Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The light intensity could not be raised to 220 lux at the waiter stations. The electricians showed the inspector these areas had previously been addressed in their light assessment.

Galley-Crown Grill

Coving was missing at some areas on top of the tiles of the equipment/deck juncture. Work was in progress to replace the deck tiles in this area.

The light intensity was less than 220 lux at the front preparation counter and at the pizza oven.

The crew member working in this area had a thick moustache that protruded past the corners of his mouth. He was not wearing a beard restraint.

The ship's records showed that the testable backflow preventers were last tested 54 weeks ago, resulting in them being two weeks overdue for their required testing.

Potable Water-Far Point Chart

The chart for the day of the inspection was mounted to show an incorrect time for the recording. At 15:30 local time, the chart pen was at 16:15 local time. The crew member responsible for the chart changing reported using current local time for the ship's water records.

The breakfast menu in the 'From the Sea Selections' had a Smoked Salmon, Bagel and Cream Cheese dish. The asterisk indicating that a dish could be ordered undercooked highlighted the cream cheese rather than the smoked salmon, which was the ingredient that was not fully cooked.

The time management plan being used for breakfast was not posted in the Deck 5 galley since breakfast was only usually prepared and served in Deck 6. The passenger unloading on the day of the inspection changed the normal meal preparation and service arrangements.

Galley-Potwash Area

Buffalo chopper lids were severely chipped, pitted and gouged creating difficult to clean surfaces in these pieces of equipment.

Galley-Garde Manger and Hot Galley

Food slicer backplate had dried food soil in the lower recessed area where the food discharges from the blade. This condition was also noted in the hot galley slicer.

Mildew was noted on the fins on the evaporator nearest the door. The evaporator at the back of the walk-in cooler was covered with ice which decreased the unit's cooling ability.

Water was collecting in two deckhead mounted light fixtures and was dripping to form an icicle in one of the cover screw holes. No food stored below was being impacted.

Food soil was noted on the deck inside the fire door channels.

Galley-Walk-In Cooler #5611

Three large plastic pans of chopped cabbage were found in the walk-in cooler at internal temperatures of 47°-57°F. These pans were recorded one time on the cooling log at 52°F 10 minutes before the inspectors entered the area. The way the pans were stacked together could have limited cooling.

Galley-Dishwashing

Heavy condensate was noted dripping from the canopy and deckhead above the soiled end of the dishwasher. No clean ware was being affected at the time of the inspection.

Buffet-Preparation Areas

The brackets with the rubber seals holding the glass panels on the top of the half walls that separate the preparation areas from the dining area and the island buffets to the brass glass supports were soiled and difficult to clean.

Galley-Hot Galley

The left tilting pan support next to the bulkhead had a difficult to clean seam where the sealant was coming out of the joint.

Bar-Promenade

The backflow preventer on the water supply line to the coffee machine was leaking from the air relief hole indicating a possible failure of the device.

Dining Room-Consumer Advisory

The dining room breakfast menu included eggs cooked to order and there was no consumer advisory in the menu or an asterisk noted at the egg dishes to indicate the advisory notice. Staff stated that the advisory change for menu's was on order and during the interim period consumer advisory cards were placed on every dining room table. The advisory was present on each table and did identify undercooked eggs as a possible risk.

Food Service General-Galleys - Undercounter Refrigerators

Several 2 and 3 door undercounter reach-in refrigerators had a warped condition on one or two of the doors where a gap was present at the lower door and the door frame when closed. The gap was large enough to pass a set of folded sheets of paper from outside to the interior of the refrigerator. This condition was less severe in some areas, but significant in the following: D5 main galley soup station - undercounter refrigerator #2 on the far left of three doors; pastry undercounter refrigerator #5 on doors 1 and 3, and #7 undercounter refrigerator on door 3; crew galley undercounter refrigerator #6 on the right of two doors; D6 main galley starboard/forward pastry service line on the right door of the undercounter refrigerator below the ice cream chest freezer; and hot galley soup station starboard on the left door of the #10 undercounter reach-in refrigerator.

