Federal health inspectors from the Centers for Disease Control and Prevention inspect cruise ships twice a year, checking everything from kitchen equipment to dishwasher temperatures, and grade ships based on what they find. A score below 86 is considered failing. Cruise ships are also required to report all illness outbreaks affecting more than 3% of passengers or crew to the CDC. Inspections and outbreaks from 2010 to the present are shown below.

The consumer advisory for the cooked to order omelets was missing on the port service line. Staff immediately placed it on the line. The omelet station and port service line was open.

The port side shutter door had a gap between where the door frame and the deck, and an accumulation of dust and debris was observed in the gap.

Three previously cleaned silverware racks were soiled and stored on a clean table.

Many of the ice machines had an accumulation of dark debris on the pan below the cuber panels. This was noted for ice machines in the Deck 14 warewash area, room service, and Deck 6 main galley. Staff stated the debris came from the motor above the panel. This finding was noted on the last inspection.

Water was continuously dripping from a deckhead technical hatch onto the deck, and no equipment was impacted.

The cords for the juice machine were lying on the counter. This was corrected.

Food Service General-Ice Machines

The interior surface of the exterior panel of the ice machines were soiled with a black substance on ice machine 14-07-04, 10-06-02, and 6-03-02.

Some of the grease drain lines for the grill grease pan housing had gaps around the lines, creating a difficult to clean space. This was noted for the drain lines for the flat grill in room service, starboard line flat grills on deck 6, grills in Crown Grill show galley. A hole where a screw was missing was also noted around the grease drain line for the grill grease pan housing in the Crown Grill galley.

A stack of clean plates were stored next the juice machines and touching the dusty water supply line to the juice machine.

The interior surface of the exterior panel for ice machine 6-03-02 was soiled with a brown substance, which was soiled over the entire surface, including the surface over where the ice drops. This is considered a food contact splash surface.

The sensor for the ice machine to determine the fill level was broken. Ice was observed overfilling in the entire machine, including where the cuber defective panels were located.

The play mat was soiled with hair and debris at the edges at the floor and mat junction. The area underneath the mats was wet, and additional debris was noted.

Recreational Water Facilities-Spa Pool

The spa pool uses a current device for exercise that fits the definition of the pool as an interactive RWF. Although staff have long ago adjusted the pools halogen level to the interactive range, the turnover rate is not adjusted and remains at 3.3 hours. Corporate had a planned renovation for piping and pumps to achieve the two hour turnover rate for this pool during the December 2014 dry dock.

The deckhead over the front bar was constructed of sail cloth, which was absorbent and not a hard material. Staff stated that this deckhead is scheduled to be changed in the December 2014 dry dock. Additionally, the back bar decorative bulkhead was severely damaged along the raised art work attached there. Broken and chipped areas were observed throughout the art piece and the bulkhead could not be properly cleaned around it. Staff stated this would be changed in the dry dock as well.

The artificial light level behind the ice machine appeared to be below the minimum 110 lux, but was hard to confirm due to a folding table stored behind. Staff should verify the level and add light as needed.

The shutters used for the bottle display of the adjacent Mermaids Bar had damaged edges all around that made them difficult to clean.

The ventilation in this in-use area was in disrepair. There was heavy condensation collected on all surfaces, including the deck, bulkheads, deckhead, ice machines, flight-type dishwashing machine, and clean and soiled areas. Two food employees were working in the area at the time of the inspection and they were sweating heavily. Condensation was dripping on clean and soiled surfaces. The area was evaluated hours after and the ventilation was repaired and the clean surfaces were still being cleaned and sanitized.After about two hours of the first visit to the area, there was heavy condensation collected on the deckhead above the soiled end of the in use flight type dishwasher. Also, condensation was dripping from a deckhead hatch to soiled items next to the dishwasher. The hatch had a large amount of condensate pooled on it.

For example: (1) there were four slotted fasteners in the food-splash zone of the Vitality coffee machines; there were more than 15 of these machines in the food areas; (2) the Aurelia espresso machines had two slotted fasteners above the milk steamers; there were at least three of these machines in the food areas; (3) the popcorn machine in front of Sundaes Ice Cream Bar had four slotted fasteners in the food-splash zone; (4) there were one to two slotted fasteners on the right side panels of each of the four compartments of the pizza oven in Sabatini Show Galley.

In addition, the food contact surfaces of the popcorn kettle and the flexible hose attached inside were soiled with food residue. The machine was stored as clean and covered with a tarp.

Most of the technical compartments for the undercounter refrigerators in the food areas were heavily soiled with dark materials that appeared to be mold.

Numerous entries had a handwriting that was the same for the PHFs names, times, and temperatures while there were signatures with significantly different handwriting from food employees for each step of the cooling process. As a result, the records seemed unreliable.

