Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

Inside one of the food equipment lockers, some previously cleaned utensils and the container they were stored in were soiled with a brown material.

The cooling log documented beef stock was cooled starting at 14:01 with a temperature of 146°F, followed by 15:56 with a temperature of 68°F. No further temperatures were documented to indicate the food was cooled to 41°F or below and no notes for this item were on the log. The preparer explained the food was reheated for immediate service.
Galley-Deck 5 - Center Beverage Station
In the food splash area of the iced tea machine, residue was inside the dispensing ports and on the underside panel. The machine had been previously cleaned and had not been in operation since the previous evening.

Provisions-Beer and Soda Room
Between most of the deckhead panels, condensation droplets formed and some were observed dripping onto the deck. Staff stated that insulation and materials for the condensation and cooling lines were scheduled to be delivered in a few months. As a temporary solution, cardboard sheets were placed on top of the boxes to protect the beverages. A few small wet areas of cardboard were observed, indicating leaking condensation.
Galley-Deck 6 - Starboard
Aft of the cold pantry, inside the lower in-use ice machine compartment, a layer of brown material was on the water collection tray. Staff explained this was rust from the small pump. The machine was cleaned and sanitized in January 2015.

The food splash area of the buffalo chopper lid was cracked. Staff showed the inspector a new lid was available to replace immediately.

Inside the grease tray housing of the flat and grooved grills, excess silicone was installed around the chute. Staff immediately replaced the silicone.

Medical-Immediate Contact Follow-up
The 48-hour follow-up interview was not documented for the cabin mate of a reportable acute gastroenteritis case crew member that reported to the medical center on 16 March.
Medical-Acute Gastroenteritis (AGE) Log
A crew member reported to the medical center on 14 March after experiencing abdominal cramps and three episodes of vomiting. The crew member's medical records identified her as a reportable AGE case, but she was documented on the AGE log as nonreportable. Subsequently, the submitted 24-hour electronic medical report for the voyage listed six reportable AGE cases instead of seven.
Medical-Acute Gastroenteritis (AGE) Log
The AGE log for the voyage ending on 1 March listed seven reportable AGE case passengers. The 4-hour electronic medical report submitted to VSP listed five reportable AGE case passengers. An additional passenger reported to the medical center with AGE symptoms after the 4-hour report was submitted, which brought the total number of reportable passenger cases to six. One passenger entry on the AGE log was listed as a nonreportable case, but appeared to be counted as a reportable case on the AGE log.
Medical-Acute Gastroenteritis (AGE) Log
The total number of passengers and/or crew members listed on the AGE logs did not match the numbers indicated on the electronic medical reports submitted to VSP. For the voyage ending on 15 March, the AGE log stated 3,000 passengers and 1,176 crew members, but the submitted 24-hour electronic medical report listed 3,348 passengers and 1,191 crew. According to medical staff, the 24-hour report contained the correct number of passengers and crew members.
Medical-Immediate Contacts
Medical staff was asking reportable AGE case crew members if they had additional immediate contacts (i.e. boyfriend, girlfriend, spouse, etc.) besides cabin mates, but it was not documented if they did not.
Potable Water-Bunker Pretest
According to staff, the initial shore side chlorine and pH values were taken before bunkering commenced, but were not documented.
Recreational Water Facilities-Hair and Lint Strainer Disinfection
The hair and lint strainer and housing for all RWFs were not cleaned and disinfected weekly dating back to 14 January. The recorded dates were 11 March, 3 March, 23 February, 11 February, 1 February, 23 January, and 14 January.

Water was continuously leaking from the relief vent on the reduced pressure assembly backflow prevention device installed on hot water laundry fill line.

The list of testable backflow prevention devices indicated one testable device installed on the two chemical dispensing connections in the cart wash room. Upon inspection the two devices installed were atmospheric vacuum breakers. The atmospheric vacuum breaker on the left connection was connected to a hose with a spray nozzle on the end.

Potable Water-Production
The records for 3-13 April showed some of the free halogen residual levels were less than 2.0 ppm during potable water production while sailing in the U.S. The watch officer was consistently recording halogen residual levels below 2.0 ppm and did not state any corrective action was taken to increase the halogen residual level. For example, on 8 April, the chlorine residual level was recorded as 1.83 ppm, 1.45 ppm, and 1.4 ppm during four hours of production. On 12 April, no calibration was recorded at the start of production, and a manual chlorine residual recording of 1.82 ppm was noted after eight hours of water production. On 13 April, the chlorine residual level was recorded as 1.67 ppm and 1.8 ppm during seven hours of production.

Staff stated that the condensate was thawing because the door to the freezer was being opened continuously during the turnaround day provisioning.