Food Service General-Galleys and Provisions

Live fruit flies or drain flies were noted in several areas of the food operations, including the following: D5 main galley - bakery - one fruit fly on forward bulkhead behind the floor mixer and one drain fly on the internal surface of the bowl in the left mixer, two fruit flies on the deckhead over the flour bin counter; pastry - one drain fly on the canopy exhaust hood exterior beside the knife locker; crew galley - aft counter one fruit fly on the deckhead near the air supply diffuser; crew mess buffet - one fruit fly on the deckhead at the starboard/aft porthole window; D6 main galley - aft/port beverage station - one fruit fly on the deckhead beside the blast chiller; provisions vegetable preparation room - one fruit fly on the deckhead over the potato peeler; and sabbatini galley - one fruit fly on the deckhead near the walk-in refrierator door.

Galley-Deck 5 Main Galley - Garde' Manger

A canape appetizer was noted in refrigeration with cold smoked salmon present. Staff stated this was for tray service to guests as part of private parties, but there was no specific advisory provided to point out the health risks associated with consumption of this undercooked fish.

Galley-Deck 5 Main Galley - Garde' Manger

One deep stainless steel pan containing shredded cabbage was found inside walk-in refrigerator 5607 with an internal food temperature between 45 and 48 °F. Staff stated that it was cut and placed in the refrigerator last night, so all was discarded.

Galley-Crew Galley - Dishwash

During active use of the conveyor dishwash machine the wash tank temperature gauge indicated a wash water tempoerature of 155 °F and a manual measure of the water revealed a wash temperature of 156 °F. The manufacturer's data plate specified a wash temperature minimum of 160 °F.

Galley-Crew Galley - Pot Wash

During active use of the hood type pot wash machine there was a water leak from the corner of the canopy exhaust hood on the clean end. There was no impact on clean items exiting this machine.

Preparation Room-Butcher Shop

There was a water leak from a seam between deckhead panels, with water poolinng on the counter below and some spilling over to the door of the undercounter reach-in refrigerator. There was no food preparation at the time.

Galley-Sabbatini Galley

The artificial light level behind the coffee and espresso machine was below the minimum 110 lux.

Bar-Pool Bar

One large plastic container of heavy cream was set on the bar counter between uses. The cream container had a 7 day discard label affixed. The manager stated that this was not on time control and it was intended for storage under refrigeration between uses.

Other-Deck 14 Hamburger Grill

During active grilling there was visible smoke that entered the canopy exhaust hood and then drifted throughout the grill area, the adjacent bar and the pizzeria. Using a paper towel to check the hood there was no suction evident.

Pantry-Tradewinds Bar - Pantry

The door to the pantry was pinned open, providing easy access to flying pests. This bar and pantry are on an open deck.

Potable Water-Cross-Connection Control Log

The ship's non-testable backflow prevention device log was incomplete. The log was divided up into decks and the log for deck 1 and 15 listed the area where a backflow device was installed, but did not list the type of device nor the plumbing system component. Also, some of the logs did not list the date that the devices were periodically inspected.

Potable Water-Far Point

The free halogen residual measured by the halogen analyzer at the far point was not accurate to within 0.2 ppm. At the time of the far point analysis, the analyzer read a free halogen value of 1.77 ppm and readings taken by the inspector's electronic test kit measured 1.33 and 1.40 ppm.

Recreational Water Facilities-Whirlpool Secondary Antientrapment

Throughout the assessment of the ships whirlpool spa and other recreational water facilities (RWF;s) the staff responsible for monitoring and maintenance of these systems was not fully aware of the operatiions. There was a need for physical review in different areas more than once. A significant error involved the assessment of what was said to be suction fittings installed on the ships 11 whirlpool spas. During the inspection the ship's staff confirmed their presence, but following the inspection evidence provided by the corporate office confirmed they were in fact removed and no suction was present in any of those whirlpools.

Medical-Crew Member Reporting

A waiter who experienced 4 diarrhea episodes beginning at 0500 on 26 May did not report until 1615 later that day. Time sheet records indicated that the employee worked from 0600-0900 and from 1200-1500.

Integrated Pest Management-Garbage Room

Active surveillance records indicated that the garbage room was only inspected once in the month of May.

Integrated Pest Management-Pesticide/Safety

The IPM plan did not have a record of all pesticides currently onboard the vessel nor did the plan have health and safety procedures to protect the passengers and crew when applying the pesticides.

