Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Medical-Crew Gastrointestinal (GI) Illness Reporting

A casino dealer with onset of vomiting and diarrhea symptoms at 11 am on 23 November did not report his symptoms to the medical department until 10:30 pm that same evening. According to the records reviewed, this individual ate lunch in the crew mess and worked from 10 am to 4 pm, and again later worked from 9:30 pm to 11 pm. There was a disciplinary warning noted in his record for this. On 15 November a buffet steward with diarhea and vomiting onset at 9 am reported his symptoms to the medical department at 4 pm that day. A review of his work history revealed this employee worked from 7 am to 1 pm that day.

Children Area-Pelican's Room

The waste bins in place for the male and female child toilets for disposal of items soiled due to either a vomit or fecal accident in the child activity area were actually bins designed for diaper disposal. The bin lid had a small opening for disposal of a rolled and sealed waste diaper, but was not large enough to place larger items, such as soiled clothing that would need to be wiped off and removed for special cleaning.

Integrated Pest Management-Pest Management Logs

There were no logs verifying that incoming shipments of parts and other supplies to the Technical Department on the ship were being inspected. All other departments had log notes for the supplies delivered indicating they were inspected for pests.

Potable Water-Potable Water Tank Maintenance

Since the previous VSP inspection in April 2014, there were four different potable water tanks which were cleaned and touch-up or fully painted on the following dates: #1 port - 1 September, #6 starboard - 15 July, #7 port - 15 June, and #10 starboard - 17 May. A review of the tank log record for each of these tanks indicated that they were painted and indicated the paint used, but in none of the records was there a statement about the paint application or the drying or curing times involved. For the final cleaning and disinfection of each tank there was a notation that prior to going into service the chlorine residual was verified to be below 5 ppm and that record was in the water log book. A review of the water log revealed that there were values for the tank suction following most of the tank maintenance activities, but there was no chlorine residual given for potable water tank #1 port, which was cleaned and disinfected on 14 September.

Bar-Deck 5 - Lobby Wine Sushi Bar

The clean red food slicer contained bits of food debris around the blade spindle and under the slicing area. This equipment was taken out of service and cleaned.

Bar-Deck 5 - International Cafe

The stainless steal panel on the left side of the wall area enclosing the panini press has a 0.5 to 1.0 cm gap where the caulkng was missing. There was food debris inside the crevice.

Galley-Deck 14 - Lido Soup Prep Area

The floor tile in this area was in soiled and in disrepair.

Galley-Deck 14 - Combination Oven

The ventilation for this oven was not adjusted properly, causing condensate to collect above the unit and drip onto the galley floor around the unit. There also seemed to be an issue with the gasket on the oven door allowing steam to escape. It was reported that the ventilation was adjusted to prevent this accumulation in the future.

Galley-Deck 5 - Garde Manager

During preparation a water condensate was dripping from a ventilation duct onto the sandwiches. There was also a similar condensate dripping on the handwashing sink 4 meters away. This condensate was dripping through a speaker. Engineering was contacted and explained that the insulation in this area was being replaced and when the ship lists it may cause water to drip in the area.

Provisions-Deck 4 - Frozen Food

Non potable water ice sculptures were not segregated and stored beside and in-contact with consumable frozen noodles inside the walk-in freezer.

Bar-Deck 16 - Calypso Bar

The sanitizer solution was below 50 ppm. This was corrected.

The inspector observed condensate from an overhead duct dripping on sandwiches in the food preparation area. Two trays of food were discarded

Galley-Deck 5 Fruit Pantry

Pass through refrigerator number 4 was listed as on time control on the posted time control plan and on the unit. However, two trolleys filled with assorted cheeses and fruits were stored in this refrigerator unit with seven day discard labels. Staff stated the food on the trolleys were under temperature control not time control. When measured the temperature of the food items was 39°F. Once it was determined the unit was indeed under time control and not temperature control the food was discarded.

The air relief vents on the backflow prevention device for the ice machine were blocked with an accumulation of sediment.

Some equipment for the old soda dispensing unit, such as the carbonators, were still located in the soda dispenser cabinet. Staff state this machine had not been used in over a year.

Several food items in the gazebo, such as a container of raw shell eggs and containers of omelet ingredients, were labeled with a time control discard time of 11:30AM. When the inspector arrived in this area at 11:45AM these items were still out on the preparation counters and food workers were in this area setting up for the lunch operation. The portable hot cabinet unit under time control also had a tray of omelets and breakfast potatoes with a discard time of 11:30AM stored in the unit at 11:45AM.

Recreational Water Facilities-Spa Activity Pool

The free chlorine levels were consistently below the required 2.0 ppm minimum for this RWF. Four samples taken by the staff and the inspector from different areas of the pool were 0.50 ppm, 0.90 ppm, 0.71 ppm, and 0.92 ppm. The passengers swimming in the pool were asked to leave the pool and the facility was closed. The analyzer chart recorder in the pump room was measuring 0.69 ppm of free chlorine.