Frozen condensate on the deckhead was thawing and dripping on the boxes below. The cardboard on two boxes of spinach lasagna sheets was soaked through. Cherry cheesecake was discovered in the pastry walk-in freezer with a label dated 02 March 2014 and with discard date of 08 March 2014. This item could not be located on the food cooling logs. The cheesecake was discarded.
Buffet-Staff Mess
Two trays of cheese on the staff mess buffet line did not have discard labels. These items were on time control.
Bar-International Cafe
The cooling log listed shredded beef prepared 05 March. The food was adequately cooled from 141°F to 70°F within two hours. For the cooling from 70°F to 41°F, the temperature was not recorded on the cooling log when the food was taken out of the blast chiller. It was unclear if the temperature of the shredded beef was below 41°F when it was removed from the blast chiller at 3:00 a.m. on 06 March. The food was discarded.
Medical-Crew Gastrointestinal (GI) Illness Reporting
On 1 February a dancer with GI illness symptom onset at 10 am did not report to the medical center until 5:45 pm and worked with those symptoms from 1:30 to 3:30 pm and again from 4:30-5:30 pm. On 3 February a buffet steward experienced GI illness symptom onset at 3 am, and did not report to the medical center until 12 noon. This crew member worked from 9:30 am to 10:30 am, ate lunch in the crew mess, then worked again from 11-11:30 am. On 1 February a security crew member experienced GI illness symptom onset at 5 am and did not report to the medical center until 10 am and ate at the staff mess 7:45 am. Finally, on 18 February an assistant cabin attendant experienced GI illness symptom onset at 1 pm and did not report to the medical facility until 7:10 pm. This crew member worked from 11:30 am to 3:30 pm. All of the above crew members became GI illnesses cases and all were disciplined.
Housekeeping-Cabin Jetted Tub Disinfection
The cleaning and disinfection of the suite jetted whirlpool tubs exceeded the 7 day requirement based on the review of records. The record reflects the suites tubs were cleaned and disinfected on 17 and 25 February, 2014 and on 18 and 27 January (8 and 9 days between treatments).
Integrated Pest Management-Incoming Supplies
Staff could not provide a record indicating flower deliveries, which occur on every turnaround day for each voyage, were inspected for pests. Staff stated they inspect the flower deliveries and report the results through the supervisory chain, but no entries for inspection deliveries of flowers were indicated in the IPM logs.
Potable Water-Distribution - Micro Testing
The staff conducting the microbiological tests for 4 random potable water samples monthly was not checking the actual water temperature or pre-warming the sample per the kit manufacturer's instructions prior to incubation. The test period is 18 hours and it relies upon a sample water temperature range of 33-38 °C prior to incubation. It was also unclear if the result at 18 hours per the manufacturer was definitive for negative sample results, as the instructions stated that definitive negative results are after 22 hours of incubation.
Potable Water-Air Gap Inspections
The backflow prevention log listed 54 air-gap locations throughout the various decks, but it was unclear if they were not inspected or the inspection was not recorded in the logs; three small air-gaps were inspected and logged. Additionally, the non-testable backflow prevention devices throughout the ship did not have entries for any on the results of the inspection on the log; staff reported these devices were visually inspected to verify their condition. The backflow log indicated notes/remarks, but none was made for any preventer.
Bar-Mermaid's Bar
No waste receptacle was observed at the port handwash station in the front bar. This was corrected during the inspection.

Potable Water-Far Point Chart Calibration
The far point analyzer was reading 0.87 ppm when the inspector took a sample with a result of 2.42 ppm. The crew engineer took a sample with the inspector and his result was 2.32 ppm. An additional test taken by the inspector and crew engineer yielded results of 2.31 ppm and 1.69 ppm, respectfully. The crew engineer calibrated the far point chart earlier in the day.
Potable Water-Instrumentation Reference Standards
Crew members in charge of potable water sampling were not knowledgeable about the reference standards used to verify the accuracy of their electronic test kits. Crew members did not know how to use the reference standards to check the calibration and did not know when the last time their test kit had been checked.
Buffet-Horizon Court - Port Beverage Station
Two pitchers of ice water used for waiter service were located immediately next to the passenger beverage station and were not covered or protected from possible contamination. A passenger was observed using the water pitcher. The pitchers were covered and moved away from the passenger beverage station.

Several containers of apples and pears out for passenger self-service did not have their own dedicated service utensil. This was corrected.

Not all breakfast menus had asterisks and consumer advisories for the animal derived food items that could be ordered raw or undercooked.

A small fruit fly was observed flying above the clean storage racks next to the dishwash machine.

Heavy steam was escaping from the sides of a covered baine marie located furthest to the right and condensing on the shelf and hood area above. The condensate was observed dripping from the deckhead hood area and onto previously cleaned plastic plate covers that were being stored on the pass-shelf below. No food items were impacted by dripping condensate. The baine marie was immediately turned down.

Condensate was dripping from the deckhead onto previously cleaned plastic plate covers as noted in the previous violation. These were sent to be re-cleaned.
Provisions-Poultry Thaw Room
A clear liquid, which appeared to be water, was dripping from a deckhead seam and directly onto 2 uncovered trays of unprotected thawing chicken. The two trays of impacted chicken were immediately discarded.