Both boxes of microbiological test reagents in the engine control room were expired. One box had an expiration date of 16 April 2014 and the other 17 August 2013.

Potable Water-Microbiological Analysis

The 18 hour test kit used for microbiological analysis of potable water for Escherichia coli was being used incorrectly, in that staff stated they collect the water sample, mix the reagent, shake. and place in the incubator for 18 hours. The kit manufacturer required that the water sample after mixing with the reagent must be warmed using a water bath to the incubator temperature prior to placement in the incubator. This violation was noted on the previous VSP inspection report.

Recreational Water Facilities-Spa Pool

Staff stated this recreational water facility was classified as a swimming pool and the halogen residual levels were maintained between 1.0 to 5.0 ppm. However, this pool had a counter-current swimming feature that made it an activity pool and the halogen residual levels must be maintained between 2.0 to 5.0 ppm. The chlorine residual measure during the inspection was 2.8 ppm and consistently above 2.0 ppm during record review.

In the walk-in cooler, there were three large hotel pans of lettuce. The cooling time and temperatures were not documented in the cooling log. Food staff stated that they were taking the cut lettuce from bags coming from the provision area, washing them, and transferring them to hotel pans. Even though they recognized that this preparation time was enough to rise the temperature of the lettuce above 41°F, they did not think that the lettuce cooling had to be documented on cooling logs.

Housekeeping-Deck 15 Men's Changing Room

There were two toilet stalls in the spa area men's changing room which open to the handwash sinks. At the exit door there was no signs to advise use of a paper towel to open the door and there was no waste bin to discard a paper towel near the door.

Medical-Gastrointestinal (GI) Illness Reporting

The ships GI illness report was incorrect on the TP9338 voyage. One crew fitter reported diarrhea, which was logged in the medical record notes as above normal and a reportable case, was listed in the acute gastroenteritis surveillance log as a non-reportable case.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

The column for underlying illness for most of the individuals listed was not completed with information that answered that question. There were various types of information listed in this column over the last five voyages, but nothing specific to any underlying illness was written in several of these logs. Part of the problem is due to the fact that the ship in fact maintains a master AGE log with instruction detailing numerous comments for this data field, and this data automatically populates the same field in the AGE log required in the VSP Manual, which is called the reportable log in the ship's electronic system.

Medical-Acute Gastroenteritis (AGE) Surveillance Log

On one AGE log there were three separate cases among crew where the time of illness onset was the exact time of the crew members report to medical of their illness. On another log there was one such case of a crew member reporting at the exact time their symptoms started. Lastly, a bar stewardess appears on the AGE log with illness onset at 7 am on 5 November, 2013, but reports to the medical center at 6:45 pm. On the medical record note one time is crossed out for illness onset and 1800 is written in. On the self administered questionnaire the crew member left blank the time of illness onset, so it was unclear when the diarrhea symptoms began.

Provisions-Cambosa Area

There were approximately 200 fifty pound mesh bags of potatoes and one pallet of boxed potatoes, one pallet containing 50 mesh bags (50 pounds each) of onions, two pallets containing 120 bags (50 pounds each) of flour, and 17 sacks of rice (50 pounds each) on one pallet in an area with open deckhead filled with exposed conduit, cable bundles and trays, wastewater pipes (one of which was leaking), exposed ducts and wires. One of the pipes over the center of this food had an open end. Staff moved the food to a more protected corridor during the inspection.

Provisions-Cambosa Area

There were six live fruit flies observed on the onions and potatoes stored on several pallets in mesh bags in this provision staging area.

Children Area-Child Activity Center

In both child toilet rooms for children under the age of 6 there were no air-tight covered waste receptacles.

There were some listed backflow preventers where there was no specific plumbing system component listed in the description, but instead it was an area statement such as hospital hot and cold or hair salon hot and cold.

In reviewing the maintenance on potable water tanks from September, 2013 thru December indicated that painting was done on tanks 1 port, 4 port, 5 starboard, and 6 starboard. The record did not include a statement that the tank paint was applied in accordance with the manufacturers recommendations.

Other-Aft Service Lift

While VSP inspectors were entering the non-food service lift food staff were observed leaving that lift with a trolley filled with a large stack of previously cleaned sheet pans which were not covered or protected. At deck 5 the lift stopped and two food workers attempted to enter the non-food lift with a trolley containing a large, open plastic bin filled with cherry tomatoes. Two of the tomatoes fell to the deck and one of the food workers picked them up and tossed them into the trolley as they rolled it away to the galley nearby. Inspectors immediately found a galley supervisor to advise about these activities and brought him to the lift corridor outside the galley. While standing at the non-food lift another food worker attempted to roll a food trolley into the lift directly in front of both inspectors and the galley supervisor. The supervisor immediately pointed to the large sign on the lift door expressly prohibiting food and food related items being transported in this lift.