The analyzer chart recorder documented a rapid decline in the free chlorine levels since the RWF was open. The inspection of the dosing area found that the pump for chlorine fell off the top of the drum with chlorine and was not dosing the RWF. The analyzer chart recorder was measuring 0.69 ppm of free chlorine and a visual alarm was active on the display. It was unclear if the alarm went off in a continuously manned space.

The water fill level of the RWF was about 2 feet lower than the skim gutter level. Swimmers were inside.

There were Virox dispensing stations in the following food areas: Cafe Caribe galley and pantries for returned trays from the cabins on deck 10 aft and deck 10 forward. Spare bottles of the chemical were stored under the dispensers. This chemical was not to be used or dispensed in food areas.

Preparation Room-Walk-In Cooler

A lexan box of cut tomatoes was not labeled with a discard label and the production date was unknown. During the discussion of the issue, staff stated that preparation took place the night prior. There was an entry for cut tomatoes on the cooling log that the staff cross-referenced to this specific box. Staff then labeled the box accordingly.

However, no cooling log entry was made for the salmon pasta salad when the ingredients were combined. A similar discrepancy was noted on the same log for a ham, cheese, and vegetable salad. A cooling log entry was made for strips of ham but no entry was made for the prepared ham, cheese, and vegetable salad.

Large sections of missing deck grouting were noted around the preparation table in the middle of the room. Standing water was present in several sections were the deck grouting was missing.

No cooling logs were available for this tray of sliced tomatoes. The product was discarded.

Specific leaks observed included onto the counter top on bain marie unit 10 in the hot galley, to the left of the ovens in pastry, and from a light fixture on the soiled end of the forward starboard light fixture.

Potable Water-Microbiological Monitoring

In 5 of the last 6 months, several random potable water sample locations were repeated in consecutive months. The repeated sample locations included the Wheelhouse, Pizzeria, and far point.

A lemon garnish and a beverage mixing stirrer was in the basin of the handwash station.

The closet inside this room contained a mix of food service equipment, cleaning equipment, and insect pesticides. Boxes of unopened plastic coffee pitchers, boxed wine glasses, uncovered and cleaned metal fruit baskets, serving tongs, a tray of cleaned glass syrup dispensers, plastic hotel pans, and a dishwash rack of uncovered and cleaned small plastic condiment containers were stored alongside and in contact with a box of insect bait stations, a can of aerosolized mosquito pesticide, a rubber hose, floor scrubbing equipment and other miscellaneous non-food related items.

The Crew Mess and other locations were used for a microbiological sample on 3 months out of the past 6 months of the required representative microbiological monitoring of the drinking water distribution system. In other months, a larger variety of sampling locations were noted.

Recreational Water Facilities-Monitoring / Signage / Testing

The ship is still in the process of evaluating the best method for automatically monitoring the halogen residual for each recreational water facility. Currently, the halogen samples are taken from the return water at the pump in the engine room which is after the compensation tanks which are on Deck 14, under the Rec Water Facilities. / New temporary signs have been installed that meet the new informational requirements of the manual except for adding each facility's bather load in the blank provided on the sign. / Equipment for testing new parameters such as combined chlorine and total alkalinity was being delivered during today's inspection. The daily log records have already been updated to address these new requirements. (New requirements in 2011 VSP Operations Manual)

Potable Water-Backflow Prevention Testing

The correct procedures are being followed for backflow prevention testing. The results, however, were showing overall pressures for each valve rather than the required pressure differential for each valve in the testable assemblies.

The ice scoop in an active bar was found with its handle in the ice. The plastic ice scoop holster where it should have been stored was in place.

Foods of animal origin that could be served undercooked such as steaks and hamburgers in the crown grill, sabitini, and room service were not identified and there were no consumer advisories on the menus.

Staff responsible for measuring thin foods did not have tip-sensitive thermometers. According to the staff, the thermometers have been ordered.

The light intensity was less than 110 lux behind and around several pieces of deck- and counter-mounted equipment including: ice machines in the galleys and deck pantries, cooking equipment, and equipment on beverage stations.

The safety signs for the whirlpools did not include the information required for the swimming pools above or a caution against pregnant women, elderly persons, or exceeding 15 minutes of exposure.

There were no procedures in the OPRP for returning the vessel to normal operating conditions after an outbreak.

There was no disinfectant in the diaper changing station.

In the girls toilet room, the child accessible toilet was 16 inches above the deck and the handwashing sink was 26 inches above the deck and there was no step stool available.

The disinfection policy and procedure (section 5.3.2) stated that staff were to use a 50 ppm halogen concentration solution for 4 hours contact time. This calculates to a CT value of 12000. The new CT value is 15300.