The dispensing nozzle for the draft beer had a build-up of a brown granular residue of more than a day's accumulation.

An in-use bulk milk container in the time controlled bulk milk dispenser near the Gazebo Grill was not discarded after 4 hours. At approximately 0900, the inspector observed the container of milk in this unit on time control with a discard time of 1230. The inspector physically wrote the time of 1230 on the cardboard surface of the product. When the inspector returned to this unit at approximately 1300, the same container of milk had the time of 1230 on the unit and a new discard time of 1400 had been written on the milk by staff. The container of milk was immediately discarded.

Electrical cords and water lines to beverage equipment at the coffee station were laying on the counter making cleaning difficult.

A large blue cooler filled with clean plates, silverware, linens, wrapped beverage straws, and other food service equipment was stored on the deck inside this corridor. The corridor was not constructed to protect the equipment from splash, dust, or other contamination. The cooler and equipment were removed. According to staff, this cooler is used for shore excursions.

Several lights had been installed throughout the food areas since the last inspection.

A nonfood worker had an onset of acute gastroenteritis (AGE) symptoms at 7:00 a.m. on February 28,2013, but did not report to medical until 6:30 p.m. the same day. According to the documentation, this individual did not work while sick. A food worker had an onset of AGE symptoms at 8:00 p.m. on February 7, 2013 but did not report to medical until February 8, 2013 at 8:30 a.m. This individual worked from 8:00 - 10:00 p.m. on February 7, 2013. A nonfood worker had an onset of AGE symptoms at 11:00 a.m. on February 8, 2013 but did not report to medical until 5:00 p.m. on Feburary9, 2013. This individual worked from 6:30 p.m. until midnight on February 8, 2013. A food worker had an onset of AGE symptoms at 7:00 p.m. on January 31, 2013 but did not report to medical until February 1, 2013 at 9:05 a.m. This individual did not work while sick. A food worker had an onset of AGE symptoms at 9:00 p.m. on February 12, 2013 but did not report to medical until February 13, 2013 at noon. This individual worked from 9:30 p.m. on February 12, 2013 until 2:00 a.m. on February 13, 2013. A food worker had an onset of AGE symptoms at 7:00 p.m. at January 15, 2013 but did not report to medical until January 16, 2013 at 1:30 p.m. This individual did not work while sick.

The ship's "Faecal/Vomit Accident Response Procedures" did not differentiate between formed and loose stools. The procedures included a form that is completed following an accident, but only "faecal" or "vomit" were choices as type of accident.

The plan never addressed a 2% threshold of passengers or crew to start the elevated levels of cleaning. She was treated while onboard and will seek additional medical treatment when she returns to Canada.

At 1615 on 19 October 2012 a passenger on the cruise ship EMERALD PRINCESS slipped on a wet tender platform deck and fractured his ankle. He had been helping his wife into the tender platform elevator, then walked backwards to take the stairs himself; he placed a foot outside the carpeted area onto a wet metal deck and slipped. He received medical treatment aboard the vessel. The ship was anchored in Frenchmen Bay near Bar Harbor ME. Chemical testing was not preformed, as no crewmembers were directly involved in the incident. The ship did not notify the Coast Guard of the incident until sending a fax to Sector Boston on 20 October 2012 at 1731, more than 25 hours after the incident occurred.

A male passenger was feeling ill and a ship's nurse was called to his cabin. The Pax became unconscious shortly after the nurse arrived. The ship's physician and medical team responded and attempted to revive the man with CPR, a defibrillator, adrenalin, and oxygen, with unsuccessful results. The Pax was pronounced deceased by the ship's physician. Cause of death determined to be Cardiac Arrest. Arrangements were made to have spouse and deceased pax return to the U.S. Downgraded to Data Collection due to cause of death from natural causes.

Downgraded due to death by Natural Causes. Passenger fell while transiting the passageway of the M/V EMERALD PRINCESS and sustained a fractured wrist. Investigation determined that the threshold was adequately marked with yellow and black caution tape to indicate to passengers that there was a marked difference between the deck levels. The passenger tripped on the threshold, fell onto his left side and did sustain a fracture to the left wrist. The passenger was taken to ship's medical and did receive treatment in the form of setting and casting the broken area. Passenger age cannot be ruled out as a factor involved in the incident. Passenger declined to disembark the vessel early.

Passenger stated that she was resting on the stateroom couch and, while getting up to respond to a knock on the stateroom door, fell. She stated that it was a "clumsy moment" and was very complimentary to the vessel crew's quick response and professionalism. The stateroom was checked by crew to verify/check for any contributing factors, i.e. lighting or carpet condition. All was found to be satisfactory.

The Passenger stated that, at approximately 16:30 on the date of embarkation, she tripped over the life jackets that her spouse had pulled out in preparation for the mandatory abandon ship drill and left on the floor of their stateroom. She stated that she sustained a compound fracture of the left Humures. The stateroom was checked by the vessel's crew to verify/rule out the condition of the carpet and lighting as contributing factor to the incident.
