel electric (main propulsion) and continued voyage.