There were four backflow prevention devices on the washing machines (2 hot/2 cold) in the deck 4 officers laundry. Two of the devices were Watts 9D and two were Watts N9. The backflow prevention device listing indicated that all four devices were Watts 9D.

Filth flies were circulating throughout the food areas of the ship and the following were counted: deck 5 main galley pastry - 3, potwash - 4, crew galley - 2, crew mess and beverage station - 10, staff mess buffet - 4, provisions meat preparation room - 3, fish preparation room - 2, horizon court galley -2, horizon court gazebo areas and buffets - over 30. There were also fruit flies found in the following areas: deck 5 main galley garde' manger - 2 and crew galley - 1. All the above areas were in inoperation during the inspection and either food or food and clean equipment was present. Staff stated this was due to a heavy fly area the ship made a port call to 3 days prior.

There was a single label used for the 7 day discard of 10 separate trays of prepared pastry dishes in reach-in upright refrigerator #9/10. The label listed assorted pastries with a preparation date of 26 March. One tray of mini cheese cakes was prepared according to the cooling log on 25 March.

Two plastic boxes of cut and washed cabbage had a temperature of 51 °F. These were prepared and stored last night according to staff, so they were immediately discarded.

The cut and washed lettuce and greens in plastic bins inside walk-in refrigerator #4799 had a food temperature of 46-51 °F. Staff brought ice to cool these items.

A sheet pan half-filled with hot ham and cheese sandwiches was held at a temperature of 118-120 °F in a hot trolley in the galley. The sandwiches were cooked some 30 minutes prior and were removed for reheating.

Sep 24, 2014

Personnel Casualties : 1 injury

On September 22, 2014 at approximately 2230 hours passenger lost balance and fell in the bathroom of stateroom C716 fracturing right distal radius while on board the Crown Princess which was docked along pier 27 in San Francisco. Passenger lost his balance and fell backward and attempted to catch himself but was not successful. The fall caused an undisplaced fracture to the distal radius right side. The ship was not moving, the lighting was adequate, and it does not appear that drug or alcohol use was a contributing factor. This casualty was not related to ship operations and there was no crew involvement.

Personnel Casualties : 1 injury

On May 4, 2014, at approximately 2200 local time, a passenger onboard the foreign-flagged cruise ship CROWN PRINCESS was injured after falling in a stateroom. The onboard physician determined that the passenger sustained a broken left arm. The passenger was treated by the medical staff onboard the vessel. The vessel was approximately 30 miles due West from Santa Barbara, California, at the time of the incident. A determination was made by the Sector San Francisco Investigations Division Chief that there would be little or no value gained from conducting an Informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

On April 28, 2014, at 0830 local time, the foreign-flagged cruise ship CROWN PRINCESS reported that a passenger tripped, fell, and fractured a bone in her right arm. The ship was moored in San Francisco, California, at the time of the incident. A determination was made by the Sector San Francisco Investigations Division Chief that there would be little or no value gained from conducting an Informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

Sector Miami was notified via CG-2692 that on 2 March 2012 at approximately 1030, an 84 YOF passenger lost her balance and fell while she was attempting to transit to her cabin's bathroom facilities. Due to her fall, the passenger sustained an injury that was diagnosed as a fracture dislocation of the right ankle. No drug or alcohol tests were conducted on the ship's crew as the casualty did not involve marine operations nor did any action or inaction on the crew's part act as a causal factor in the casualty.

A male passenger had just gotten onboard the Crown Princess prior to the vessel getting underway to Bermuda. He was walking down the passageway to his cabin and became unsteady, tripped and fell, hitting his head on the bulkhead. He was seen by the ship's physician, and due to the hematoma to his left temporal area, was disembarked and taken to a local hospital for further evaluation and released a few hours later. No post casualty drug or alcohol tests were administered to the ship's crew as no action or inaction by the crew contributed to the occurrence of this casualty.

On 07OCT11, a passenger on the C/S CROWN PRINCESS (ON 733770) was walking in an interior space on the vessel when she tripped and fell to the deck, fractured her kneecap. The passenger was not under the influence of alcohol at the time of the accident. The ship was at anchor in Bar Harbor, ME.

04OCT2010: MSD Belfast received a CG-2692 from the C/S Crown Princess regarding a passenger that suffered an injury. 0800 on 04OCT2010 a passenger missed stepped going down a set of stairs, fell and injured her left arm. Passenger was taken to the ship's doctor whom determined that the injury was a fractured humerus. An inspection of the area where the injury occurred was done by the safety officer. Inspection by the ship's safety officer revealed that the area was in good order and was well lit. Drug and alcohol testing were not conducted, as no one other than the passenger was involved. Downgraded from Informal to Data Collection due to the Deep Water Horizon Response.