Beneath the preparation counter, one previously cleaned cutting board in storage was soiled with food debris. This cutting board was rewashed and sanitized.

Water was leaking from the deckhead onto boxes of lima beans. A plastic bin had been placed on top of the lima beans to collect the water.

The cooling logs indicated pasta was prepared on 5 February, however the food item had a 7-day discard date beginning on 6 February. Cooling times for the pasta were 16:09 at 158°F and 18:09 at 38°F on 5 February.

On the day of the inspection, the cooling logs indicated mixed vegetables began cooling at 08:45 with a product temperature of 158°F. At 11:15 there was no 2-hour cooling time written on the logs.

The light intensity was less than 220 lux on all areas of the preparation counter across from the flat grill and on all areas of the preparation counter above the fresh eggs undercounter cold holding unit. Staff replaced the light covers and the light intensity on the fresh eggs preparation counter was increased.

Inside undercounter refrigerator #8, a container of ground lamb had a discard label indicating a production date of 28 January and a use by date of 5 February, exceeding the 7-day discard requirement. Staff working in the area stated the food product was not prepared there.

Galley-Deck 5 - Potwash

Excess steam emanated from the potwash machine during the wash cycle. According to the gauges, temperatures and pressure for the wash and rinse cycles were in compliance with the manufacturer's specifications.

The light intensity was less than 110 lux between and behind the beverage equipment.

The light intensity was less than 220 lux on the two preparation counters.

The time control plans indicated potentially hazardous foods were to be discarded every 3.5 hours at 14:00, 17:30, 21:00, and 23:00. Foods, such as cheese, sliced lettuce, and sliced tomatoes, in the counter cold top were not labeled with their discard times.

Medical-Crew Member Reporting

A review of the acute gastroenteritis (AGE) logs for the last five cruises revealed there were three crew members who experienced AGE symptoms and continued to work after those symptoms began. Specifically, (1) an engine cadet had an onset of AGE symptoms on 31 January at 12:00, but did not report to the medical center until 3 February at 16:55; (2) an assistant buffet steward had an onset of AGE symptoms on 20 January at 09:30, but did not report to the medical center until 16:00 that day; (3) a spa massage therapist had an onset of AGE symptoms on 17 December at 10:00, but did not report to the medical center until 18 December at 09:00. A similar violation was written on the previous inspection.

Recreational Water Facilities-Pool in Spa Area

The free residual halogen measured by the staff's and inspector's test kits was consistently over 10 ppm on several checks. Staff closed the pool until the halogen level could be corrected.

The turnover rates and bather loads for all of the RWFs were calculated using the manufacturer's flow rate of the pumps and not the actual rates measured by flow meters. The turnover rates and bather loads were not calculated during the previous inspection.

Recreational Water Facilities-Analyzer-Chart Recorders Sample Lines

The sample lines for the analyzer-chart recorders for 6 out of the 9 RWFs were still in progress to be located after the compensation tanks. The staff estimated a target completion date of June 2013. During the previous inspection it was found that all RWF sample lines were after the compensation tanks.

Potable Water-Bunkering and Production

There was no record on the charts that the halogen and pH analyzer-chart recorders were calibrated at the beginning of bunkering or production or when potable water production was re-started after a period of technical water production. Also, the charts were not initialed.

On 23 January, the free residual halogen was not monitored for several hours when the analyzer-chart recorder malfunctioned.

The distillate water lines from the evaporators to the potable water chlorine injection were not striped or painted in accordance with ISO 14726 standards (blue/gray/blue). The lines were striped blue/brown/blue.

A rack of soiled carafe lids and a stack of several soiled plate covers were stored directly on the deck in front of the soiled drop-off counter.

There were potentially hazardous foods placed out for self-service on tables on a daily basis from 17:00 to 19:00 where time was used as a public health control, but there was no time control plan available in the area.

Condensate collected along the seams of the stainless steel deckhead panels and some deckhead mounted light fixtures of the walk-in vegetable cold holding box No. 4723 and was dripping onto the deck below. According to the staff, moisture had accumulated in the stainless steel deckhead panels and this would be mitigated during the next dry dock.

A green and black substance soiled numerous seams of the deckhead panels of the vegetable holding box No. 4723. In addition, the ventilation fan in the corner was soiled along the stainless steel guard.