A small shop containing various packaged snack foods and candy in deck 8 forward had several boxes of snacks including potato chips, cheeze and other snacks stacked on the deck below the already full bulkhead mounted storage shelves. These foods were not on deck stands and were in direct contact with the carpeted deck.

The large circular light fixtures between the pastry counter and the main buffet line were not shielded or equipped with shatter-resistant light bulbs.

The sanitizing solution in the sanitizer bucket for this area was less than 50 ppm chlorine when measured by the inspector.

The two exhaust vents on the deckhead above the middle hot buffet station had a significant amount of dust accumulated.

The previously cleaned deli slicer had a significant amount of dried food residue on the recessed ledge of the back plate.

The ice bin of the counter mounted ice machine had a black residue on interior right side of the bin.

Two fruit flies were observed in this locker which was used to store display items. When the inspector moved blankets and linens on the top shelf of the storage rack several small ants or mites were seen scurrying around. No food was stored in this locker.

The drain line for the handwash sink in the soiled drop-off area was leaking which caused water to pool on the deck.

The main disposal drain for the flight-type dishwasher was partially clogged. This caused a significant amount of water to overflow and pool on the deck where it eventually drained to the scupper lines around the machine. Maintenance was aware of this issue and had initially addressed it two days prior to the inspection. However, a more significant problem than expected was encountered and maintenance needed to order additional equipment to correct the problem.

A significant amount of condensation was accumulating on the deckhead above the soiled loading area of the flight-type dishwasher. Condensate was seen dripping on the dishwasher conveyer belt below and also the deck below, which added to the standing water from the partially clogged drain.

The soap dispenser for the handwash station to the left of the soda locker was clogged and did not dispense soap when the inspector went to wash his hands.

Approximately 10 fruit flies were observed in this pantry which was used as a staging area for racks of soiled room service trays. When the inspector arrived the door to this pantry was open and several racks were present with discarded food and beverages. Water was also pooled on the deck in the back of the pantry near the mop sink.

The previously cleaned flat top grill had food debris present in the corners of the drip channel leading leading to the drip tray chute.

Four pans of tomato soup about six-inches in-depth were in the blast chiller when the inspector arrived at 2:30PM. The cooling log indicated the pans were placed in the blast chiller at 9:00AM and reached 64 °F when checked by staff at 11:00AM. The inspector took multiple samples of each pan and yielded temperatures between 50°F and 79°F. Staff immediately took each pan of soup and divided it up into smaller portions about three-inches in-depth and placed the trays back into the blast chiller. At 3:00PM at the end of the 6 hour cooling process the inspector and staff took final temperatures of each pan of tomato soup. Four of the ten pans of tomato soup did not reach 41°F or lower and were discarded.

The cooling log indicated the pans were placed in the blast chiller at 9:00AM and reached 64°F when checked by staff at 11:00AM. The inspector took multiple samples of each pan and yielded temperatures between 50°F and 79°F. Staff immediately took each pan of soup and divided it up into smaller portions about three-inches in-depth and placed the trays back into the blast chiller. At 3:00PM at the end of the 6 hour cooling process the inspector and staff took final temperatures of each pan of tomato soup. Four of the ten pans of tomato soup did not reach 41°F or lower and were discarded.

0936- All steering gear components and the rudder stock, shall be of sound and reliable construction to the satisfaction of the Administration. The steering gear linkage is worn and the port and starboard units have temporary repairs holding the linkage together, 16 A/B/C.

Electrical

1499- Emergency source of electrical power in passenger ships shall provide suitable measures taken for safeguarding emergency operations. Port generator room vent is wasted, pre-heating system hose is wasted, supply ventilation is wasted, and exhaust ventilation has excessive oil in it. Both port and starboard generator batteries need to be secured.

Pollution Prevention/Response

1730- Any ship of 400GT but less than 10,000GT shall be fitted with oil filtering equipment and shall be of a design approved by the Administration. Vessel OWS has patches on the bilge pump line, OWS drain line, and discharge line to the bilge tank. There are signs of wastage throughout the piping system, 16 A/B/C, MARPOL Annex 1/14.

On August 4, 2014, at approximately 2250 local time, a U. S. citizen-passenger slipped, fell and broke his right wrist while walking across a deck aboard the foreign-flagged cruise ship STAR PRINCESS. The vessel was off the coast of British Columbia, Canada, when the incident occurred. The injured passenger was treated by the ship's medical staff and remained aboard for the remainder of the voyage. A determination was made by the Sector San Francisco Investigations Division Chief that there would be little or no value gained from conducting an Informal investigation for this casualty. As such, this case was downgraded to the Data Collection level of investigation.