The food and beverage manager directed the first cook to immediately report to medical, and not to return to work until cleared by the doctor.

The medical records indicated the steward reported to medical on 11 September 2011 at 17:00. It was unclear if the steward worked during the period of onset until reporting to medical.

Five pans of various types of fish in the walk-in refrigerator were measured at between 47 and 48°F. These fish had preparation dates of 19 May. These pans were located on the second shelf from the top. There were other bins of fish on the bottom two shelves, and these were all 41°F or below. The air temperature of the walk-in was measured at 48°F. The bulkhead mounted air cooling unit was on defrost and warm air was coming from the bottom of the unit. The cover on the cooling unit was removed and there was excessive ice build-up at the bottom of the unit, which prevented the unit from cooling properly. The ice was removed and the unit began to function properly. The five pans of fish were discarded.

Preparation Room-Fish

The air temperature of the walk-in refrigerator was measured at 48°F. The bulkhead mounted air cooling unit was on defrost and warm air was coming from the bottom of the unit. The cover on the cooling unit was removed and there was excessive ice build-up at the bottom of the unit, which prevented the unit from cooling properly. The ice was removed and the unit began to function properly.

Bar-Wheelhouse

There were two small black flies around the draft beer taps on the front counter. The bar was closed at the time of the inspection.

Medical-Storage Room

Boxes of breakfast cereal and tea were stored in this room with a mop and a mop bucket.

On September 22, 2014 at approximately 2340 hours a passenger on board the GOLDEN PRINCESS fractured her left ankle while playing table tennis on deck 15 zone 5 while underway off the coast of northern California. The ship was in moderate seas, the lighting was adequate, and it does not appear that drug or alcohol use was a contributing factor. This casualty was not related to ship operations and there was no crew involvement.

Sector Honolulu Command Center received call from 1st mate onboard the HPV Golden Princess reporting that a passenger was injured on April 1, 2014 while moored in Los Angeles. The injury occurred while conducting passenger drills, a 79 year old male was waiting at his muster station when he began to feel weak in the legs. Passenger turned around to take a seat and suddenly collapsed to the ground. On April 3, 2014 the injured party reported to the medical staff the incident that had happened on the first and complained of a sore ankle. Medical staff examined him and found through x-rays that he had suffered a non-displacement, fracture of the right ankle.

On October 28, 2013 a 57 year old woman onboard the MV GOLDEN PRINCESS slipped while descending aft passenger stairs. Passenger was treated on board at medical facility and found to have a fracture of the left ankle. Area was investigated by the crew and found to be well lit and free of defects. Handrails were available and fit for use. No direct crew involvement with incident.

Drug and alcohol testing were not conducted on the injured passenger. Do to the length of time it took to contact the injured passenger and the passenger's condition (heavily medicated) the 96 hour work rest history was not collected.

Material Failure (Vessels)

Coast Guard Marine Safety Team (MST) Maui investigated the loss of hydraulic pressure in the forward davit arm of the number 17 lifeboat tender aboard a passenger vessel under Bermuda flag Administration. While the vessel was recovering the number 17 lifeboat tender at the end of the day's passenger transfer operations, crewmembers noted a decrease in hydraulic pressure in the forward davit arm system after raising the lifeboat tender to the embarkation deck without passengers aboard. Crewmembers brought the lifeboat safely to its cradle with the use chain falls. MST Maui investigators boarded the GOLDEN PRINCESS at anchorage and confirmed that no personnel injuries resulted from the incident, nor damage to the lifeboat tender or systems other than the forward davit arm hydraulics. Tender could be deployed if necessary in an emergency but not recovered. Flag Administration granted the vessel permission to proceed on its scheduled voyage, being satisfied that the aggregate functioning survival craft systems aboard the vessel were greater than 125% of total number of passengers and crew aboard. MST Maui investigators concurred and notified the COTP, who granted permission for the vessel to depart U.S. waters.

Golden Princess reported an injury to a crewman that occurred the previous day while moored at Pier 10 in Honolulu Harbor. A Filipino male caught his hand between the side of the ship and a moving platform, de-gloving his right index finger. There was no fracture but the crewman was referred to an orthopedic surgeon in Honolulu. The ship has since sailed and is in port Nawiliwili, Kauai while the crewman remains in Honolulu

On February 6, 2012, a male passenger on the GOLDEN PRINCESS fell at the gangway and fractured his left wrist. The injured passenger went to medical on February 7, 2012 and an x-ray was taken. The x-ray results were obtained on the morning of February 8, 2012 with the diagnosis of a fracture of the left wrist. The ship’s crew inspected the gangway and noted it was in a good condition and dry.

On 12 October 2011, a passenger onboard the C/S GOLDEN PRINCESS tripped in his cabin fracturing his wrist. The passenger woke up to use the bathroom at approximately 0445 and walked to the bathroom in the dark. He tripped for an unknown reason, fell and used his right arm to break his fall. His right arm hit the lip on the shower which fractured his wrist. The passenger met with the doctor onboard the vessel and upon arrival into port, approximately 2 hours later, he was placed in a cab and sent to Little Company of Mary Medical Center in San Pedro, CA.

On May 8, 2011, Sector Los Angeles-Long Beach received notification of a deceased passenger onboard the C/S GOLDEN PRINCESS. The vessel was underway in the Pacific Ocean (International Waters). The passenger was in his cabin and was found lying on the floor and unresponsive by his wife. Another passenger was doing CPR for approximately one minute when the ship's medical team arrived and took over. They continued CPR (including chest compressions, intubation and ventilation, intravenous drugs) for approximately 21 minutes with negative results. The passenger was declared deceased at 1748 local time on May 8, 2011. The passenger had a history of medical conditions. According to the Los Angeles County Coroner's Report the cause of death was determined. Case closed.

On board medical team administered x-ray and applied 3 sutures to the laceration. On board medical team found it in the best interest of the crew member to keep him onboard to be monitored.

Grounding

Sector Honolulu Command Center (SCC) received a report that the Spirit of Lahaina passenger vessel had run hard aground as they were outbound in the channel from Lahaina, Maui. The report came to the SCC from CDR Neubauer, who receive dthe report from the Port Manager of Operations. The vessel had 60 passengers, all of which were ferrried of the vessel to shore by a good samaritan. No pollution and no flooding were reported post accident. SCC directed STA Maui to proceed to the scene to validate the location of the vessel, varify if there is any pollution, and verify that the local aids to navigation (ATON) are watching properly. The channel is lined with private aids which are not verified by CG ATON. A MSIB was issued regarding entry and exit precautions and is included in the Correspondence of this activity. The SPIRIT OF LAHAINA was removed from the north side of the channel by the vessel KAULANA, after which it returned to its slip under it's own power and steerage. No injuries or pollution resulted from the grounding. Vessel inspection requirements were issued and resolved under Inspection Activity 3918509. Due to the failure to timely notify the CG of the incident the actions of the master have been referred for enforcement.

3807346 - A cruise ship passenger slipped and fell while descending a stairway on the P/V GOLDEN PRINCESS. His wife, who was behind him on the stairs, reports that he probably missed a step. As a result, after his fall his left leg was determined to have fractured. No vessel deficiency is established; no crewmember was directly involved in this SMI. As a result, drug and alcohol testing was not required or completed. Injured was walking into the "Donatello Dining Room" (Deck 6, starboard side entrance). Reportedly he was an able walker and was not carrying anything at the time of the incident. He was wearing regular dress shoes. His wife asserted that the step was not properly lit and marked. The area of his fall was inspected immediately after she made this assertion. The location where he fell was reported to be in good order. The area was properly lit in both artificial and natural illumination and marked with no defects. The vessel was transiting sheltered waters at the time of the incident.

Cabin was inspected to be in good condition with all lighting working. Passenger did not have the lights on and was barefoot at the time. Injury due to passenger falling and had nothing to do with problems on the ship.

12JULY2010 - Passenger aboard the M/V GOLDEN PRINCESS died due to natural causes. Passenger was hospitalized two weeks prior to the cruise for acute rectal hemorrhage and collapse at home. Passenger was admitted to ships doctor at 2015 on 10 July 2010. Passenger deceased at 0247 12 July 2010. Cause of death: Unmanaged ongoing gastrointestinal hemorrhage, acute renal failure, pneumonia, multi-organ failure, acute pulmonary edema, and cardiac arrest/failure.