Marine Safety Team Maui received a report that a passenger died while aboard a large passenger vessel on a voyage from Nawiliwili, Kaua'i, to Lahaina, Maui. The passenger collapsed while eating dinner and became unresponsive. Shipboard medical staff responded, conducted CPR, and transported the passenger to the ship's medical center. The passenger never regained consciousness and did not respond to resuscitation efforts. This investigation was downgraded to data collection in accordance with the Marine Safety Manual, volume 5, section B.7.A.2.b (natural cause).

On September 28, 2013, a passenger onboard STAR PRINCESS was injured after she slipped while walking on a pool deck. The passenger reported the injury to the vessel's crew the following day, the vessel's medical staff x-rayed the passengers leg and determined that she suffered a fractured patella.

The fall resulted in a fracture to her left shoulder on the head of its left hummers. Vessel is enroute Ensenada, Mexico.

08.12.12 pax was walking down an eight step atairway to the pax alley, as pax opened the door leading to pax acommodation area stepping over the threshold pax cought her left foot and fell fwd onto her left side sustaining injury of a hairline fracture to her left intertrochanteric region. The area inspected revealed no faults or conditions that could have contributed to the accident. There is an abundance of precautionary signs including advising pax to use handrails and to watch their step, the area at the time of the accident was lit naturally.

On April 19, 2012, U.S. Coast Guard Sector Honolulu received a report of an injured passenger on the STAR PRINCESS. The passenger fell at the pool/hot tub area and broke his right tibia. Access to the hot tubs and the main pool is by three wooden steps up, a short platform providing access to the tub and then three steps down to the pool. After using the pool, the passenger was descending the steps situated on the port side of the pool. As he reached the bottom step he extended his right foot and placed it on the wet deck. His foot slipped out from under him causing him to fall backwards to the deck. The steps are constructed of hardwood. The deck is of steel construction, covered by an API non-slip surface.

On April 17, 2012, a female passenger on the STAR PRINCESS fell and broke her right hip while line dancing. The accident occurred on deck 7 aft. Situated at this location, and spanning the width of the ship, is the Vista lounge. This is a small theater at the aft of which is a small stage area. At the time of the accident, the wooden stage itself was lowered to be level with the surrounding deck. There was a line dancing instructor teaching a group of approximately 100 passengers and the injured passenger was participating in the class. As she changed direction in time with the rest of the class, she struck her right rear calf with her left foot. This caused her to lose balance and fall to the deck, landing on her hip. The fall resulted in a fracture of the femur and pelvis.

On April 14, 2012, U.S. Coast Guard Sector Honolulu received a marine casualty report from the Bermuda flagged cruise ship STAR PRINCESS, IMO # 9192363. A female passenger, a US citizen, missed a step in the dining room, fell, and broke her left humerus while the vessel was transiting from San Francisco, California to Nawiliwili, Hawaii at position 34-58N, 129-38W. Weather at the time of the incident was reported as 13-17 mph winds out of the Northwest with good visibility and clear skies.

On April 13, 2012, the Sector San Francisco Command Center (SCC) received notification from the M/V STAR PRINCESS that a passenger tripped onboard the vessel and fractured her left arm. At the time of the incident, the STAR PRINCESS was transiting outbound towards the Golden Gate Bridge (GGB). The patient was treated onboard the STAR PRINCESS and remained onboard for the duration of the voyage. The investigation revealed that the passenger tripped over a raised grating on the open deck, and there was no crew involvement. No Enforcement Action (EA) recommended.

On 31 March 2012 at approximately 1818 local time, the Sector San Francisco's Command Center (SCC) received notification from the Chief Mate on the M/V STAR PRINCESS that a female had sustained a pelvic fracture. The incident occurred 750 Nautical Miles (NM) SW of San Francisco as the vessel was transiting to Ensenada, Mexico from Honolulu, Hawaii. Sector San Francisco was notified of the incident due to the injured party (US Citizen) residing in its Area of Responsibility (AOR). The injured passenger will remain on board until the STAR PRINCESS arrives in San Francisco on approximately April 3, 2012. On April 16, 2012, the Investigating Officer (IO) received a Medical Report from the Senior Doctor on board the STAR PRINCIESS at the time of the incident. It was reported that the injured party sustained the injury when she tripped and fell while entering the Theater. No indications of wrong doing were present. Drug and alcohol testing was not administered as no credentialed Mariners were directly involved with this injury. No Enforcement Action (EA) is recommended.

On March 21, 2011 at approximately 1210 the passenger vessel STAR PRINCESS had a fire onboard that resulted from a battery discharge test. During test of transitional power battery #81 exploded due to short circuit and as a consequence caught fire. Fire immediately extinguished by crew.
